다. 한 근 동 연 마우 에 미 지 (PM2.5)
과 우울행동 (depressive-like behavior)과 보고하 다. 하지만
간 상 한 연 통한 역학 거 거 없 실 다. 본
연 미 지 과 우울 생과 규 도시
트 하여 하 행 었다.
연 : 본 연 연 간 (2002 2010 지) 울시
같 에 거주하 우울 삽 진단 없 15 에
79 지 27,270 집단 상 행 었다. 미
지 지 3개 특 지 (district-specific
exposure index) 사 하 다. 미 지 지 우울 생
사 개 에 재 란 에
해 보 한 귀 통해 었다. 본 연 한 당뇨병,
심 계 질 , 만 폐쇄 폐질 과 같 만 질 여 에 라
행하여 질 지닌 에 미 지 건강
향 달라지 지 여 평가하 다.
결과: 간 (2008 1월 1 2010 12월 31 지) 우울
생 험 미 지 가함에 라 가하 다. 2007

연간 미 지 도 10 µg/m3 가 당 험비 (hazard ratio) 1.44
(95% 신뢰 간: 1.17, 1.78) 었고, 2007 2010 지 평균 미
지 도 10 µg/m3 가 당 험비 1.59 (95% 신뢰 간: 1.02,
2.49) 었다. 사건 (event) 나 단 (censor) 생하 지 12개월
미 지 도 동평균 10 µg/m3 가하 험비 1.47
(95% 신뢰 간: 1.14, 1.90) 었다. 질 여 에 라 하
미 지 과 우울 생과 질
보 하고 집단에 그 지 않 집단에 보다 게 나타났다.
결 : 본 연 집단에 미 지 과 우울 생
과 연 계 하 다. 미 지
가할 우울 생 험 가 었 질 여 에 라
하여 하 러한 질 보 하고 에
게 나타났다.

연 거 행 가 없다. 한 연 지 우울
상 평가 도 하거나 행 료 하 , 청 료
했 라도 항우울 (antidepressant) 사 보 등 하지 않아
우울 에 한 상 진단과 평가하 에 한계가 었다.

찰연 계 가 근거 다고 평가 트
연 계 통해 하도 하겠다. 한 당뇨병, 심 계 질 , 만
폐쇄 폐질 같 만 질 여 에 라 러한 향

2. 연
2.1. 료 집 주 변
2.1.1. 연 상 연 계
본 연 우리나라 건강보험 진 1,025,340 비
하여 연 공개한 료 건강보험공단 본 트DB (The
National Health Insurance Service-National Sample Cohort, NHIS-NSC)
하여 행하 다 (16). 건강보험공단 본 트DB 우리나라 체 건
강보험가 료 여 별 (남/여), 연 (0 , 1~79
지 5 단 , 80 상), 득 (지역 직 가 각 20 ,
료 여 0 )에 라 1,476개 하고 각 에
체 집단에 비 에 맞 만 (random
sampling)하여 한 료 다. 본 료 사망 등 한 연도별
격 상실 연 탈락 고 연도별 집단 신생아 본틀에 신생아
신규 가 어 2002 1월 1 2010 12월 31 지 매 약
100만 규 가 지 도 계 었다. 건강보험공단 본 트DB
별, 득 , 시 주 , 상병 (International Statistical
Classification of Diseases and Related Health Problems, 10th revision; ICD-10),

처 약 시 , 료 비 시 등 원과 래 료
보 포함하고 다. 한 본 료 격 상 연도별 1차
건강검진 생애 건강진단 검 에 한 건강검진 보 포함하
고 다. 그런 연 들 득 애 지닌 에 고 득
애 지니지 않 에 보다 러한 건강검진 검 낮
보고하고 어 (17,18) 건강검진 검 료가 우리나라 체
집단에 한 지닌다고 할 없다.
2002 트에 포함 1,025,340 2007 연 15 에

지 연 간 한 라도 울에 거주하 사람 234,073
었다. 연 간 울 같 에 주 지하 사람
73,359 었고 2002 1월 1 2007 12월 31 사
질병 10 (ICD-10)에 우울 삽 진단 없
사람 69,999 었다. 마지막 2002 1월 1 2007 12월
31 사 건강검진 27,270 에 해 본 행
하 다 (Figure 1).
본 연 주민등 , 양 , key에 해 체변
사 하 고, 연 , 주 , 득 , 애 도, 애 에 어 그
룹 시행하 , 학 등 민감상병 마 킹하여 비식별 한
후 연 공개 료 하여 행 었 울 학 병원 연

2.1.2. 우울
본 연 상병 상 우울 에피 드 (ICD-10 드: F32.x)가 택
닌 재 억 (selective serotonin reuptake inhibitor), 삼 계
항우울 (tricyclic antidepressant), 아민산 해 (monoamine
oxidase inhibitor), 닌- 에피 프린 재 억 (serotonin-
norepinephrine reuptake inhibitor), 타 항우울 처 함께
우울 하 다. 본 연 에 2008 1월 1 2010 12월
31 지 간 생한 사건 (event)
귀 한 행하 다 (Figure 2).
민감도 에 항우울 처 보 하지 않고 1) 상병 드
F32.x, 2) 상병 드 F32.x, F33.x (recurrent depressive disorder), F34.1
(dysthymia), F41.2 (mixed anxiety and depressive disorder) 사건
하여 행하 다.

2.1.3. 미 지 평가
한민 도 울 9.96 km2에 47.00 km2 (평균 24.21 km2)
25개 어 다. 우리나라에 미 지 2007
울시 내 27개 25개 에 한 행 었다 (
, 에 각각 2개 재). 본 연 에 울시보건
경연 원에 득한 2007 2010 지 울지역 27개
에 한 미 지 료 하 다.
본 연 에 건강보험 본 트DB 비식별 해 시
주 보 공하므 미 지 평가하 해
시 별 지 (district-specific exposure index) 다 과 같 생 하
여 에 하 다. 1) 2007 연평균 미 지 도 (
간 1 ), 2) 2007 2010 지 미 지 도 평균 (
간 1 간 3 ), 3) 2007 2010 사 사건
나 단 지 12개월 미 지 도 동평균 지
행하 다. 특 미 지 도 12개월 동평균 시간 변동
(time-varying exposure) 지 2007 1월 2007 12월 지
평균, 2007 2월 2008 1월 지 평균, … , 2009 12월
2010 11월 지 평균, 2010 1월 2010 12월 지 평균
계열 루어지 시간 귀 (time-dependent Cox
regression analysis) 통해 우울 과 하 다 (Figure 2).

러한 지 울시 내 27개 에 얻 24시간 료
평균 계산한 후 통해 계산하 2개 가
에 해 평균값 지 할당하
다. 각 에 미 지 경 프 에
랐 량 도 (gravimetric method)에 라 행 었다.

2.2. 통계
2.2.1. 미 지 과 우울 생
본 연 에 미 지 지 우울 생 사 별
과 나 (15 에 17 사 5 단 ) 각 에 신
험 (baseline hazard) 링하 해 별과 5 단 나 그룹 변
변 (stratification variable) 귀 하여 하
다. 간 2008 1월 1 사건 생, 사망, 끝 (2010
12월 31 ) 가 생하 시 지 달 단 계산하 다.
미 지 에 한 지 시 별 2007 연평균 미
지 도 2007 2010 지 미 지 도 평균 하
다. 한 시간 통해 2007 2010 사 사건
나 단 지 12개월 미 지 도 동평균에 우울 생
험도 하 다. 가 1개월, 3개월, 6개월, 9개월, 15개월,
18개월, 21개월, 24개월 동평균에 해 도 진행하여
간과 우울 생 사 사건 생 간 에
라 어떻게 달라지 지 평가하 다. 미 지 료 2007
울시 25개 에 해 가 했 므 주 에 한
가 한 간 12개월 미 지 동평균에 해 한 결과

시하 다. 15개월, 18개월, 21개월, 24개월 동평균 한 에
결 한 2006 료 가 하 다.
미 지 과 우울 생 사 비 여 평가하
해 플라 평 (spline smoothing method) 사 하여 험비 커브
(hazard ratio curve) 하 비 (nonparametric analysis)
행하 다 (19).
다 과 같 개 (individual-level) 재 란변 들에
해 보 었 , 별과 나 변 득 (10 ),
연 상태 (비 연, 과거 연, 재 연, 답), 주 여 ( 주하지 않
, 주 에 1 미만, 주 에 1~2 , 주 에 3~4 , 주 에 5
상, 답), 규 운동 여 (규 운동하지 않 , 주
에 1~2 , 주 에 3~4 , 주 에 5~6 , 거 매 , 답)에
해 보 하 다. 득 한 다 개 보 건강검
진 결과에 얻었다.
개 공변량 각 연 상 가 거주하 ,
15 상 비 , 경 사 경 만 도 (10 리 트 척
도), 당 사 복지시 거주 , 탈지 (20) 같 맥락 공변
량 (contextual covariate)에 해 도 보 었다. 보 통계청
가통계포 (Korean Statistical Information Service) 통해 득하 다.

건강검진 보 2002 2007 지 행 검진 보 하
간 여러 건강검진 검 연 상 에 해
간 (2008 2010 지)에 가 가 운 보 개
공변량 하 다. 개 보 변 건강검진 결과
2006 과 2007 에 행 건강검진 통해 집 었다
(73.2%). 맥락 공변량 2005 보만 가 했 15 상
별 비 과 2010 보만 가 했 별 탈지
에 2007 보가 었다. 보 변 상 결 에 해
결 에 한 주 생 하여 득 보 한 할 도
처리하 다.

2.2.2. 질 득 별
2002 1월 1 2007 12월 31 지 당뇨병, 고 압 포함한
심 계 질 , 만 폐쇄 폐질 과 같 만 질 여 에 라
동 한 하여 행하 다. 당뇨병 상병
ICD-10 E10.x E14.x 지 , 고 압 포함한 심 계 질 I10.x
I15.x 지, I20.x I25.x 지, I60.x I69.x 지 하 다.
만 쇄 폐질 J40 J44.x 지 하 다.
득 별 2007 격DB 득 10 보 하
여 득 (0~3 ), 간 득 (4~6 ), 고 득 (7~10 )
하여 동 한 행하 다.

2.2.2. 민감도
결과 강건 평가 해 다양한 민감도 행하 다. 첫
째 재 란 통 하 해 별 생 , 도
(도 /시가 ), 당 공공 등 맥락 공변량
가 보 하 다. 째 에 해 과 (random effect)
다 시간 (multilevel time-dependent Cox model)
각 연 상 거주 가 보 한 시간 통해
재 집락 (clustering) 란 보 하 다. 러한
에 별 미 지 차 신 내 미 지 변동 변
고 다. 째 결과가 특 질병 드 트에 해
향 지 않 하 해 다양한 사건 하여 결과
하 다. 째 비 림 (misclassification bias)
재 란 피하 해 연 간 매 (G30.x, G31.x, F00.x
F03 지) 진단 연 상 하고 행하 다. 다
째 주 에 었 2002 1월 1 2007 12월
31 지 우울 삽 진단 사람 포함하여 재 하 다. 여

사람 우울 사건 포함하여 진행하 다. 곱 째 본

여 건강검진 보가 사람뿐 아니라 없 사람에 해 도 득

여 째 주 보 변 상 결 에 해 결 에 한
주 (missing indicator category) 생 하 나 한 재 비 림
가 고 하 해 사 (complete case analysis) 행하
다. 아 째 진단 신뢰 에 한 평가 해 우울 진단 항
우울 처 신건강 학과에 행 경우만 사건 재 하
여 시행하 다. 마지막 차 경과학원에
득한 미 지, 산 질 , 아 산가 , 도 하여
료 한 행하 다. 에 2002 2010
지 동 시 에 거주했 2002 2005 사 건
강검진 고 우울 진단 없 15~79
115,954 상 었고, 시 사 질 고 하여 시
에 해 과 주고 12개월 경 해 도 동평균
지 시간 귀 하여 하 다.
든 SAS 통계 프 그램 ( 9.4, SAS Institute Inc., Cary, NC,
USA)과 R 통계 프 그램 ( 3.1.0, R Foundation for Statistical
Computing, Vienna, Austria) 하 p-value < 0.05 한 것
하 다.

3. 결과
3.1. 연 상 특징
연 간 울 같 에 거주하 85,758 건강검진 보
가 연 15~79 2002 2007 사 우울
진단 없 27,270 본 에 포함 었다 (Figure 1). 본 연
료에 포함 2007 울지역 거주 비 했 본 에 남
, 40~69 상 가 상 많 포함 었다 (Table 1). 연 에
포함 27,270 사 학 특징 Table 2에 시 었다.
상 54% 남 고, 다 40~59 다. 상 다 가
상 득 가지고 었고 담 피우지 않았
주 하지 않았다. 한 가량 규 운동 하지 않았다.

4. 고찰
4.1. 연 결과 약 결과 비
본 건강보험 청 료 트 한 에 미 지

4.3. 결과 생 학
미 지 도 (respiratory tract)에 염 과 야 할
신 염 과 염 매개체 (inflammatory mediator) 신
어질 알 다 (22). 러한 염 매개체
신경계 내피 포 사 체 결합하여 체 시
고 역체계 통해 포 (cell junction) 신경단 (neural
protein)에 한 가 항체 (autoantibody) 생 야 할 다 (23,24).
러한 과 통해 생 항체 액-뇌 벽 (blood-brain barrier)
란시 고 신경염 (neuroinflammation), 신경계 산 상 (brain
oxidative stress), 신경 학 변 (neurochemical change) 킬
다. 뿐만 아니라 미 지가 폐 직 (lung tissue compartment) 통
과하여 신 계 들어간 후 뇌에 도달하여 신경계 염 산
상 킬 가 보고 도 하 다 (12,25).
한편 우울 생과 하여 염 들 신경 학 변 통해
우울 생에 한 역할 한다 거들 다 (14,26). 우울
주 당뇨병, 심 계 질 , 만 폐쇄 폐질 과 같 만 염 과
질 과 동 알 (27), 우울 가진 에
신 하 염 매개체 도가 보고 다 (28). C

미 지에 한 낮 항 , 항상 지 가진 사람들에
그 지 않 사람들에 함 보고한 다 (31). 하지만 러한
거들에도 하고 본 연 에 찰 생 학 에
해 직 탐 한 연 한 편 향후 본
에 해 보다 직 탐 하 연 가 가 필 하다고 하
겠다.

4.4. 연 상에 한계
본 연 연 상에 해 가지 한계 지니고 다. 본 연
605 km2 에 천만 상 하나 도시에 해
행 었 연 결과 (generalizability)에 한계 볼
다. 그러나 본 연 상 , 에 상 동질
다 도시에 행 었 염
역학연 들 할 없었 집단 (population stratification),
사 차 에 보 지 않 란 (residual confounding)
다 강 다.
본 연 에 상 많 연 상 가 고 득 었
득 (underrepresent) 었 본 료에 2008

다. 한 득 에 건강보험 건강검진 검

본 연 상 에 개 공변량 5.3%가 결 었다. 본 연

4.5. 미 지 평가에 한계
본 연 에 미 지 평가에 역시 지 다.
염 역학연 에 고 니 링 (fixed-site pollution
monitoring) 료 하여 평가하 것 리 사 근
나 (32,33), 평가하 것 염 개
에 해 비차별 (non-differential misclassification) 야 하여
가 동시 것 보고 고 다 (34). 하지만 러한
한계에도 하고 본 연 에 찰 었다.
본 연 상 , 동질 에도 하고

4.6. 우울 평가에 한계
우울 평가에 한계 역시 재한다. 본 연 에 한 료
연 공개 하 나 본 청 료 가공한 료
진단 에 해 평가 지 않았다. 라 본 연 에 결과지
에 한 가 해 상병 뿐 아니라 항우울 처
보 함께 하여 우울 했 뿐 아니라 F32.x 상병 만
한 경우, F33.x (recurrent depressive disorder), F34.1 (dysthymia), F41.2
(mixed anxiety and depressive disorder) 포함하여 한
경우 등 다양한 에 해 민감도 행하 다. 결과 주
에 사 한 안 사 에 게 다 지 않았 나
차 감쇠 (gradual attenuation) 보 (Table 10), 비
차별 에 할 가 다. 한 우울 진단 항우울
처 신건강 학과에 행 경우만 사건 재 하여
시행했 도 찰 었다.

5. 결
본 연 건강보험 청 료 트 하여 미 지
과 우울 생 사 에 한 거 시하 다. 특
미 지 한 우울 험 당뇨병, 심 계 질 , 만 폐쇄
폐질 등 만 질 에 게 나타났다. 하지만 본
연 에 미 지 도 많 진 미 지 연간 평균 도
나 계보건 에 가 드라 시한 연간 평균 도 (10 µg/m3)
보다 았다. 한 도- (concentration-response
relationship) 양 집단 과 미 지 차
해 지역마다 다 에 본 연 결과 다 지역 직
하 에 주 가 필 하다고 할 다. 향후 연
미 지 과 우울 생 사 다양한 집단과
지역에 평가할 필 가 다.

Abstract
Long-term fine particulate matter exposure and depression in
a health insurance data-based cohort

Introduction: Previous studies have associated short-term air pollution
exposure with depression. Although an animal study showed an association
between long-term exposure to particulate matter ≤2.5 µm (PM2.5) and
depression, epidemiological studies assessing the long-term association are
scarce. Therefore, we investigated the association between long-term PM2.5
exposure and depression.
Methods: A total of 27,270 subjects aged 15–79 years who maintained an
address within the same districts in Seoul, Korea throughout the entire study
period (between 2002 and 2010) and without a previous depression
diagnosis were analyzed. We used 3 district-specific exposure indices as
measures of long-term PM2.5 exposure. Cox proportional hazards models
adjusted for potential confounding factors measured at district and
individual levels were constructed. We further conducted stratified analyses
according to underlying chronic diseases such as diabetes mellitus,
cardiovascular disease, and chronic obstructive pulmonary disease.
Results: The risk of depression during the follow-up period (2008–2010)
increased with an increase of 10 µg/m3 in PM2.5 in 2007 (hazard ratio [HR]
= 1.44, 95% confidence interval [CI]: 1.17, 1.78), PM2.5 between 2007 and
2010 (HR = 1.59, 95% CI: 1.02, 2.49), and 12-month moving average of
PM2.5 until an event or censor (HR = 1.47, 95% CI: 1.14, 1.90). The
association between long-term PM2.5 exposure and depression was greater in

subjects with underlying chronic diseases than in subjects without these
diseases.
Conclusion: Long-term PM2.5 exposure increased the risk of depression
among the general population. Individuals with underlying chronic diseases
are more vulnerable to long-term PM2.5 exposure.
