    specified remedial education because he failed to complete an appraisal assignment within the

    PA Board of Crane Operators Allegheny County Kenneth R Schmidt, license no. LCO000814 of Bethel Park, Allegheny County, was ordered to pay a

    civil penalty of $1,000, because he operated a crane, offered himself for employment as an individual who

    may operate a crane or held himself out as a crane operator while unlicensed by the Board. (11/16/16)

    PA Board of Dentistry Philadelphia County Jim Cheng, license no. DS031471L, of Philadelphia, Philadelphia County, was indefinitely and actively

    suspended, to be immediately stayed in favor of probation; was ordered to pay a civil penalty of two

    thousand, five hundred dollars and pay the costs of investigation of one thousand, five hundred and

    seventy-four dollars; was ordered to cease and desist from allowing uncertified or unlicensed persons

    from performing any activity requiring a license or certificate to practice in the Commonwealth of

    Pennsylvania; had a public reprimand placed on his permanent disciplinary record with the Board; and

    shall attend and successfully complete at least ten hours of remedial education, because he knowingly

    permitted radiological procedures to be performed by personnel who have not passed an examination

    approved by the board, and engaged in unprofessional conduct by delegating to a person duties that the

    dentist knew, or had reason to know, the person was not competent to perform or not authorized to

    Regina Palumbo of Philadelphia, Philadelphia County, was ordered to pay a civil penalty of five hundred

    dollars and cease and desist from performing radiologic procedures in the Commonwealth of

    Pennsylvania unless and until the Board issues her the appropriate authorization to practice the profession,

    because she was performing radiologic procedures without first passing the radiologic procedure

    Wayne County Newton C. Mullins, Jr., license no. DS019687L, of Honesdale, Wayne County, was revoked based on felony violations and crimes of moral turpitude. (11/08/16)

    Out of State Theodore Ira Goldberg, PA license no. DS029782L, of Milford, Massachusetts, was reprimanded and issued a civil penalty of $2500 based on disciplinary action taken by another state. (11/08/16)

    Charles F. Hertzog, PA license no. DS024908L, of Northport, New York, was automatically suspended

    John Patrick Filutze, license no. DS029426L, of Painesville, Ohio, was ordered to pay a civil penalty of

    one thousand, five hundred dollars and had a public reprimand placed on his permanent disciplinary

    record with the Board, because he had a license to practice dentistry or dental hygiene or certificate for

    expanded function dental assisting revoked, suspended or had other disciplinary action imposed or

    consented to by the proper licensing authority of another state, territory or country or had an application

    for license refused, revoked or suspended by the proper licensing authority of another state, territory or

    Alfred Derwood Dennis, license no. DS026543L, of Willingboro, New Jersey, permanently and

    voluntarily surrendered his license because he was found guilty of a felony in violation of the laws of this

    PA Board of Professional Engineers, Land Surveyors and Geologists No disciplinary actions taken.

    PA Board of Funeral Directors Delaware County Theodore F. Hawkins, Jr., license nos. FD012105L and FS012105L, of Chester, Delaware County, was

    revoked and a $10,000 civil penalty imposed for practicing without a current license. (11/02/16)

    Catherine B. Laws Funeral Home, Inc., license no FR000349L, of Chester, Delaware County, was

    revoked and a $10,000 civil penalty imposed for practicing without a current license and violation of a

    lawful disciplinary order of the Board. (11/02/16).

    PA Board of Landscape Architects

    Allegheny County Mark McKenzie, an unlicensed individual, of Pittsburgh, Allegheny County, was ordered to pay a civil

    penalty of $500, costs of investigation of $345.10, and ordered to cease and desist from using the name

    "Landscape Architectural Services, Inc.", or any other name containing a derivative of the title "landscape

    architect", because he used a derivative of "landscape architect" implying that his business and/or he was

    engaged in the practice of landscape architecture while neither business nor person was registered and

    PA Board of Massage Therapy

    Allegheny County Dallas James Harris, an unlicensed massage therapist of Pittsburgh, Allegheny County, Ms. Judy L.

    Scarfone, L.M.T., license no. MSG002798 and Metamorphosis Salon and Day Spa Inc., an unlicensed

    facility in Pittsburgh, PA, were jointly ordered to pay a civil penalty of $4,000; Mr. Harris will be ordered

    to cease and desist from holding himself out as a massage therapist and both Ms. Scarfone and

    Metamorphosis Salon and Day Spa Inc. will be ordered to cease and desist from hiring unlicensed

    massage therapists, because Mr. Harris worked as a massage therapist without a license and Ms. Scarfone

    and Metamorphosis Salon and Day Spa Inc. both hired unlicensed massage therapists. Also,

    Metamorphosis Salon and Day Spa, Inc. operated as a massage facility without a proper license.

    Delaware County Christopher Ronald Timberlake, license no. MSG009039, of Upper Darby, Delaware County, had his

    license to practice as a Massage Therapist temporarily suspended, pending a hearing, on the grounds that

    his continued practice within the Commonwealth may be an immediate and clear danger to the public

    PA Board of Medicine Allegheny County Entezam Asim Sahovic, license no. MD430177, of Pittsburgh, Allegheny County, voluntarily and

    permanently surrendered his license because he is guilty of immoral or unprofessional conduct including a

    departure from and/or failure to conform to the ethical standards of the profession. (12/06/16)

    Aladdin Zafar Syed, license no. MD017769Y, of Presto, Allegheny County, was indefinitely suspended

    for no less than three years, with such suspension immediately stayed in favor of no less than three years

    of probation, because of his violation of a lawful order of the Board previously entered by the Board in a

    Dauphin County Joyce E Chohany, license no. YM003426L, of Harrisburg, Dauphin County, was indefinitely suspended,

    because she is unable to practice the profession with reasonable skill and safety to patients by reason of

    Elk County Tariq Mahmood, license no. MD037766L, last known of Ridgway, Elk County, was revoked based on

    disciplinary action by another state, failure to report same to the Board and convictions of numerous

    surgeon with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic,

    ordered to pay a civil penalty of $10,000 and prohibited from performing angioplasty and stenting

    procedures of the venous system for chronic cerebrospinal venous insufficiency or multiple sclerosis

    patients in Pennsylvania, because he had a license or other authorization to practice medicine disciplined

    no less than three years, effective July 25, 2014, with such suspension immediately stayed in favor of

    proper licensing authority of another state, territory, possession or country, or a branch of the Federal

    Secretary of the Commonwealth – Notaries Public Allegheny County Mark F. Ballog, notary commission I.D. No. 1006121, of Pittsburgh, Allegheny County, was suspended

    for a period of six months; with such suspension immediately stayed in favor of probation, assessed a

    civil penalty of $500 and $300 probation assessment and must successfully complete a notary practice and

    procedure course during his suspension period, because he failed to provide a copy of his notary registry,

    to complete the commissioning process by failing to take the oath of office or recording a surety bond but

    continued to perform notarial acts; to record his official signature, to be of good character, integrity, and

    ability; and to be familiar with the duties and responsibilities of a notary public. (11/28/16)

    Lackawanna County The Secretary of the Commonwealth accepted a Consent Agreement in which Thomas Gallagher of

    Mayfield, Lackawanna County, permanently and voluntarily surrendered his notary commission I.D. No.

    1055552, along with any other licenses, registrations certificates or permits authorizing him to practice as

    a notary because he failed to properly identify the individual appearing before him by knowingly allowing

    another person to pose as another; failed to maintain an accurate notary journal; failed to require personal

    appearance; and to be of good character, integrity, and ability; and to be familiar with the duties and

    responsibilities of a notary public. (11/28/16)

    Northampton County Charles E. Wynn, notary commission I.D. No. 1182781, of Easton, Northampton County, was assessed a

    civil penalty of $500 because he failed to maintain an accurate notary journal and failed to be familiar

    with the duties and responsibilities of a notary public. (12/15/16)

    Philadelphia County Patricia Edwards of Philadelphia, Philadelphia County, permanently and voluntarily surrendered her

    notary commission I.D. No. 1044792, along with any other licenses, registrations certificates or permits

    authorizing her to practice as a notary because she failed to know through personal knowledge or have

    satisfactory evidence that the person appearing before her was the party who executed the document; to

    maintain an accurate notary register; to be familiar with the duties and responsibilities of a notary public;

    and to be of good character, integrity, and ability. (11/28/16)

    PA Board of Nursing Allegheny County Becky I. Leist, license no. PN100439L, of Pittsburgh, Allegheny County, was automatically suspended for one year based on her misdemeanor conviction under the Drug Act. (03/29/16) David J. Kohnfelder, license no.     verdict, disposition in lieu of trial or an Accelerated Rehabilitative Disposition in the disposition of felony
