MEDICATIONS

Instruct patient to take her home medications at the right time, right dose, and right route and also emphasized the importance of taking them. Instruct patient to take the following:

• Ciprofloxacin 500mg 1 tab 2x a day• Metronidazole 500mg 1 cap 3x a day• Celecoxib 200 mg 1 cap 2x a day

• Omeprazole 20 mg/cap OD• Proninerv cap, 1 cap OD• Estamin 1 cap OD• Feronerv tab, 1 tab OD• Heraclene 1g/cap, 1 cap 3x a day

EXERCISE

Encourage patient to do mild exercises such as walking and to have an adequate sleep and rest. Instruct patient to avoid strenuous activities, avoid straining, and lifting heavy objects.

TREATMENT

Instruct to have an adequate rest and sleep to reduce fatigue. Encourage to do deep breathing exercise and teach how to divert his attention to reduce pain.

HYGIENE

Instruct to observe proper hygiene like taking a bath every day, brushing his teeth 3x a day, and washing hands before and after eating. Advise patient to maintain proper environmental sanitation. Instruct patient to perform colostomy care. Inform the client to report to a health care provider if there is presence of peristomal redness, skin breakdown and changes in stomal color. Educate the client and family regarding infection control precaution, including proper disposal of used pouched since these cannot be flushed down in a toilet.

OPD CHECK – UP

Instruct patient to attend to him scheduled OPD check-up on July 13, 1012 (Friday) at 2 pm for further evaluation of her condition. Inform client that it is important to return for check-up because the health care team will evaluate her condition and refer if there are other abnormalities assessed.

DIET

Encourage patient to drink 8 to 10 glasses of water per day to keep him hydrated. Instructed patient to eat 3x a day, avoid grilled foods, and not to skip meals. Instruct patient to avoid eating too much low fiber and fat rich food. Instruct patient to eat foods rich in carbohydrates for energy, proteins for muscle build up and skin repair, vitamins (vitamin C) such as citrus fruits, and minerals for fast wound healing.

SPIRITUAL

Encourage patient to always pray for fast recovery and attend the holy mass every Sunday or whenever he has time and to strengthen his faith in God.
