Vol. 23 | Issue 1 Winter 2016

In recent months, Milwaukee Catholic Home has received recognition for excellence in many areas of its operations. Thanks to our dedicated staff, volunteers, and friends, we have received a five-star rating from

the Centers for Medicare and Medicaid, recognition in U.S. News and World Report as one of the best nursing homes in the country, and a flawless clinical survey from the State of Wisconsin.

In this issue of Focus, we will talk a little more about what excellence means at Milwaukee Catholic Home, the goals we’re setting, and the new initiatives we’re introducing to ensure that we continue to set and exceed the standards for senior care and services in southeastern Wisconsin.

5-Star Excellence at Milwaukee Catholic Home

2 | Milwaukee Catholic Home

In our Strategic Plan, rolled out in

2014, our Board of Directors and

leadership team here at Milwaukee

Catholic Home challenged ourselves

to pursue new goals and ways of

thinking in order to continue our

100-year tradition of excellence in

senior care.

The first of our four strategic goals

is an important one: “Develop a

culture of performance excellence to

drive organizational and individual

success.” It sounds simple, right?

But, as our team is well aware,

performance excellence involves the

commitment of every staff person,

volunteer and partner in each of our

service areas.

There are a number of systems,

methods, programs, and books that

offer organizations ways to achieve

performance excellence. One such

book is The Advantage, by Patrick

Lencioni, which our leadership

team will be studying this year in an

effort to increase the tools we use

to achieve and measure excellence

in many areas of our operations.

But excellence goes well beyond

what is measurable. This hit

home for me when I saw photos

from our new Intergenerational

Education Program, which was

carried out in partnership with

Catholic East Elementary School

this fall. We featured these

photos in our last issue, and you

probably noticed the smiles, the

excitement of hands-on learning,

the connections being made.

As important as the data we

collect will be in showing the value

of intergenerational programming,

it can never be the whole story.

The real story is told through the

relationships that were formed

between students and older

adults and the ways they were

able to learn from each other.

In this issue, we’re shining a

light on just a few of the ways

that Milwaukee Catholic Home

exhibits excellence, in our programs, financial and clinical

performance, collaborations, and

creative thinking. As friends of the

organization, we hope you will be

part of one or more of these great

initiatives. As ever, we are grateful

for your involvement and generosity.

How do we measure excellence? An interview with Vice President of Clinical Integration Katie Grant

Katie Grant, Milwaukee Catholic Home’s Vice President of Clinical Integration, was brought on staff in 2015 to guide the implementation of our new electronic medical records system.  She will be spearheading efforts to develop new systems for QAPI (quality assurance and performance improvement).

What is your background and experience in health care?I am a bachelor’s prepared, registered nurse and have served in leadership positions for all 22 years of my nursing career.

What is your role at MCH and why was it created? My role was created to assist in the development of staff through support, training and improvement of operating systems and procedures. Part of my responsibility was to implement electronic medical records for MCH. I am also responsible for developing systems and processes around quality assurance and performance improvement (QAPI).

What is QAPI and why is it important at an organization like MCH?QAPI is important because it gives us the skills to solve problems and prevent their recurrence, allows us to seize opportunities to achieve new goals,

provides fulfillment for caregivers as they become active partners in performance improvement, and, above all, provides better care and improved quality of life for our residents.

How will we be working with other organizations in the future to benefit the senior community throughout Milwaukee? We are trying to develop partnerships with other faith-based facilities. We don’t have to recreate the wheel! We can partner together, share resources and learn from each other. ■

State of WI recognizes MCH for clinical excellenceWe are proud to announce that MCH has been recognized for its excellence in healthcare with a deficiency-free clinical survey from the State of Wisconsin. This rare achievement is a testament to the strength of our team and our ongoing commitment to the highest level of quality and to always making the needs of our residents our top priority. Congratulations and thank you to all of our staff for your hard work, dedication, and compassionate care for the older adults who call MCH home! ■

New partnership with Alverno supports residents’ unique wellness goalsThis spring semester, Milwaukee Catholic Home is welcoming a group of graduating nursing students in a new partnership with Alverno College.

Each visiting student is paired with a resident from either The Residence or Assisted Living to serve in the role of “nurse navigator.” In this role, the nursing student guides the resident through the often complicated world of geriatric healthcare, managing existing health challenges and identifying long-term wellness goals.

The philosophy of the Alverno College nursing program, which focuses on self-advocacy and whole-person wellness, is a perfect fit with Milwaukee Catholic Home’s focus on residents’ independence and ability to direct their own care.

The one-on-one relationship between nursing student and resident is an especially important part of the program. Each student will meet with his or her resident partner weekly for the entire semester, developing a deeper understanding of that person’s individual needs, barriers, and strengths.

“For the residents, the benefit is awareness of self-monitoring their health,” said Amy O’Connor, Director of Resident Life at Milwaukee Catholic Home. “They’ll bring up lots of things for the residents to think about on a more consistent basis to maintain a healthy lifestyle.” ■

John McGivern to feature Milwaukee Catholic Home in Around the Corner(cont’d from page 1)

John McGivern, the well-known Milwaukee actor and comedian, paid MCH a visit a few months ago to film a segment for his PBS TV series Around the Corner. John, whose mother is a current resident of MCH, was a guest and entertainer at the closing celebration of our 100 Year Anniversary on October 1, 2015. John enjoyed spending some time on our campus and speaking to residents and staff about MCH’s history and presence on Milwaukee’s East Side.

The episode will air on PBS Thursday, March 24th at 7:00 pm on Channel 10.1, but friends of MCH have the opportunity for a special sneak preview event on Tuesday, March 22nd. All are invited to join us for cocktails at 6:00 pm, followed by a special screening of the episode at 7:00 pm and a question-and-answer session with John and his producer, Lois Maurer. RSVPs are required, as seating is limited. Please contact Anne Catalane at (414) 220-3250 or [email protected]. ■
