Residency Program Information The University of Virginia Health System (UVaHS) has 6 ASHP-accredited residency programs: PGY1 Pharmacy (established 1991), PGY2-Critical Care (established 2002), PGY2-Drug Information (established 1998), PGY2-Health System Pharmacy Administration (established 2010), PGY2-Oncology (established 2011), and PGY2-Pharmacy Informatics (established 2011). The accreditation survey for the PGY1/PGY2 Administration/MS Degree residency took place in May 2013 and was successfully approved for accreditation in August 2013. In 2013- 2014, our first PGY2 Solid Organ Transplant Resident completed residency training. The accreditation status of the program is currently “Candidate,” and the on-site accreditation survey is scheduled for October 2014. Resident Bios Michael Bedenbaugh, PharmD, MBA PGY2-Health System Pharmacy Administration Michael, a 2012 graduate of the South Carolina College of Pharmacy, completed the remainder of the PGY1/PGY2-Health System Pharmacy Administration/MS Residency program during the 2013-2014 year. His quality improvement project evaluated organizational compliance to the Pyxis override policy. His longitudinal project assessed the impact of various cost control strategies on the overall pharmaceutical expenditures at the organization. Michael looks forward to remaining at the University of Virginia Health System as the Manager of Pharmacy Supply Chain and Finance. Kelly Gaertner PGY2-Oncology Kelly, a 2011 graduate of Duquesne University, completed her PGY2-Oncology residency during the 2013-2014 year. Residency Program Information

The University of Virginia Health System (UVaHS)

has 6 ASHP-accredited residency programs: PGY1

Pharmacy (established 1991), PGY2-Critical Care

(established 2002), PGY2-Drug Information

(established 1998), PGY2-Health System Pharmacy

Administration (established 2010), PGY2-Oncology

(established 2011), and PGY2-Pharmacy Informatics

(established 2011). The accreditation survey for the

PGY1/PGY2 Administration/MS Degree residency

took place in May 2013 and was successfully

approved for accreditation in August 2013. In 2013-

2014, our first PGY2 Solid Organ Transplant

Resident completed residency training. The

accreditation status of the program is currently

“Candidate,” and the on-site accreditation survey is

scheduled for October 2014.

Resident Bios

Michael Bedenbaugh, PharmD, MBA

PGY2-Health System Pharmacy Administration

Michael, a 2012 graduate of the South Carolina

College of Pharmacy, completed the remainder of

the PGY1/PGY2-Health System Pharmacy

Administration/MS Residency program during the

2013-2014 year. His quality improvement project

evaluated organizational compliance to the Pyxis

override policy. His longitudinal project assessed the

impact of various cost control strategies on the

overall pharmaceutical expenditures at the

organization. Michael looks forward to remaining at

the University of Virginia Health System as the

Manager of Pharmacy Supply Chain and Finance.

Kelly Gaertner

PGY2-Oncology

Kelly, a 2011 graduate of Duquesne University,

completed her PGY2-Oncology residency during the

2013-2014 year. She has previously completed both

a PGY1 residency at Riverside Methodist Hospital

and a community practice residency at Allegheny

General Hospital. Within hematology/oncology, her

interests include primarily solid tumors and

supportive care, but she has also grown to enjoy

malignant hematology. Kelly completed a quality

project assessing sepsis alerts in the neutropenic

population; and her longitudinal research project

evaluated guideline-driven supplemental Gram-

positive antibiotic coverage for febrile neutropenia.

J. Nate Hedrick

PGY2-Critical Care Nate, a 2012 graduate of the University of North

Carolina Eshelman School of Pharmacy, completed

his PGY2-Critical Care residency during the 2013-

2014 year. His medication use evaluation assessed

the appropriateness of alteplase use during cardiac

arrest events. His longitudinal research project

evaluated appropriateness of antimicrobial selection

upon referring hospital transfer to the medical

intensive care unit. Nate has accepted a position as a

Clinical Pharmacy Specialist on the University of

Virginia Emergency Medicine team. Prior to

transitioning into this role, Nate will enjoy some

time off partaking in his favorite hobbies: reading,

cooking, and woodworking.

Michelle Hurrell Bradley

PGY2-Solid Organ Transplant

Michelle, a 2011 graduate of the Mercer University

College of Pharmacy and Health Sciences, was

UVaHS’ first solid organ transplant pharmacy

resident. She completed her PGY1-Pharmacy

residency at the

University of Alabama at

Birmingham (UAB)

Hospital. Her current

areas of interest include

solid organ transplant

(SOT), specifically liver

transplant, and infectious

diseases within the SOT

population. Michelle’s

longitudinal residency

project evaluated

adherence to and

outcomes associated with

the newly implemented

risk stratified immunosuppression regimen in renal

transplant patients at UVaHS. Her medication use

evaluation evaluated the use of anti-

thymocyte globulin within the institution. In her free

time, Michelle enjoys playing with her two cats and

seeing movies. Although she is sad to leave,

Michelle is excited about beginning her career as the

liver transplantation pharmacy specialist at THE

Ohio State University Medical Center in Columbus,

Ohio, where she will take everything she learned at

UVA to benefit her future liver transplant recipients.

Terran Rice

PGY2-Informatics

Terran, a 2011 graduate of University of the Pacific

Thomas J. Long School of Pharmacy and Health

Sciences, completed his PGY2-informatics residency

this year and will continue his career as a senior

pharmacy applications analyst at the University of

Virginia Health System. His current areas of interest

include data analysis, data warehousing, and the

application of technology to allow more efficient

workflow. Terran’s quality improvement project

evaluated the impact of alternative medication alerts

for medications on shortage. His longitudinal

research project evaluated TPN compounding

workflow to facilitate network server

implementation. Outside of pharmacy, Terran’s

hobbies include cooking and computers. He is

excited about continuing his career here at UVA.

Emily Prabhu

Medication Use Policy Fellow

Dr. Prabhu is a 2011 graduate of Butler University in

Indianapolis, Indiana. She completed her PGY1

residency at Saint Joseph

Regional Medical Center in

South Bend, Indiana and her

PGY2 Health System

Pharmacy Administration

Residency at UVaHS. This

year she completed a

Fellowship in Medication

Use Policy at UVaHS. Her

interests outside of work

include traveling, horseback

riding, and cheering on the

Butler basketball team. Go

staying at the University of

Virginia as a Medication Use

Policy Coordinator.

Thomas DiPiro

PGY1-Pharmacy/PGY2-Health System

Pharmacy Administration

Tom is a graduate of the South Carolina College of

Pharmacy at the Medical University of South

Carolina campus, where he earned his PharmD in

2013. He is also a graduate of The Citadel, where he

completed his MBA in 2012. His current

professional interests include pediatrics and

emergency medicine. During his PGY1 residency

year, he completed a quality improvement project,

which assessed adherence to the immune globulin

(IVIG) guideline at the University of Virginia Health

System. His research project evaluated fall risk

associated with antihypertensive use at UVAHS.

Tom is looking forward to completing his PGY2-

Health System Pharmacy Administration and

Masters in Health System Pharmacy Administration

from Virginia Commonwealth University School of

Pharmacy during the 2014-2015 year. Outside of the

hospital he follows college football, and roots for the

Georgia Bulldogs on Saturdays.

Amanda Liszewski

PGY1-Pharmacy

Amanda is a graduate of the Duquesne University

Mylan School of Pharmacy in Pittsburgh,

Pennsylvania, where she earned her PharmD degree

in 2013. Her current areas of interest are critical care

and solid organ transplant. Amanda’s internal

quality project assessed the adherence to insulin

protocols for treatment of patients with diabetic

ketoacidosis and hyperosmolar hyperglycemic state

in the emergency department. Her longitudinal

research project evaluated differences in blood

glucose control and other clinical outcomes between

patients with septic shock who received

hydrocortisone as intermittent bolus doses and those

who received hydrocortisone as a continuous

infusion. Amanda is looking forward to staying on at

UVA as a PGY2-Critical Care pharmacy resident.

Outside of work, her interests include music, running

and Pittsburgh sports.

Lia Lynch

PGY1-Pharmacy

Lia is a graduate of the University of North Carolina

Eshelman School of Pharmacy in Chapel Hill, North

Carolina, where she earned her PharmD in 2013. Her

professional interests are hematology/oncology and

infectious disease. During her PGY1 year, Lia

completed a quality improvement project, which

evaluatated the use of brentuximab, carfilzomib, and

nab-paclitaxel within the University of Virginia

Health System. Her longitudinal research project

assessed unfractionated heparin dosing in obese

patients. Lia is very excited to be continuing her

residency training as one of two PGY2-Oncology

residents at UVA. Outside of her time spent in the

hospital, Lia enjoys spending time with her husband

cheering on her favorite team, the NC State

Wolfpack, wine tasting in the Virginia countryside,

and baking.

Laura Mincemoyer

PGY1-Pharmacy

Laura is a graduate of the University of

PittsburghSchool of Pharmacy in Pittsburgh,

Pennsylvania, where she earned her PharmD degree

in 2013. Her current areas of interest include solid

organ transplantation and critical care. Laura’s

medication use evaluation assessed appropriate

utilization of the UVaHS nurse driven heparin

nomogram for treatment of patients diagnosed with

acute coronary syndromes and arterial or venous

thromboembolism in the Emergency Department.

Her longitudinal research project evaluated the

efficacy of the institution’s Labor and Delivery

insulin infusion protocol and associated rates of

neonatal hypoglycemia. Laura is looking forward to

moving further South as she has accepted a position

at the University of North Carolina as clinical

pharmacy generalist. In her spare time, Dr.

Mincemoyer enjoys reading, baking, and cheering on

her Penn State Nittany Lion football team!

Amy Morris

PGY1-Pharmacy

Amy is a 2013 graduate of the University of Georgia

College of Pharmacy in Athens, Georgia. Her current

areas of interest include oncology and infectious

disease in immunocompromised hosts. Amy’s

medication use evaluation assessed the use of the

new oral anticoagulant rivaroxaban (Xarelto) in

oncology patients. Her research project evaluated the

relationship between immune-mediated dermatitis

and outcomes in patients treated with ipilimumab

(Yervoy) for metastatic melanoma. Amy is excited to

continuing her residency training at the UVaHS as a

PGY2-Oncology resident. Her interests outside of

pharmacy include horseback riding, running, and

trying all the amazing restaurants that Charlottesville

has to offer.

Karen Partlow

PGY1-Pharmacy

Karen is happy to be back in Charlottesville where

she received her undergraduate degrees in

Biochemistry and Sociology from the University of

Virginia. After spending five years as a

reimbursement consultant, she returned to school and

earned her PharmD from the University of Maryland.

Her professional interests are critical care and

infectious disease. Karen completed a medication

use evaluation to determine the proportion of septic

surgical patients identified by a systemic

inflammatory response syndrome (SIRS) best

practice alert. Her longitudinal research project

examined etiologies of 30-day medication-related

readmissions at UVaHS. Now that she has completed

PGY1, Karen is excited to stay on at UVaHS as a

PGY2-Critical Care resident. Outside of work she

enjoys watching sports (Go Nats and Caps!),

cooking, and chasing chipmunks with her beagle,

Hootie.

Hawai'i, The Daniel K. Inouye College of Pharmacy

with a practice site at Straub Hospital, Honolulu,

Hawai'i.

Upon completion of her PGY2 critical care program

at University Hospital Case Medical Center, Jessica

S. Traeger (PP ’13) will continue her career there as

a clinical pharmacist.

Hasan Kazmi (PP ’13) completed his PGY2 in

cardiology at The Ohio State University Wexner

Medical Center and has accepted a position at

Novant Health Prince William Medical Center in

Manassas, VA.

News and Announcements

Amanda Williams (PP ’10, CC ’11) and her

husband, Matt, announce the birth of their son,

Cooper Thomas Williams, on July 8, 2014. Cooper

weighed 7 lbs., 3 oz and was 19 inches long.

Tina G. Hardison (CC ’08) and her husband Lewis,

announce the birth of their son, Lewis Hardison III

on January 23, 2014. He weighed 8lbs, 4.9 oz and

was 20 inches long.

Melanie (Brooke) Bernhardt (CC ’05) and her

husband, Jason, announced the birth of her daughter,

Greta Anne, on October 7, 2013. She weighed 8 lbs.,

1 oz. and was 19.5 inches.

Trisha Branan (CC ‘08) and her husband, Jep,

announce the birth of their daughter, Harper Grace,

on September 26, 2013. She weighed 7 lbs., 14 oz

and was 20 inches long. She was welcomed home

by her big brother Ethan.

Kelly Branham (PP '08) married Tyler McAllister

on September 7, 2013 in Penhook, VA. The happy

couple is in the process of building their first house.

Kelly is an Emergency Medicine Clinical Pharmacy

Specialist at Carillion in Roanoke, VA.

Celeste Caulder (PP ‘05) and her husband, Will,

announce the birth of their son, Connor McCoy, on

August 13, 2013. She weighed 7 lbs., 10 oz and was

19 ¾ inches long.

Brenda Clark (CC ‘10) and her husband, Eric,

announce the birth of their daughter, Teagan

Marissa Clark, on July 23, 2013. She weighed 8 lbs.,

1 oz. and was 21 inches long. She was welcomed

home by her big brother Landon.

Carrie Hall Moon (PP ‘12) and her husband

announce the birth of their son, Ethan, on July 2,

2013. He weighed 8 lbs., 10 oz. and was 20.1

inches.

Helen Choi (PP ’06), and her husband announce the

birth of their daughter, Audrey, on February 7, 2013.

Bethany (Strickland) Delk (CC ‘07) and her

husband, Christopher, announce the birth of their

daughter, Lenora Bay Delk, on July 29, 2013. She

weighed 6 lbs., 13.5 oz. She was welcomed home by

big brother, Sullivan, 2 yrs.

Professional Announcements

Denise (Kockler) Lowe (DI ‘01) and the VCU

Health System PGY1-Pharmacy Program were

awarded the 2013 ASHP Residency Excellence

Program Award.

Kyle Luedtke (PP ’12, CC ‘13) and Phillip

Mohorn (PP ’12, CC ‘13) were granted Board

Certification in Pharmacotherapy (BCPS), and

Nicole Watts (PP ’11, Onc ‘12) was granted Board

Certification in Oncology (BCOP) in 2013.

Scott Anderson (IT ‘12) will begin his second term

as the Virginia Health System Pharmacy (VSHP)

Region 1 President this year.

Katharine Kinsman (PP ’07) accepted a position as

a clinical pharmacy specialist at the University of

Washington Medical Center/Seattle Cancer Care

Alliance.

Melanie (Brooke) Bernhardt (CC ’05) was

promoted to Assistant Director of Clinical Services

and Outcomes at Texas Children’s Hospital.

Celeste Rudisill Caulder (PP ‘05) has been named

the International Director for the South Carolina

College of Pharmacy and the Coordinator for the

PGY2-Infectious Diseases Residency at the South

Carolina College of Pharmacy.

Ellie Elliott (PP ‘98) was named the Director of

Pharmacy for the Center for Behavioral Medicine in

Kansas City, MO and the Northwest Missouri

Psychiatric Rehabilitation Center in St. Joseph, MO.

Phillip Mohorn (PP ’12, CC ‘13) published a

Pharmacy Times article entitled ‘How Does the

Media Portray Pharmacists? Hero, Villain, or Most

Accessible Health Care Provider.’

Kristi (Peterson) Hofer (PP ‘98, DI ‘02) accepted a

new position at ASHP – Director of Scientific

Projects in Certification Development. In this role,

she is responsible for the development of review

course and recertification programs for pharmacy

specialty practice areas.

Bethany (Strickland) Delk (PP ’06, CC ‘07)

accepted a new position as Pharmacy Clinical

Coordinator, Internal Medicine at the University of

Virginia Health System in October 2013.

Trent Sutherland (PP ‘00) accepted a new position

as a clinical pharmacist at the VA Medical Center in

Martinsburg, WV.

Trisha Branan (CC ’08) accepted a new position as

Clinical Assistant Professor at the University of

Georgia College of Pharmacy.

Lori Dupree (PP ‘00) accepted a new position as a

Clinical Assistant Professor at the University of

Florida College of Pharmacy, Jacksonville Campus.

She will continue to serve as an internal medicine

practitioner at UF Health Jacksonville.

Denise (Kockler) Lowe (DI ‘01) was promoted

from Clinical Associate Professor to Clinical

Professor at Virginia Commonwealth University

(VCU) School of Pharmacy.

Christine Whong (PP ‘05, CC’06) accepted a

position as Medical Editor for Epocrates in San

Francisco, CA.

Teaching Accomplishments

We know that education is an important part of

many of our residency graduates’ careers. The

following awards have been received by our

graduates.

Marcia Brackbill (PP ‘98) was awarded the 2014

P2 Golden Apple/Teacher of the Year, and 2004,

2006 and 2012 Preceptor of the Year from the

Bernard J. Dunn School of Pharmacy at Shenandoah

University. In addition, she received the 2011, 2012,

and 2013 Winchester Medical Center PGY1

Resident Preceptor of the Year awards.

Trisha Branan (CC ’08) was awarded the 2011-

2012 Georgia Health Sciences/UGA College of

Pharmacy Residency Preceptor of the Year.

Celeste Rudisill Caulder (PP ‘05) received the

2013 Kappa Epsilon/Merck Vanguard Leadership

award in February 2013.

Nicole Watts (PP ’11, Onc ‘12) received the 2013-

2014 Preceptor of the Year award from VCU School

of Pharmacy.

Young Ran Lee (PP ’08) was awarded the 2012-

2013 Preceptor of the Year from Texas Tech

University Health Sciences Center School of

Pharmacy. He was also a member of the 2013-2014

P2 Teaching Team of the Year (Infectious Diseases)

at Texas Tech University Health Sciences Center

School of Pharmacy.

to share the accomplishments of the UVA Health

System Department of Pharmacy in FY14. You can

access the report here.

JOB OPENINGS

The Department of Pharmacy would love to have

former residents help grow our team! We currently

have the following positions available:

Clinical Pharmacist—Clinical Quality Response

Team

The department is expanding the clinical program

and is recruiting pharmacists for the Clinical Quality

response Team. This team will provide additional

clinical support to evenings and weekends with a set

three week rotation pattern working every third

weekend. The successful candidate will provide clin-

ical support including order optimization/perfection

through verification support, emergency response

events, clinical consults, drug information and real

time investigation of patient safety or team member

safety events. Applicants should possess a Doctor of

Pharmacy degree and be eligible for licensure in the

Commonwealth of Virginia. PGY1 residency is re-

quired, PGY2 residency in Critical Care or Internal

Medicine preferred or equivalent hospital experi-

ence.

Clinical Pharmacist—Hematology/Oncology Team

UVA Health System continues to expand clinical

pharmacy services within the Emily Couric Cancer

Center, stem cell transplant program and HOPE clin-

ics. Oncology clinical pharmacists divide their work

time between inpatient and outpatient clinical ser-

vices and infusion center. Clinical pharmacists mon-

itor patient drug therapy, actively participate in in-

terdisciplinary rounds, provide pharmacokinetic and

therapeutic consults, patient education, report ADRs,

perform MUEs and quality reviews, and assist with

precepting pharmacy students and residents. Mem-

bers of the heme/onc team are also engaged in the

development and optimization of Beacon Oncology

treatment plans in EPIC. Applicants should possess

a Doctor of Pharmacy degree and be eligible for li-

censure in the Commonwealth of Virginia. PGY2-

oncology residency training, with dispensing experi-

ence, is preferred or equivalent hospital experience

in the area of oncology.

Clinical Pharmacist—Pediatrics

The department is expanding the pediatrics team

with the clinical pharmacy program. Pediatric clini-

cal pharmacists rotate between clinical and opera-

tions-based functions in the pediatric pharmacy and

clean room. The successful candidate will join an

team of experience practitioners working under the

leadership of the pharmacy clinical coordinator for

the UVA Children’s Hospital. Team members moni-

tor drug therapy, participate in interdisciplinary

rounds, provide pharmacokinetic and therapeutic
