Section I: OVERVIEW

1.1 Description of Unit The Department of Otolaryngology is a full department within the Faculty of Medicine. It has both a clinical and academic mission in accordance with the following statement: It is the objective of the department to continue its academic and research efforts and to remain at the forefront of modern otolaryngology practice. The mission of the Department of Otolaryngology is to provide excellence in all aspects of otolaryngology - head and neck surgery by teaching, undertaking research and through exemplary clinical practice in the broad field covered by this discipline. 1. The educational component encompasses the teaching of McGill undergraduate medical students

and McGill otolaryngology residents. The department also accepts post-graduate fellows for subspecialty training. Responsibility in continuing medical education (CME) towards the teaching of allied health care professionals is deemed of equal importance.

2. The department participates in both clinical and basic research in the areas of head and neck oncology, vestibular physiology, rhinology, neurotology and laryngology.

3. An important mandate of the department is towards the provision of high quality clinical care in general otolaryngology and in the subspecialty areas of head and neck oncology, neurotology, laryngology, rhinology, facial plastic and reconstructive surgery and communicative disorders.

These goals will be achieved by: having available sufficient medical and support staff to maintain proper health care delivery to service

the hospital and community network of patients the recruitment of highly qualified individuals who can provide subspecialty expertise and stimulating

teaching to undergraduates, residents, fellows and other health care workers working closely with other medical specialists and allied health professionals to provide a multi-

disciplinary approach toward the best possible care for patients working closely with basic researchers to foster a stimulating academic atmosphere and active

participation as educators in the Faculty of Medicine of McGill University

1.4 Message from the Chair (Dr. S. Frenkiel) The Department of Otolaryngology is pleased to provide McGill University and the Faculty of Medicine its 2007-2008 Annual Report. The following pages will highlight both the individual and collective productivity of the Otolaryngology faculty and the Department. We view ourselves as primarily a clinical service, but nevertheless, provide a strong academic contribution to the university. During the past few years, our department has maintained a steady growth in both clinical and academic activities. This year, our productivity has become even stronger with respect to research performance and peer reviewed publications. This is largely to the credit of an active teaching staff that devotes considerable energy towards the academic process. On behalf of McGill University, I offer my compliments to our academic staff, many of who have achieved national and international recognition for their academic work. In the summer of 2007, I completed my tenure as President of the Canadian Society of Otolaryngology – Head and Neck Surgery. However, our McGill Department continued to play an active role in the affairs of our national society where many of our staff hold important administrative positions and where many of us present papers, symposia and workshops to the Canadian Otolaryngology community. I acknowledge the participation of our otolaryngology residents and select McGill medical students who have presented their research at this important national event. In recent years, our Department has achieved considerable success through staff retention and through careful recruitment. Since the start of my tenure, new professors who have joined our department include Drs. Sam Daniel, Mindy Black, Richard Payne, and Lily Ngyuen. Each of these individuals has contributed to the strength of the department and towards its academic performance. Dr. Sam Daniel was appointed as a clinician-scientist, representing a rather unique and rare position in Canadian Otolaryngology. Through a combination of departmental, university and national grant support, he successfully started the McGill Auditory Sciences Laboratory, which has now achieved international recognition. Dr. Mindy Black has become an important member of the Speech and Swallowing Unit situated at the Montreal General Hospital. Dr. Richard Payne completed a Head and Neck Oncology fellowship and has become prominent in the field of thyroid surgery. Dr. Lily Ngyuen, after completing her fellowship in Pediatric Otolaryngology, has now graduated as a McGill Faculty Scholar in Education. She will continue to play an important role as a medical educator for both undergraduate and post graduate training. We anticipate the arrival of two strong recruits who are each in the process of completing their two years of fellowship training. Dr. Alexander Mlynarek will be joining our department in 2009 with special expertise in micro-vascular surgery. Dr. Marc Tewfik is projected to join our department in the year 2010 to enhance the growing subspecialty of Skull Base Surgery. We look forward to them joining us. With respect to the anticipated move of our department within the McGill Superhospital project, the current plans will locate the Department of Otolaryngology at the Glen Site to be part of the development of the MUHC Head and Neck Oncology mission. Nevertheless, our department will maintain a presence

at the Mountain Site with respect to the growing skull base program as a multi-disciplinary effort with neurosurgery. In the past year, our departmental fundraising efforts have grown by leaps and bounds. I would like to compliment the work of Dr. Richard Payne and our active fundraising committee. This year, the Annual Head and Neck fundraiser grossed more than $500,000. This is more than double from the previous year and is largely to the credit of individuals who devoted their time generously and passionately towards helping our department. The money that is raised is used to enhance the important research and academic activities at McGill Otolaryngology. Our department is presently in the process of future planning that will enhance our academic mission. We anticipate the development of a number of Centers of Excellence that will incorporate both a clinical and academic mission. These Centers of Excellence will be developed in the areas of Head and Neck Surgery, Skull Base Surgery, Neurotology and Voice and Dysphasia. In parallel, Pediatric Otolaryngology will develop its own areas of expertise. It is also on the agenda of the Chair to propose to McGill University a name change for our department which will read as follows “Otolaryngology – Head and Neck Surgery”. This is in keeping with a trend that currently exists in North America. We are awaiting a similar Royal College decision on behalf of the Canadian Otolaryngology Specialty Committee before making this request to the University.

Section II: PAST YEAR’S ACTIVITIES

2.1 Highlights

The department held its 14th Annual McGill Head and Neck Fund Raising event on May 21, 2008. A record amount of more than $500,000 was raised for the department’s research and academic programs. This represents more than double the amount from the previous year. The co-chairs for this event were Dr. Richard Payne and Dr. Saul Frenkiel.

The department held its successful CME Course – 12th Annual Update in Otolaryngology at Mont

Tremblant on March 13-15th, 2008. Registrants came from across Canada, the United States and Great Britain. Course directors were Drs. Michael Hier, Jamie Rappaport and Karen Kost.

The department welcomed the following visiting professors:

Beirut Lebanon on January 24, 2008. The participants of Lebanon included Dr. Amine Haddad and his staff. This was organized by Dr. Mark Samaha.

Publications from our staff in peer-reviewed journals for the academic year 2007 were 48

articles. In addition, other publications appeared as textbook chapters and in electronic media format. (see Appendix IV)

Dr. Melvin Schloss stepped down as Director of Pediatric Otolaryngology after 32 years of service in this position.

Dr. Martin Black stepped down as Chief of Surgery, Jewish General Hospital after serving for 17

years in this position. Dr. Françoise Chagnon stepped down as Director of Professional Services (DPS) of the MUHC.

2.2 Teaching and Learning (Undergraduate and Graduate)

2.2.1 Undergraduate (Dr. Lily Nguyen – Director)

Our Department of Otolaryngology has a mission to educate medical students in the management of disorders of the ear, nose, and throat, as well as head and neck surgery. The faculty must be commended for their commitment to undergraduate teaching, which is accomplished on a voluntary basis across the four teaching hospital sites. The Department’s numerous involvements include lectures for the Introduction of Clinical Medicine (ICM) rotations, and supervision during the Practice of Medicine (POM) electives. The Department of Otolaryngology participated in the education of all medical students registered at McGill in the following courses. During the ICM-A (Professional Skills) rotation, students are taught the basics involved in a proper head and neck physical examination. The ICM-Surgery Sub-Specialty (Senior Clerkship) lectures include topics such as “Head and Neck Cancers” and “Head & Neck Emergencies”, whereas the ICM-C (Introduction to Clinical Medicine) rotation introduces students to the subjects of “Ears & Hearing Problems”, “Nose, Sinuses and Rhinorrhea”, “Neck Masses” and “Sore Throats”. Our department is enjoying an increasing popularity for elective rotations amongst medical students, with 30 students rotating through this last academic year. The feedback from students has been universally positive. Finally, a database of presentations geared towards medical students has been created and is available to all staff through their administrative secretary. In addition, it is available to all medical students at the undergraduate surgery portal at the Faculty of Medicine, McGill University.

2.2.2 Postgraduate ( Dr. John Manoukian – Director) This year our department will welcome 4 new incoming R1 residents to the program. Two of these residents came through CaRMS positions. They were our 1st and 2nd choices. They are Noah Sands and Yalon Dolev. Our third resident comes to us from the Saudi Mission – Saud Al-Romaih and the 4th resident comes from Kuwait – Faisal Al-Obaid. Moving from R1 (Core Surgery) to R2 are 4 residents Keith Richardson, Nathan Jewett, Michael Chater and Hani Al-Marzouki. The McGill Otolaryngology Residency Training program is very strong and healthy with a total of 19 Residents and three fellows, two in pediatrics (Hasan Al-Shemari followed by Fahad Al-Saab) and the other in JGH Head & Neck Program (Hadi Hakami). The Academic program consisted of the weekly Grand rounds, Hospital and Tumor board rounds, the Visiting Guest Speakers and Visiting Professors Program, the Academic weekly lectures and four Journal Clubs. The grand rounds and lecture series format is in constant change for the better as per residents’ request and RPC. These changes are coordinated by Dr Mark Samaha. Last year the quality of grand rounds significantly improved with participation of senior residents and staff alike, as well as several invited professorships. Grand Rounds this year will include some Can-MEDS type lectures e.g. an expert from the CMPA will give us a Grand Rounds on informed consent and medico-legal issues in Otolaryngology. Last year academic lecture series were very successful, judging from the feedback from the residents. Elements that were particularly noted by the residents are the organizational aspect, the structure in blocks focusing on the different subspecialties with exams immediately following each block, and the length of the lectures and quality of speakers. This year there will be extra CanMEDS topics including “informed consent, avoiding conflict situations, breaking bad news, dealing with difficult patients/situations, critical appraisal of the literature from a clinical standpoint, and presentation skills”. Some of these topics will be a repeat from last year’s experience, with the use of professional patient actors performed in the McGill Simulation center, which was very successful as per the residents and involved staff. These were coordinated by Dr Mindy Black & Jamie Rappaport. There were also numerous mini -seminars of half and full days throughout the year which were highly appreciated by the residents. The mini- seminars were in Pediatrics, Laryngology performed in the McGill Simulation center with fresh porcine larynges, Vestibular, and two days of Facial Plastic arranged by Dr Nabil Fanous. There was also a very successful temporal bone drilling lab course performed in the McGill Simulation center for the residents under the supervision of Dr. Sam Daniel.. I think much more can be done in this wonderful highly technical facility that we have. There were four McGill and 1 Canadian in training written exams as well as two oral exams prepared in order to assist residents towards the Royal College Certificate exam in the R5 level. All four McGill residents this year successfully passed their Royal College exam (Drs. Khalil Al-Macki, Fahad Al-Saab, Mark Tewfik and Hasan Al-Shemari). The enrichment year was successful with five excellent research projects done by Dr. Jonathan Young, Valerie Brousseau, Yahya Al-Badaai, Yaser Al-Rajhi and Yammine Nadine. However, to improve the schedule of the resident’s project completion, in order to better compete in the Resident Poliquin-Medtronic Competition of the

Canadian Society of Otolaryngology, there is a plan underway to change the start date of the enrichment year. Instead of the beginning of PGYR4 academic year, the resident work starts at the middle of PGYR3 year and ends at the middle of PGYR4. New for the up-coming academic year 2008/09, the residents rotating in the research enrichment year will have a month of community rotation at the Lakeshore General Hospital. Dr. Richard Lafleur is the Director of the Otolaryngology Division at the Lakeshore General Hospital, the OTL Department also has 3 other staff (Dr. Roger Lebel, Dr. Jack Rothstein and Dr. Gilles Belisle). This rotation allows residents to experience community- type otolaryngology practice in a small hospital setting with affiliated outpatient clinics. Rotation specific objectives and evaluation have been instituted. This is a specific requirement of accreditation for residency programs in Otolaryngology. Special community rotations to Inuit communities in the far north continue to play an important role in the residency training program. This is in large part to the efforts of departmental staff including Drs. Karen Kost, Jamie Rappaport, Mark Samaha, Anthony Zeitouni and Robert Sweet who make the local arrangements for this important clinical service and resident education experience. Presentations at the Residency Research Day were again at a high standard. Our department was extremely well represented and involved at the very successful Canadian Society Otolaryngology annual meeting in Jasper Alberta. We had a total of 18 residents’ podium and poster presentations. Our departmental financial policy supports the residents by covering part of their expenses at national and international meetings. This year each R1 resident in the core surgery program will rotate again for 2 months in our otolaryngology department. The Residency Program Committee met 6 times during the past year. The most important issue regarding our resident training is the coming Royal College’s external review of our program that will be on September 4, 2008. We, staff and residents, are well prepared for this evaluation. We had an internal review by McGill University on March 26, 2008 that highly praised our department for the changes we have made since our last Royal College Review of April 2006. If the Royal College would visit today they would find our residents now experience a very good atmosphere and healthy learning environment. I am confident we will pass successfully this turbulent period.

Some projects (e.g. Dr Young) have gone very well, but others have been less successful. Difficulties stemmed from: 1. A reduced priority assigned to research due to shortened 6-month intensive research period 2. Scheduling difficulties where research was not assigned priority even during the intensive 6-month

period 3. Impossibility of producing high-quality papers for Canadian OTL Meeting after only 3 months of

2.2.5 Fellowship Program (Dr. Jamie Rappaport – Director) The Fellow(s) will be directly responsible to the Director of the Fellowship Training Program and the

Otolaryngologist-in-Chief of the hospital where the Fellow is based. The Fellow(s) will be classified as research or clinical, as determined by the Fellowship Committee and confirmed by the office of the Dean of the McGill Faculty of Medicine.

The Fellow(s) will follow similar research guidelines as Enrichment year residents. This will include submission of a research proposal, implementation of the research project, presentations to the committee (initial, mid-year progress, fellowship grand rounds), and submission of a final report. The Fellow(s) must publish a minimum of one research-oriented paper during the fellowship year. Attendance at Research Topics lectures (Dr. Segal) is encouraged.

Clinical and teaching duties and course work will be determined by the individual supervisor and the fellowship committee. This will include rounds with the resident staff, lectures to medical students and residents, as well as supervision of resident consultations and discussion with attending staff.

Fellows will not compete with the residents for clinical or surgical case material, but will rather complement the training experience of the residents. This will include appropriate operative room supervision and teaching.

The 5th Melvin D. Schloss Lectureship was held at the Montreal Children’s Hospital with pediatric surgical grand rounds on February 4, 2008. Dr. Michael Rutter from the University of Cincinnati College of Medicine was our guest speaker. He gave an interesting talk on “Current advances in the management of laryngotrachael stenosis”

The 11th Annual Raymer Lecture held at the Jewish General Hospital on April 17, 2008 was presented by Dr. Erin Wright from the University of Edmonton. His topic was on ”Contemporary Management of Inverted Papilloma”.

Pediatrics organized grand rounds inviting our staff to attend Dr. Sharma’s presentation on “Cochlear Implants” at the Montreal Children’s Hospital on May 7, 2008.

On May 15th, 2008 Dr. Michael McKenna from Harvard University was the invited guest at our 16th Annual Resident Research Day/James D. Baxter Lectureship. His topic was on “The etiology of otosclerosis”. He presented on “Superior canal dehiscence syndrome” at resident teaching rounds the following morning.

The 62nd Annual meeting pf the Canadian Society of Otolaryngology – Head and Neck Surgery was held in Jasper, Alberta from June 1- 4, 2008. The department was well represented there. We had a total 20 paper presentations, including 2 in the Medtronic-Poliquin resident competition, 6 symposia/workshops, and 4 posters.

2.2.7 Rotation to the Quebec North – Kujjuaq, Pivurnituq, Inukjuak, Ivujivik, Salluit

During the residency program, residents at all levels have the opportunity to spend one to three weeks in Northern Quebec with a staff member. Although not compulsory, this rotation is strongly encouraged and seems to be enjoyed by the vast majority of residents. This rotation allows residents to experience rural medicine in a culturally and geographically unique Canadian Inuit aboriginal society. Dr. Kost is the Director of Otolaryngology at the Centre Hospitalier Inuutsilivik in Pivurnituq coordinates otolaryngology health services provided to the Nunavik Region. The region is divided into two geographic areas: the Ungava Bay region and Hudson’s Bay Region. Drs. Zeitouni and Rappaport service the Ungava Bay region, and Drs. Kost, Daniel and Sweet provide services to the Hudson’s Bay Region. During their week in the North, residents have the opportunity to work one on one with their staff during outpatient clinics and in the operating room. This allows for intense teaching and has significant educational benefits for both the ambulatory setting and in the operating room, where the pressures of a large hospital do not apply. During a five-day workweek, surgery is scheduled for a minimum of 4 out of the 5 days, and consists primarily of otological procedures. Residents are taught step by step, and one on one, how to perform a tympanoplasty with a graded increment of technical experience being acquired over the week. By the end of the week, most residents are able to perform a Type I tympanoplasty entirely on their own. In the ambulatory setting, residents are exposed to a wide variety of otolaryngology complaints with a predominance of otology related disorders. These include hearing loss, tympanic membrane perforations and other abnormalities, as well as malignancies particular to the Inuit population such as nasopharyngeal carcinoma. In addition to the academic value of the rotation, weather conditions and time permitting, residents are taken out into the tundra and may participate in activities such as igloo building and ice fishing. The CanMEDS roles are actively integrated into this rotation as they are into other rotations.

2.3 Research

2.3.1 Departmental Laboratories OTL Research Laboratory: (Dr. A. Katsarkas, Dr. H. Galiana) The Dizziness Clinic. This Clinic continues to function without interruption at the Royal Victoria Hospital. Cases are practically referred from all over Quebec and as of yesterday we have evaluated 23,205 patients during 60,196 visits. A number of these patients undergo routine ENG testing (about one out of three) and VEMP (Vestibular Evoked Myogenic Potentials) testing. Some of these patients undergo also more advanced testing. Almost 30% of these patients are referred from or to Neurology. During the past year part of this work was presented at the American Academy of Otolaryngology in Washington, where I was interviewed by the lay press after the presentation; the Pan-European congress in Vienna, Austria, where I was also a guest speaker; the OTL Association of Quebec in Gatineau, Quebec; the Pan-Hellenic Society in Thessaloniki, Greece; an international meeting in Athens, Greece. I was also invited to write an article, which was published two months ago for the Parkhurst Exchange, a medical journal, read by 20,000 physicians, I was told. I was also invited to write a review article on dizziness in aging for the American journal of Geriatrics. The OTL Vestibular Research Laboratory. The research continues by testing volunteers, patients with known specific disease entities who do not need such testing for diagnosis and patients who need further testing for diagnosis. The data processing is carried out by graduate students and Ms. Heather Smith, M.Eng. This project is supported by grants from CIHR until the year 2010. Dr. H. Galiana, who is presently the Chair of the Department of Biomedical Engineering, supervises the data processing. The clinical aspects are supervised by Dr. A. Katsarkas. After a number of graduate students and a lot of work, we finally proved that the fast phases of nystagmus are also induced by the vestibular signal. This will change the processes we assess vestibular dysfunction. This work will be presented at the meeting of the European Academy of Otolaryngology in Manheim, Germany next May. A rotating platform has been built in the Laboratory by a group of mechanical engineering students in an attempt to study how the somatosensory information changes in patients who lost vestibular function. At present, one mechanical engineer graduate student continues the project. The task is to control the platform and the information from the sensors attached to the feet by the computer. Dr. Ross Wagner, a biomedical engineer from the Department of Biomedical Engineering, provides valuable assistance in the laboratory. This assistance is greatly appreciated.

McGill Auditory Science Laboratory: (Dr. Sam Daniel)

The McGill Auditory Science Laboratory has been quite productive, thanks to the hard work by staff and trainees. We celebrated our 5th anniversary this year with great news: Our research program on “Expériences en sciences auditives: Prévention de l’ototoxicité; diagnostique et traitement de la surdité conductive” ranked first in the FRSQ fundamental research program. Our CIHR grant on ototoxicity ranked second in the committee. We are currently the only otolaryngology laboratory in the country that currently holds 2 active CIHR grants, and the only otolaryngology laboratory funded by FRSQ in Quebec. This year we have a new recruit M Dan Citra, a veterinary with extensive animal research background. Our research fellow Dr Sofia Sahmkow has completed her 2-year fellowship in June and matched in otolaryngology at Universite de Laval (a first for an IMG). Her work on the effect of Triethanolamine Polypeptide Oleate Condensate on hearing in a chinchilla ear model was recently published in Laryngoscope. She will be much missed. Our laboratory continues to collaborate actively with our partner Dr Robert Funnell who has a worldwide-recognized expertise in middle ear mechanics and modeling. We also actively collaborate with DR Luc Mongeau in Mechanical Engineering with whom we have recently submitted a joint NIH grant. Auditory Mechanics Laboratory (Dr. Robert Funnell) A major focus of our effort was on vibration measurements in Sam Daniel's lab at the Montréal Children's Hospital. The experimental set-up was upgraded and important progress was made in obtaining new measurements of eardrum vibrations in the gerbil. A master's student, Shruti Nambiar, has been working on this, and a new Ph.D. student, Nima Maftoon, started recently. Shruti also worked with people from the MNI on methods for aligning serial histological sections, with the goal of creating accurate 3-D models of some important small features in the middle ear. In the doctoral research of Li Qi on modelling the mechanics of the newborn ear, a second major modelling paper was accepted for publication (it came out this week) and an important paper on the analysis of newborn tympanograms was written; it has just been revised and resubmitted. Li passed his oral defence in May. This work has already led to useful insights relevant to the early diagnosis of hearing

loss in newborns. We also continued external collaborations, most notably with Willem Decraemer in Belgium and Michel Audette in Germany. Decraemer visited here for ten days in 2007 June; then and over the rest of the year we co-authored a book chapter and worked on the first non-linear models of the gerbil eardrum. With Michel Audette we contributed to work on registering our high-resolution human ear model to patient-specific clinical CT scans, with potential applications to surgical planning and image-guided surgery.

Wireless Informatics in Health Care: (Dr. Bernard Segal)

My current research aims to facilitate safe wireless informatics usage in healthcare by unblocking obstacles to wireless usage in healthcare. As Principle investigator of a highly-inter-disciplinary (otolaryngology, engineering, epidemiology, medicine) multi-hospital (Jewish General Hospital, MUHC (RVH & MGH), St Mary’s Hospital), multi-university (McGill, Concordia, University of Montreal, University of New Brunswick) research project, I have received significant funding for this research, which will evaluate the use wireless informatics to reduce medication errors in emergency rooms.

I continued my collaborations, most notably with Willem Decraemer from Belgium and Sam Daniel in Otolaryngology. Decraemer visited here for ten days in June; then and over the rest of the year we co-authored a book chapter and worked on the first non-linear models of the gerbil eardrum. In Sam Daniel’s lab at the Montréal Children’s Hospital, the experimental set-up was upgraded and important progress was made in obtaining new measurements of eardrum vibrations in the gerbil. In the doctoral research of Li Qi on the mechanics of the newborn ear, a second major modelling paper was accepted for publication and an important paper on the analysis of newborn tympanograms was written. (It is being revised and is expected to be accepted soon.) This work has already led to useful insights relevant to the early diagnosis of hearing loss in newborns. In teaching, since my long-time intensive course to final-year medical students is no longer given because of changes in the medical curriculum, I have begun work on various applications of computers to teaching, specifically in the areas of interactive 3-D models and interactive simulations. I pursued contacts with the University of Leipzig and the Fraunhofer Institute (both in Germany), and met with teachers in the Faculty of Medicine to identify applications for 3-D models in medical and dental teaching at McGill.

Upgraded my Web server to permit students to run interactive finite-element mesh generator, for use in Selected Topics asssignment and for demonstration to Mike Rosengarten; provided some information to Rosengarten concerning Web-based interactive simulations using Matlab.

With upgraded server, restored quizzes used in Unit 4 of Basis of Medicine. Provided feedback to NYU Educational Informatics group on interactive 3-D models for teaching

anatomy. Pursued contacts with Michel Audette (University of Leipzig, 3-D surgical simulators) and

Rossen Rashev (Fraunhofer Institute, interactive simulation framework) which may lead to valuable collaborations.

Met with Elaine Davis and Karen Samonds to identify applications for 3-D models in medical and dental teaching at McGill; based on our previous work, I started developing a pelvis model for use by Davis in her lectures and a head model for use by Samonds. Li Qi’s Ph.D. thesis was submitted and positive external review was received. The second major paper based on his doctoral research on the mechanics of the newborn ear was accepted for publication by J. Acoust. Soc. Am. A third paper is under revision and is expected to be accepted soon.

I co-authored an invited book chapter with Willem Decraemer on the anatomical and mechanical properties of the eardrum. He visited here in June and we made good progress on non-linear modelling of the eardrum.

Nicolas Ellaham’s M.Eng. thesis was submitted and accepted. In follow-on work by Shruti Nambiar, important progress was made in obtaining new measurements of eardrum vibrations in the gerbil, in Sam Daniel’s lab at the Montréal Children’s Hospital. I designed and built a new experimental set-up.

Shruti also carried out an interesting project in collaboration with Louis Collins and Mallar Chakravarty, applying their non-linear brain warping techniques to the middle ear.

Further work was done with Dusica Maysinger in Pharmacology, applying 3-D visualization and quantification methods to fluorescence optical sectioning microscopy and quantum dots.

Appendix III: PROGRAM AND HOSPITAL REPORTS

McGill Head and Neck Oncology Program (Dr. Martin Black – Director) CLINICAL SERVICES Active ,busy, multidisciplinary oncology clinics at the Jewish General and the MUHC evaluate and treat over 300 new cancer patients annually.We treat patients with malignant tumors of the oral cavity.pharynx and larynx as well as a large number number of thyroid cancer patients.We treat an increasing number of complicated H&N skin cancer patients and provide an important diagnostic service for Lymphoma patients presenting in the H&N. In addition, an increasing number of patients with benign neoplasms of the salivary glands and the parathyroid glands are seen and operated. Patients and their families are seen and thoroughly evaluated by the by the H&N team; surgeons,radiationoncologists,medical oncologists,nurses,speech therapist,dietician and this team is supported by dental consultants ,psycho-social professionals and hospital staff that are dedicated exclusively to them.Multidisciplinary Tumor Boards meet weekly at both sites to make treatment recommendations to patients. We also provide a consultant service to doctors who may present their patients case for tumor board suggestions. At the RVH site there is a Skull Base tumor board organized in a similar fashion with the MNI. The Tumor Boards are linked at both sites by tele medicine which allows for consultation and learning for the whole staff .This is done weekly and special monthly grand conferences for education are conducted via tele medicine links. TEACHING McGill Otolaryngology residents,medical students,general surgery,plastic surgery,oral surgery and anesthesia residents as well as Radiation Oncology residents rotate through the H&N services.Active teaching is ongoing in the clinics ,ORs and at tumor boards.We have had a H&N fellow for the past several years and Dr L Kalmovitch from Israel is finishing this year. Community teaching is provided for the public and for professionals. Seminars on thyroid cancer and Mini Med courses outside the hospitals have been given RESEARCH Clinical treatment protocols in collaboration with other universies and hospitals [e.g.RTOG, radiation therapy oncology group,NCIC, national canadian institutes of cancer ] evaluate the best possible treatmnt options available.We offer these protocols of treatment to our patients as well as conventional ones.Clinical and basic research on thyroid cancer looking at new markers and treatments for this increasingly common disease is being done. McGill based treatment protocols using biologic response modifiers and immunotherapies in conjunction with chemotherapy and radiation are exciting novel approaches to cancer treatment for our patients.A new protocol for the evaluation of Brachytherapy in the treatment of tongue cancer is opened for patient accrual. We have just finished a superb study on salivary gland transfer for the prevention of xerostomia [dry mouth] after radiation therapy

MUHC Adult Sites (Royal Victoria Hospital/Montreal General Hospital)

Highlights: In order to provide the best possible care, the Department at the RVH has undergone a profound transformation that has led to national recognition. These efforts have permitted us to greatly improve the technological base, both in our clinics and in the OR. With the aid of our donors the equipment is now of a cutting-edge world-class caliber. The most significant physical change is the construction of two symmetrical surgi-suites in the clinics for purpose of performing minor procedures. These suites were equipped with high-end Zeiss video-microscope connected to a secure electronic archiving system. We feel that this system will have a far-reaching impact on resident education in the field of otology. These suites also permit cervical ultrasonography and biopsies to be done in an ergonomic and comfortable setting for physicians and patients. Trans-nasal esophgoscopy is another cutting edge technique that can be performed as well. A new sterilization center has been built in the clinic to safely deal and support the procedures done in the clinic. All patients that come to clinic are recorded on a hospital and / or Departmental database, which have significant implications for research. The Department has specific databases for dizziness, head and neck cancer and skull base surgery (on the hospital server). Each of the examining rooms and each surgi-suite are now equipped with high-speed computers connected to the hospital main server. This permits the staff to view radiology, pathology, and other investigations with ease, allowing them to discuss the findings with the patient with little delay. We have invested heavily in our teaching facilities. A dedicated computer connected to the backbone of the hospital, LCD TV and projection facilities are some of the changes made this year. We continue to invest in our residents’ and fellow’s study room and office. The MGH and RVH have fostered and encouraged the development of patient teaching tools. These include a patient passport that was recently announced as the model for a province wide passport by Phillip Couillard. A patient information booklet; interactive computer with disease specific information for patients; multiple DVD’s, computer generated animations (by Molson Imaging at McGill) and most recently new patient information website: www.cancerorl.ca. All these efforts have led to considerable national recognition. Our MGH and RVH easily obtained accreditation in 2007 and won praise and national recognition for our patient education accomplishments. The head and neck team did very well in the site visit from the Lutte contre le Cancer and is well on the way to getting level 4 status. The skull base team, as an important part of the neuro-oncology submission was given level 4A status. It was the first team to get that status in the Province! The MGH clinic is fully equipped with a laryngoscope and wall-mounted microscope in each examining room. There is also a battery-powered laryngoscope available to staff and residents for in-patient/emergency consultations. The out-patient operating room is equipped with

a high-quality microscope with a video monitor, and has recently been outfitted with 2 operating room quality lights to allow for more complex procedures. Plans are underway to increase the sophistication of some of the equipment to allow for a wider variety of out-patient laryngeal procedures. The Voice and Dysphagia laboratories are equipped with a digital system which is used for recording examinations, and also allows for highly specialized outpatient procedures such as vocal fold augmentations. These laboratories are ideal for teaching because of the high quality imagery, and provide residents their only exposure to voice and swallowing disorders. Specialized laryngeal. Botox clinics are run through the MGH for patients with laryngeal dystonias.

Highlights:

1. There continues to be a high clinical volume of patients who are seen in the following

specialty clinics:

o Head and neck oncology (Drs. Black, Hier, Payne) o Rhinology (Dr. Frenkiel) o Otology (Dr. Rappaport)

2. General clinics are under the supervision of Drs. Finesilver and Fried.

3. Residents’ supervised clinics occur twice weekly.

4. The department supervised a head and neck fellow, Dr. Limor Muallem Kalmovich from

Israel. 5. Operating room volumes were maintained during the year including major head and neck

cases, endoscopic sinus surgery, microscopic middle and inner ear surgery, and complicated airway problems.

6. The Head and Neck Oncology Program achieved a level 3-4 status from the Provincial

committee “La lutte contre le cancer”. 7. Dr. Martin Black stepped down as Chief of the Department of Surgery. The hospital

welcomed two new administrators:

Dr. Lawrence Rosenberg, Chief of Surgery, and Dr. Hartley Stern, Director General of the hospital.
