This directory contains all active DHMP participating providers, and is current as of the date noted on the front and back cover; any additions/deletions will be reflected in future updates. If you are a POS member you haveaccess to all physicians listed in this directory, but have the choice to find a provider outside these networks.

This Provider Directory, and all other member informa-tion, is available in other languages, Braille, large print, and audiotapes. Please call Member Services (number below) if you need this Provider Directory in a different language or form.

DHMP provides interpreter services for many languag-es at no cost to our members. If you would like to use an interpreter during your clinic visits, please tell the Appointment Center representative when you make your appointment. If you would like to use an interpret-er for any other health care need, please call Member Services.

DHMP also offers TTY/TDD services for the hearing impaired. The TTY/TDD phone number for Member Services is listed in the “Important Phone Numbers” section on the previous page. If you need a sign lan-guage interpreter or other assistance during your clinic visits, please letMember Services know before your appointment date so arrangements can be made with an interpreter. MemberServices can also help you find physicians andprovide details about their services and professionalqualifications.

While you are not required to select a primary care provider, these practitioners can assist you to maintain and monitor your health as well as access the services of specialty care physicians.

Your primary care provider can help you access the wide range of medical services from our network spe-cialists and facilities. You do not need a referral or prior authorization to see an OB/Gyn for family planning (pregnancy) services or Gynecology.

You have the right to see any primary care provider who participates in our network and who is accepting members. For children, you may designate a pediatri-cian as the primary care provider.

Changing Your Primary Care ProviderIf you decide to select a new primary care provider,there is no need to tell us. You can change yourselection at any time. Our web site provides the

most up-to-date information on providers that partici-pate in the DHMP network. Or call Member Services at 303-602-2100 if you need more information.

What to do in an EmergencyIn a life or limb threatening emergency situation,call 9-1-1 or go to the nearest hospital emergencyroom. Notify your PCP as soon as reasonablypossible so that he/she can coordinate subsequentcare and schedule follow-up treatment. Whenliving or traveling outside of the network area,only emergency and urgent care services will becovered. If you or a family member have beenseen at/or admitted to a non-network hospitalfor emergency care, you will need to call DHMPMedical Management at 303-602-2140 within 48hours of seeking treatment or as soon as reasonablypossible, so we may assist in the coordination ofyour care.

Visiting a Network SpecialistSpecialty care is a covered benefit only when avalid referral is made by your PCP. Specialistsparticipating in the DHMP network are listedlater in this directory. You will also find listingsfor chiropractic care, acupuncture and massagetherapy. (this benefit does not apply to CHP+members).

Level One Physicians is a primary care clinic located on the Denver Health campus with access toon-site laboratory, radiology, and pharmacy services, Level One Physicians is here to provide care foryou and your entire family. This is a primary care clinic staffed by Family Practice, Internal Medicine,and Pediatric providers. Primary OB/GYN care is also offered through Level One Physicians clinicproviders. Behavioral Health Services are available on-site as well as certain limited specialty services.Care is by appointment only, but staff strive to provide same day access when available to meet theneeds of your family.
