Etymology

A Swahili phrase KI DENGA PEPO meaning asudden seizure by a Demon.

A spanish word for Affectation used to describe the mincing walk of the affected persons.

• It is a Mosquito borne viral infection characterized by sudden onset of fever ,severe headache,retro-orbital pain,myalgia,arthralgia,nausea & vomitting.

Etiology

• Family: Flaviviridae

• Genus: Flavivirus

• Species: Dengue virus

• Genome: (+)ssRNA

Pathogenesis

Regional lymph nodes

Replication in macrophages

Clinical signs in animals

Clinical disease from DEN virus only occurs in humans.

Non human primates such as chimpanzees,macaques& gibbons develop high levels of viraemia long

enough for mosquitoes to become infected.

Clinical signs in humans

Two forms of DF.

Classic DF

DHF/DSS

Classic DF has 4-to -7 days i/p,with

fever

severe headache

retro-orbital pain

petechial rash on trunk

DHF/DSS initially indistinguishable from classic DF but progresses to produce clinical signs

bleeding(esp.gastrointestinal)

shock.

Diagnosis in humans

Tentative diagnosis based on

clinical signs

Tourniquet test

history of travel to endemic areas

Confirmation by

serological tests(igM,igG captureELISA)

PCR

Virus isolation

Treatment in humans

Classic DF is self limiting only supportive care is required

Acetominophen for fever and pain

i/v fluid adminstration for patients with DHF\DSS.

whole blood transfusions or platelets in patients with severe bleeding.

Prevention and control

No approved vaccines,thus prevention depends on control of & protection from the bites of mosquitoes.

Getting rid of open sources of water

mosquito netting

use of insect repellants (DEET,most effective)
