NEW ADVERSE EVENT FORM Enclosed you will find the new DHS Adverse Event Form. Please make copies for your files and begin using this form effective immediately to report any Adverse Events or Change in Member Condition.
QUEST INTEGRATION PROGRAM
The DHS Med-QUEST Division will begin their new QUEST Integration program to replace the current QExA program effective January 1, 2015.
The following (5) health plans are participating in the QUEST Integration program:
Aloha Care, HMSA, Kaiser, Ohana Health Plan and United Healthcare
If you are planning to contract with any or all of the above health plans, please be sure you do so now. Call our office for any questions or concerns.
CARE GAPS: UHC CLIENTS ONLY
Our office has sent out letters to those caregivers with UHC clients, which lists the gaps in their care that UHC requires to be completed. If you have received a letter for your client, please be sure to contact their primary care physician to schedule an appointment to complete the health measures listed in the box. When the appointment has been scheduled, please call our office to inform us of the date of the appointment for our records. If you have any questions regarding the letter you received, please feel free to call our office for clarification.
TIP #1: WASH HANDS OFTEN Keeping hands clean is one of the most important steps you can take to avoid getting sick and spreading germs to others. Wash your hands with soap and clean running water, and rub them together for at least 20 seconds. Cover your mouth and nose with a tissue when you cough or sneeze. If you don’t have tissue, cough or sneeze into your upper sleeve or elbow, not your hands. TIP #2: FOOD SAFETY As you prepare holiday meals, keep yourself and your family safe from food-related illness. Wash hands and surfaces often. Avoid cross-contamination by keeping raw meat, poultry, seafood, and eggs away from ready-to-eat foods and eating surfaces. Cook foods to the proper temperature. Refrigerate promptly. Nightingale will dedicate a special after-hours RN line for our clients and caregivers effective December 26, 2014. There will be just one number to call!

M-F 4:30pm – 8:00am & weekends/holidays

UPDATING DOCUMENTS
Just a friendly reminder from our office staff that if you have any new forms for your client (doctors orders, history & physical, TB, etc.) to please fax or mail it to our office as soon as you can.
Also, if your visiting RN has updated your clients Service Plan in your chart, please fax it to our office (fax: 488-2151). It is important for us to have a copy in our files to keep track of the clients care and because CTA and the health plans also check our office charts to ensure the Service Plan are being updated.
INCREASE IN ROOM & BOARD

There will be an increase in the monthly Room & Board amount effective in January 2015. We currently do not have the amount of the increase, but as soon as that information is received we will pass it on to all of you.
MEDICATION & MAR ‘s
From our Medical Transcriptionist: Jerome Rosacia
Please be sure that all of your client’s medication orders match what is printed on the bottle as well as the MAR and medication list. If there is a discrepancy in any one of these, please be sure to correct it with your client’s physician, your visiting RN as well as our office. To ensure our clients’ safety, we need to ensure that all medications orders are current and accurate. Please do not hesitate to call your visiting RN if you have a question regarding your client’s medications.

Grandma Browne ’s Hot Apple Cider
4 cups apple juice ½ cup frozen orange juice 4 cups water ½ cup sugar 4 whole cloves ½ inch cinnamon stick 3 whole all spice Combine all ingredients in a pot and simmer. Keep on low heat and serve immediately.
