As we journey through this lifeand witness the passing of thatgeneration just before ours, webegin to realize just how shortthis life is. Why are we not moreloving to each other? Why do wespend time hating and harmingone another? In the natural course of life, wehave come to accept the passingof our mothers and our fathers, this tells us that the nextgeneration on the way out is ours. Our children are nowhaving children, this makes usgrandparents, our children’schildren will soon be havingchildren, this makes us greatgrandparents. Do you honestlybelieve that you will be aroundto see your children’s, children’s,children’s having children? Theunstoppable passing of your

Ifwe want to do our part in makinga better world it is imperative forthose with infant children toteach them to love and care forothers. The best way to do this isto educate your children. Preparethem for a successful life ahead.A child unprepared to face life’schallenges will become amenace to society. Let’s stop hating and startloving each other. Those who hateand harm others are testimoniesof the ill prepared. Ask yourself, were you prepared or ill preparedto face life challenges? Theanswer can be had by simply ask-ing yourself another question:Are you a hater or are you a lover?

Dear Editor; Thank you for the enlighteningarticle last week by Mr .Leonides Sanchez. I fullyagree with his view and reiteratethe need to instill in our childrenthe skills necessary for theirhonorable survival. In furtherance of Mr. Sanchez’views, I am one of those disgustedby the increasing number ofadults and now children, messageon a Bristol board in hand, begging at speed bumps along ourhighways. In some instances theystand dangerously in the middleof our highways and in many

instances they are femalechildren. Every time I come upon thissituation, it makes me ask: Arewe creating a society of beggars? Why do these people notorganize fundraising events intheir communities? Is it becauseit is easier to beg than to workfor what we need? I would prefer any time to stopand spend ten dollars at a Bar-B-Q fundraising event than to dropa shilling into the outstretchedhands of beggars on our highways.Kudos!!! Very well said Mr.Sanchez.Yolanda Alvarado

Dengue fever, also knownas breakbone fever, is amosquito-borne tropical diseasecaused by the dengue virus.Symptoms include fever, headache, muscle and joint pains, and a characteristic skin rash thatis similar to measles. In a smallproportion of cases the diseasedevelops into the life-threateningdengue hemorrhagic fever, resulting in bleeding, low levelsof blood platelets and bloodplasma leakage, or into dengueshock syndrome, wheredangerously low blood pressureoccurs. Dengue is transmitted byseveral species of mosquitowithin the genus Aedes, principally A. aegypti. The virushas five different types; infectionwith one type usually giveslifelong immunity to that type, but only short-term immunity tothe others. Subsequent infectionwith a different type increasesthe risk of severe complications. As there is no commerciallyavailable vaccine, prevention issought by reducing the habitat andthe number of mosquitoes andlimiting exposure to bites. Typically, people infected withdengue virus are asymptomatic(80%) or only have mildsymptoms such as anuncomplicated fever. Others havemore severe illness (5%), andin a small proportion it islife-threatening. The incubationperiod (time between exposureand onset of symptoms) rangesfrom 3–14 days, but most oftenit is 4–7 days. Therefore, travelers returning from endemicareas are unlikely to have dengueif fever or other symptomsstart more than 14 days afterarriving home. Children oftenexperience symptoms similar tothose of the common cold andgastroenteritis (vomiting anddiarrhea) and have a greater riskof severe complications, thoughinitial symptoms are generallymild but include high fever. The characteristic symptomsof dengue are sudden-onsetfever, headache (typicallylocated behind the eyes),muscle and joint pains, and a rash. The alternative name for dengue,"breakbone fever", comes

Dengue Fever

from the associated muscle andjoint pains. The course ofinfection is divided into threephases: febrile, critical, andrecovery. The febrile phase involves highfever, potentially over 40 °C (104°F), and is associated withgeneralized pain and a headache;this usually lasts two to sevendays. Nausea and vomiting mayalso occur. A rash occurs in 50–80% of those with symptomsin the first or second day ofsymptoms as flushed skin, orlater in the course of illness(days 4–7), as a measles-likerash. A rash described as"islands of white in a sea ofred" has also been described. Some petechiae (small redspots that do not disappearwhen the skin is pressed, which are caused by brokencapillaries) can appear atthis point, as may some mildbleeding from the mucousmembranes of the mouth andnose. The fever itself isclassically biphasic orsaddleback in nature, breakingand then returning for one or twodays. In some people, the diseaseproceeds to a critical phase asfever resolves. During thisperiod there is leakage of plasmafrom the blood vessels whichtypically lasts one to two days. This may result in fluidaccumulation in the chest andabdominal cavity as well asdepletion of fluid from thecirculation and decreased blood

supply to vital organs. There mayalso be organ dysfunction andsevere bleeding, typically fromthe gastrointestinal tract. Shock(dengue shock syndrome)and hemorrhage (denguehemorrhagic fever) occur inless than 5% of all cases ofdengue, however those who havepreviously been infected withother serotypes of dengue virus("secondary infection") are at anincreased risk. This critical phase, while rare, occurs relatively morecommonly in children and youngadults. The recovery phase occurs next, with resorption of the leaked fluidinto the bloodstream. This usuallylasts two to three days. Theimprovement is often striking, and can be accompanied withsevere itching and a slow heartrate. Another rash may occur witheither a maculopapular or avasculitic appearance, which isfollowed by peeling of the skin.During this stage, a fluid overloadstate may occur; if it affects thebrain, it may cause a reduced levelof consciousness or seizures. Afeeling of fatigue may last forweeks in adults. There are no approved vaccinesfor the dengue virus. Preventionthus depends on control of andprotection from the bites of themosquito that transmits it. TheWorld Health Organizationrecommends an Integrated VectorControl program consisting offive elements: * Advocacy, social mobilizationand legislation to ensure that

public health bodies andcommunities are strengthened; * Collaboration between thehealth and other sectors (publicand private); * An integrated approach todisease control to maximize useof resources; * Evidence-based decisionmaking to ensure anyinterventions are targetedappropriately; and * Capacity-building to ensure anadequate response to the localsituation. The primary method ofcontrolling A. aegypti is byeliminating its habitats. This isdone by getting rid of opensources of water, or if this is notpossible, by adding insecticidesor biological control agents tothese areas. Generalized sprayingwith organophosphate orpyrethroid insecticides, whilesometimes done, is not thoughtto be effective. Reducing opencollections of water throughenvironmental modification isthe preferred method of control, given the concerns of negativehealth effects from insecticidesand greater logistical difficultieswith control agents. Peoplecan prevent mosquito bites bywearing clothing that fullycovers the skin, using mosquitonetting while resting, and/or theapplication of insect repellent(DEET being the mosteffective). However, thesemethods appear not to besufficiently effective, as thefrequency of outbreaks appearsto be increasing in some areas, probably due to urbanizationincreasing the habitat of A.aegypti. The range of the diseaseappears to be expanding possiblydue to climate change.

The last three weeks haveprovided a consistent narrativeof fast, attacking football andexcitement - exactly as thegame's rulers would haveimagined it when they handed thetournament to Brazil. Even the line-up for the lasteight has a balance that bringspleasure to the purists, as fourteams from the Americas arecomplimented by a quartet fromEurope - all of them groupwinners. The destiny of this World Cupwill be decided in the iconicsurroundings of the Maracana inRio de Janeiro on Sunday weekbut all quarter-finalists have theopportunity to show where thebalance of power lies betweenthe two continents. The seven World Cups held herein the Americas have failed toproduce a European winner. Onthe flip side, Brazil are the onlynation from the Americas toachieve a win on Europeansoil - when they triumphed 5-2in the final against hosts Sweden56 years ago. So for Netherlandscoach Louis van Gaal, France'sDidier Deschamps, Germany'sJoachim Low and the BelgianMarc Wilmots there is the dualopportunity to make history fortheir countries and exertEuropean domination. For the likes of Luiz FelipeScolari, attempting to win hissecond World Cup with Brazil, and Argentina's Alejandr oSabella in particular, there is thechance to send Europe homeempty-handed once more. At this stage Europe has oftendominated. When Argentina,Brazil, Paraguay and Uruguayreached the last eight in SouthAfrica four years ago, it was thefirst time representation from theAmericas had been so strongsince the Mexico World Cup in1970. So a similar number hererepresents a continuedresurgence and an advance onthe dark days of 1990 and 1994when only one nation from theAmericas reached the last eight -although Brazil did what matteredin the United States and claimed

the trophy against Italy onpenalties. It was an era when the greatDiego Maradona was criticallyon the wane for Argentina, eventhough they advanced to lose adesperately dull final to WestGermany at Italia 90 - the SouthAmericans showing they couldstill usually get one team in at theWorld Cup's sharp end. The figures started to stack upa little more attractively with twoteams from the Americas at thisstage in 1998, 2002 and 2006 -and now we have the face-off herein Brazil. So do these figures represent alasting shift in the balance ofpower back towards theAmericas, or are there justgeographical forces at work? Germany coach Low believestournaments held in SouthAmerica hold a special appeal forthose born there. As he preparedfor an all-European quarter-finalagainst France, he said: "TheSouth and Central Americans -Argentina, Colombia, Brazil,Mexico, Costa Rica - havecertainly shown that on theircontinent they want to showtheir class. "They want to show the entireworld 'this is where we'replaying, this is our continentand we will do whatever isnecessary to satisfy our fansand our countries'. "It was no surprise that these

teams performed so well. Theyare very strong; they are at homeon this continent and you canfeel that."The continued influx of South

American players into Europesince those struggles in the early90s has bred familiarity with thestyle and demands, producing

more rounded players andperhaps helping in the WorldCup environment. The threesuperstars of the tournament sofar - Brazil's Neymar, LionelMessi of Argentina and theemerging Colombian JamesRodriguez - ply their trades atBarcelona and Monacorespectively. South America football writerTim Vickery also believes thecontinent benefited from achange in the qualificationprocess. "There can be no doubt thatthe so-called lesser nations inSouth America have madedramatic strides since theintroduction of the marathonWorld Cup qualification formatin 1996," he said. "Regularcompetitive games andguaranteed income has allowedthem to appoint good coachesand build proper sides. "The South American nationsrealised that the forces ofglobalisation take their playersto Europe at an ever-earlierage. The youth sides are wherethey secure them for the long-term future of the national team

The South, Central and NorthAmericans have also bought intothe fast-flowing, high-tempostyles that have been a feature ofthis World Cup. The African sidessuch as Nigeria and Algeria haveshown the way too. It would be wrong to suggestdefence has become a forgottenart but the most successful teamshave embraced the mood ofswift, counter-attacking intent. Algeria coach VahidHalilhodzic , whose teamcreated such a fine impressionbefore going out to Germany inextra time in the last 16, believesthe demands of the Europeanleagues have also given theAmericas an opportunity to maketheir mark here. "The debacles of someEuropean teams like Italy,Spain or even England -although for me England wasnot a surprise - were due to thefact they were a little bit tired,"he said. "There was somesaturation because their ownnational leagues were difficult,

challenging, with a highnumber of matches. "They rested for a fewdays but it's not enough. There was a lot of physicaltiredness but also psychological- a lack of motivation I'veseen in some of the Europeanteams. For me, some playerswho had won a number ofthings and were not reallywilling to win here. They didn'twant to make more moneyor really give it their all towin the World Cup." He believes this particularlyapplied to the dramatic decline inholders Spain, who were expectedby many to fly the flag forEurope once more as theyattempted to break through theSouth American barrier andretain the trophy. "There was a lack ofambition," he said. "They wonabsolutely everything in thelast decade and I think theywere not willing to fight anymore." One element that has alwaysbeen regarded as having asignificant influence is theheat of the Americas, inparticular the fierce temperature

of two tournaments held inMexico. Former England wingerChris Waddle said: "Of coursethere is an advantage forplayers who have been broughtup in hot climates, but I thinkwith modern sports scienceand preparation techniquesit is not as much of a factor as itwas. "I played in Mexico in 1986and that was really hard. It wasplayed at altitude and thetemperatures were up about 35to 40 degrees. "In some ways we werethe guinea pigs back thenand now, in the modern era, you've got dieticians, sportsscientists and all mannerof people who give youadvice on how to get themost out of your body. Ithink it is a more levelplaying field for Europeanteams coming to places likeBrazil now." And on that level playingfield in Brazil, a wonderfulWorld Cup has delivered astraight fight for supremacybetween Europe and theAmericas.
