Royal Australian Navy Rear Admiral James GOLDRICK AM CSC RANR, ACT For distinguished service as Commander, Border Protection Command, Commander, Joint Education and Training, and Commandant of the Australian Defence Force Academy, and for outstanding scholarship in the study of Australian naval history.Rear Admiral Goldrick's distinguished service and devotion to duty in senior command positions have been critical to the effectiveness of the Australian Defence Force now and into the future. His contribution to the knowledge and study of Australian's naval history has been outstanding. Rear Admiral Goldrick has greatly increased the operational effectiveness and enhanced the national and international reputation of the Australian Defence Force.

Australian Army Major General Grant Douglas CAVENAGH AM, Vic For distinguished service to the Australian Defence Force as Commander Joint Logistics and as Head Land Systems Division.Major General Cavenagh has led substantive change in the areas of strategic logistics and the procurement of equipment that has resulted in improved delivery of Defence capability to the nation. His comprehensive and forthright approach is indicative of his distinguished service, bringing great credit upon himself, the Australian Army and the Australian Defence Force.

Major General Gerard Paul FOGARTY AM, ACT For distinguished service as Deputy Commander Joint Task Force 633 in Iraq, Director General Personnel - Army and as Head People Capability. Major General Fogarty has been fundamental in the realisation of Australia's national interests and in leading profound strategic change for Australian Defence Force personnel. His diligence, clear desire to improve the understanding and management of people and sense of duty are hallmarks of his distinguished service and bring great credit upon himself, the Australian Army and the Australian Defence Force.

Captain Simon Giuseppe OTTAVIANO RAN, ACT For distinguished performance of duty in warlike operations as Chief of Staff Headquarters Joint Task Force 633 on Operation SLIPPER from July 2011 to January 2012.Captain Ottaviano's distinguished performance as Chief of Staff at Headquarters Joint Task Force 633 directly contributed to the Australian campaign, particularly with regard to improving the procedures for managing critical incidents and matters of national strategic sensitivity, and the capacity to better sustain deployed, widely dispersed operations. Captain Ottaviano was a driving force and inspiration for the cohesive, positive environment which

typified the headquarters throughout his deployment.

Commander Andrew Paul QUINN RAN, ACT For distinguished performance of duty in warlike operations as the Commanding Officer, HMAS Toowoomba on Operation SLIPPER from June to October 2011.Commander Quinn's leadership enabled HMAS Toowoomba to achieve an outstanding level of performance in the conduct of coalition maritime operations as directed by the Commander, Combined Maritime Forces. Commander Quinn contributed significantly to the protection of life and security at sea in the Middle East Area of Operations, particularly through operations aimed at terrorism, piracy and other illegal activities.

Her application of high standards, demonstrated performance and unwavering commitment underpinned the success of the Establishment during a particularly busy period.

Commander Mitchell Robert LIVINGSTONE RAN, India For outstanding achievement while Commanding Officer HMAS Pirie engaged in the rescue of survivors from a foundered vessel at Christmas Island on 15 December 2010. Commander Livingstone acted without hesitation and remained calm when he led the ASSAIL THREE crew in HMAS Pirie in their efforts to recover survivors after Suspected Irregular Entry Vessel 221 broke up on the rocks of Christmas Island in violent weather conditions. His focused leadership and decisiveness inspired the crew to extraordinary acts of bravery and skill to save others. Commander Livingstone's leadership, sound judgement and shiphandling ensured the survival of many who would have otherwise drowned.

Commander Paul James MOGGACH RAN, ACT For outstanding achievement in the performance of duty as the Commanding Officer of 817 Squadron from August 2009 until decommissioning of the Squadron in December 2011.Commander Moggach has executed his duties in command of 817 Squadron until decommissioning under unique, complex and challenging circumstances. The Squadron provided the maximum level of capability to the Australian Defence Force until the last day of service due to Commander Moggach's professionalism, leadership and loyalty to his sailors, officers and his country.

Warrant Officer Michael John QUINLAN, WA For meritorious achievement as the Submarine Escape Training Facility Training Officer at HMAS Stirling, Western Australia.Warrant Officer Quinlan ensured the safety of all who train at the Submarine Escape Training Facility by designing and implementing a training and certification package for the largest single group of water-working instructors ever trained in the Royal Australian Navy. He enhanced already stringent safety procedures in an unforgiving workplace.
