Georgia Provider- G Meetings

• Purpose: To allow Georgia DHS to collaborate with our provider partners to form and test hypotheses surrounding CFSR standards and Kenny A outcomes to further improve these outcomes for our children and families.

Provider-G Meetings

• DHS began hosting 2 monthly Provider-G meetings in October 2009

• The meetings are divided into two groups:• CCI-Child Caring Institution• CPA-Child Placement Agencies

• Each group’s Provider meetings is specialized for their data

• DHS is reviewing global data for CPA’s and CCI’s but focuses on the data of the top 15 CPA providers and Top 20 CCI providers.

• These providers represent over 50% of the children in CCI/CPA homes across the state

Performance-Based Permanency Initiative

• Purpose: To Improve Georgia’s performance on CFSR outcomes related to reunification and permanency for children in foster care and allowing provider partners the opportunity to share in the responsibility.

Performance-Based Permanency Initiative

• Agencies who currently have youth in their

care that meet all of the following criteria are

eligible to apply for the initiative: • 13 years of age or older

• 24 or more months in care

• With a primary or concurrent permanency plan of Reunification or APPLA

• In a group home, child care institution, or CPA foster home

Performance-Based Permanency Initiative

• DHS submitted to provider partners a list of youth our records indicated they currently have in their care, meeting the previously mentioned criteria• Over 150 agencies received a list of children

• 498 children in CCI’s

• 248 children in CPA’s

• Agencies were advised the need to further assess the child to determine if the Performance-Based Permanency Initiative is appropriate for the child.

Performance-Based Permanency Initiative

• All agencies interested in participating in the permanency initiative were required to attend the Permanency Initiative Training Session on December 8, 2010.• 105 agencies attended

• CCI Agencies began enrolling on December 14, 2009 • As of January 6, 2010

• 6 agencies have been approved

• CPA Agencies will begin enrolling February 2010

Performance-Based Permanency Initiative

• Office of Provider Utilization and Outcome Management (OPUOM) will review all agency readiness forms and approve/disapprove the agency to participate in the Permanency initiative.

• If agency is approved Agency must submit Permanency plan for identified child within 30 business days of approval to the OPUOM unit.• Permanency Plan must include the DFCS

caseworker’s approval

Performance-Based Permanency Initiative

• Provider agencies are required to submit monthly reports to the DFCS case manager as well as OPUOM outlining progress and any continuing barriers to permanency and reunification.

• Services should result in at least 70% of accepted children be placed in a permanent living environment and that 80% of those children remain stable at the six-month anniversary of their discharge.

Performance-Based Permanency Initiative

• Provider Agencies will be paid performance incentives as they reach benchmarks along the timeline. • Payment 1 ($12,000) - Step Down to permanent

placement (enrollment in school, parental supervision and safety plan submitted)

• Payment 2 ($14,000)- Discharge (school or vocational attendance 80% of the time during the past 30 days, zero incidents of serious physical aggression in the last 30 days)

• Payment 3 ($9,000) - 90 days post dischargeTotal- $ 35, 000

Performance-Based Permanency Initiative

• Outcomes will be tracked by OPUOM and reviewed with providers at the monthly Provider-G Meetings

• OPUOM will conduct random site visits monthly to determine and assess if data submitted are accurate• Written report of the findings will be submitted

to Contract monitoring monthly

Performance-Based Contracting

• Purpose: To allow Georgia DHS to emphasize on results related to output, quality and outcomes rather than how the work is performed. Performance-Based Contracting will allow DHS to have clearly defined objectives and timeframes, use measurable performance standards, quality assurance plans and provide performance incentives and penalties to payment outcomes

Performance-Based Contracting
