CVPA Membership and Tournament Adult Waiver/Release- READ BEFORE ACCEPTING This waiver is a legal document which may afect your legal rights to claim damages arising from injury and/or economic loss you sufer from playing , waiting to play or preparing to play pickleball. If you have any questions or concerns as to what legal rights may be efected or given up by signing this document, you should consult with a lawyer or legal representation before signing . RELEASE OF LIABILITY, WAIVER OF CLAIMS ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT BY AGREEING WITH THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS INCLUDING THE RIGHT TO SUE. Participation in the sport of Pickleball and use of any recreational facilities involves a risk of accidental injury or death. I assume all risks, hazards incidental to the game, and I, my heirs, executors, administrators release from responsibility and agree to indemnify and hold harmless Comox Valley Pickleball Association, their ofcers, directors and volunteers from any and all liability, including negligence, for any loss, illness, damage, expense or injury, including death, that I may sufer. Being of legal age, I have read fully the previous paragraphs and I know, understand and appreciate these and other risks that are inherent in the activity. I hereby assert that my participation is voluntary and that I knowingly assume all such risks. By agreeing to this document in our registration process you will waive certain legal rights including the right to sue. Photo/Video Release Waiver I hereby consent to and authorize the use and reproduction by the Comox Valley Pickleball Association (CVPA) and its agents of any and all photography and/or videography that has been taken of me for any purpose, without compensation to me. All digital image and/or video fles are wholly owned by the CVPA, who reserves the right to use these images and/or video for publicity purposes.
