A full curriculum and training program was developed for approximately 60 interprofessional faculty and students from nursing, pharmacy, physical therapy, medicine, dental medicine, social work, and public health. All of the faculty and students participated in the training and then went on interprofessional home visits for older persons who have multiple chronic conditions and high emergency department use, as an effort to provide interprofessional feedback to primary care providers to improve these older persons quality of life and reduce this ED use.
