Current Departmental Duties at the Division of Nuclear Medicine, School of Medicine, Johns Hopkins University
- Serving as a Clinical Physicist in Nuclear Medicine at the PET Service Center
- Serving as a member of the division’s Physics and Informatics Core
- Serving as a general point-of-contact for quantitative methods in nuclear medicine and
molecular imaging
- Serving as a member of the Radioactive Drug Research Committee
- Specific job descriptions also involve performing dosimetry calculations for research projects,
e.g. new diagnostic and therapeutic radiopharmaceuticals, performing radiotracer kinetic
modeling and performing own research projects
Current Research Projects
1. Dose Reduction on Pediatric Molecular Imaging
Responsible for the development of pharmacokinetic (PK) models for two of the most commonly
used radiopharmaceuticals in pediatric nuclear medicine: 99mTc-DMSA for renal and 99mTc-MDP for
bone imaging. This involves analysis of the pediatric patient data from Boston Children’s Hospital
to validate and optimize the PK models. The overall goal is to reduce the administered doses in
pediatric diagnostic nuclear medicine without compromising the quality of the diagnostic images,
thus optimizing the diagnostic imaging in this area of pediatric nuclear medicine.
2. 3-Dimensional Radiobiological Dosimetry (3D-RD)
Responsible for performing patient-specific dosimetric calculations using 3D-RD, an in-house
developed patient specific software package. Responsible for the ongoing development of the
software package including the development and validation of a graphical user interface for 3D-RD.
3. I-124 PET/CT based tumor and normal organ dosimetry for I-131 therapy of metastatic
differentiated thyroid cancer (DTC)
Performing dosimetry calculations for metastatic differentiated thyroid cancer patients at
MedStar Washington Hospital Center, for both normal organs and tumors, to compare the
utilization of OLINDA and 3DRD methods for dose calculations and to study overall patient
outcome in the treatment of thyroid cancer.
4. Technical method on quantitation of small volumes (tumor dosimetry)
Developing a PET segmentation algorithm to better quantify small volumes (i.e. tumors) in
PET as needed for dosimetry calculations.
5. Combined model-based and patient-specific dosimetry for therapy of metastatic prostate cancer
(PCa) using 18F-DCFPyL, a PSMA-targeted PET agent.
Utilizing the Monte-Carlo based models developed for the salivary (using EGS code) and
lacrimal glands (using Geant4 toolkit) to calculate S-values for absorbed dose calculations for
Donika Plyku Page 3
various therapeutic isotopes (i.e. Ac-225, At-211) and PSMA-targeted molecule DCFPyL for
treatment of PCa patients.
Previous Research Projects
1. I-124 PET/CT based tumor and normal organ dosimetry for I-131 therapy of metastatic
differentiated thyroid cancer (DTC) – a comparison of recombinant human thyroid stimulating
hormone (rhTSH) versus thyroid hormone withdrawal (THW) patient preparation methods
Performed dosimetry calculations for metastatic differentiated thyroid cancer patients at
MedStar Washington Hospital Center, to study and compare the two patient preparation methods,
rhTSH and THW, in I-124 PET/CT based I-131 therapy of metastatic DTC.
2. Combined model-based and patient-specific dosimetry for risk evaluation of 18F-DCFPyL, a
PSMA-targeted PET agent for patients with metastatic prostate cancer (PCa)
Performed dosimetry calculations for metastatic prostate cancer patients at Johns Hopkins
Hospital to evaluate the use of a new PSMA-targeted diagnostic imaging agent (18F-DCFPyL) in
the imaging of prostate cancer patients. Responsible for the development of Monte-Carlo based
models for the calculation of the absorbed dose to non-standard organs, i.e. lens of the eye, lacrimal
and salivary glands for 18F.
