The month of festivals is now on - with Ganesh Puja - Durga Puja – Deepawali to celebrate. We are late for wishing you for Ganesh Puja and Durga Puja but let us wish all our readers a very h a p p y a n d p r o s p e r o u s D e e p a w a l i . E n j o y y o u r Deepawali with lots of fruits and ze ro o i l swee t s and namkeen. Two more Saaol centers were inaugurated this month – one in Tambram in Chennai and the other in Yamuna Nagar in Haryana. Three more centers are very are in queue for inaugrat ion and they are Kishangunj in Bihar, Meerut in UP and Vashi in Navi Mumbai. All the three will be inaugurated in the next one month. The Tambram center covers lower or

south end of Chennai near the airport and is the third center in this city. This was officially

t hi n a u g u r a t e d o n 3 0 September,2019 by Dr. Chhajer. We already have one center in the Center of Chennai – West Mamblam and one in North Chennai in Kilpauk. Yamuna Nagar is the second biggest city of Haryana and Dr. Chhajer was there to inaugurate SAAOL

thcenter on 6 October, 2019. This will be our fifth Center in Haryana – the others being Karnal, Hisar, Rohtak and Faridabad. Saaol's newest addition in Heart care is a four hour camp cal led “Saaol Audiovisual Vacc ine by Education against Heart Attack” or simply “SAVE Heart Attack”. This can be called “Heart Attack Vaccine” .

This camp has been developed by Dr. Chhajer and his team as a mass education program for Prevention of Heart attack. It has about 12 lectures in Video Format recorded by - Dr. Bimal Chhajer which can be shown in a group or individually to educate people so that they exactly understand what all they need to do to prevent heart attack by 98% surely.

To begin with this program is available in Hindi and we are in the process of developing the English version. As we need to be in terac t ive wi th hear t patients solving their question after each talk – we need a Saaol trained and certified expert for better execution of this “Save Heart Attack” camp. All these talks by Dr. Chhajer are very simple and anyone paying little attention can understand what is Heart

Attack and how to prevent them by l i f e s ty le and med ica l management. Our First “Save heart Attack” or “Heart Attack Vaccine” camp was held in

thJaipur on 16 September – on the birth day of Dr. Bimal C h h a j e r . T h i r t y t w o participants attended this 4 hour camp. Amongst other n e w s – S a a o l ' s E m a m i

thsponsored camp was held on 8 September, Delhi HLTP camp

thwas on 29 September. Our Bangladesh Saaol Franchisee Center owner and noted poet, activist Mr. Mohon Raihan conducted a talk in Japan in

thTokyo on 30 September,2019. Saaol is in talks to open a center in Japan now. Dr. Chhajer may visit Tokyo in near future at their invitation. . We have now 8 centers in Delhi NCR and we plan more centers in Mumbai in near future.

( )Saaol Audiovisual Vaccine by Education1 . H e a r t a n d v a s c u l a r C T angiography can detect narrowing or obstruction of blood vessels allowing potentially corrective therapy to be done.2. CT angiography may give more precise anatomical detail than other tests, particularly in small blood vessels.3. Many patients can undergo heart and vascular CT angiography to diagnose blood vessel problems.4 . H e a r t a n d v a s c u l a r C T angiography is faster, non-invasive.5. CT Angiography is a useful way of detecting arterial (such as narrowing of blood vessels in the heart) and venous disease, as well as structural abnormalities of the heart before there are symptoms or when symptoms are not clearly related to blood vessel disease.6. There is also potentially less

d iscomfor t because contras t material is injected into an arm vein.7. No radiation remains in a patient's body after heart and vascular CT angiography.8. X-rays used in standard CT scans have no immediate side effects.

1. In some people especially aged (above 75 years) if Calcium score is very high CT will not be a very good t o o l t o e x a c t l y p r e d i c t t h e percentage of blockage.2. If the heart rate is very high (above 80 beats per minutes) normal CT scan can not be done. So, patients need medicines to reduce the heart rate below 70 per minutes or use Dual source CT scan which can successfully carry on scanning even if the heart rate is above 100 per minutes.

Saaol Times, October, 2019

To t rea t hear t disease we need to Diagnose the condition of the

patient – the number of blockages, location of the blockage and percentage of blockage. The only option till now was to send the patients to Catheter Angiogram – done by the cardiologists in Heart Hospitals. Not only it was a mini operation with i ts possible c o m p l i c a t i o n s b u t t h e cardiologists would create a panic o f H e a r t a t t a c k a f t e r t h e angiography to force the patients to Bypass surgery or Angioplasty. But after 2005 – CT Coronary Angiogram came into being. And since then I became a fan of CT Angiogram and have got it done on lacs of my patients. People used to ask me a common question – how much radiation would go inside the body by this test? Let me give you a very clear picture here. The bad effects of Radiation on the body is measured as Mili Sievert (mSv). One x ray chest leads to 0.1 mSv. A normal person usually gets 1.5 to 3.5 mSv per year exposure of Radiation from Sun rays/ultraviolet rays and e n v i r o n m e n t – w h i c h i s

equivalent to 15-35 chest X rays. Maximum one can tolerate is 50 mSv in a year. Excess radiation lead to many diseases including cancer. In 2007 due to the old technology one CT coronary Scan used to give about 12.4 mSv – but now with the improved machines it has gone down to just 1-2.7 mSv. (equivalent to 10-27 Xray chests). A catheter angiography in cardiac hospital leads to 7mSv (equal to 7 0 c h e s t x - r a y s ) a n d o n e

Angioplasty can lead to 15-20 mSv(equivalent to 150-200 X ray chests) radiation exposure. So, please be aware of Angioplasty a n d C a t h e t e r Angiographies!

Saaol Times, October, 2019

How is CT Angiography done?CT Coronary Angiography is the latest non-invasive test, which has created a lot of hope for the heart patient. This is a simple test, there is no admission required. You just lie down on the table, one injection will be given and the table moves in such a way that your chest is inside a round circle. The scan now takes about one minute and the test is over. CT angiography will be able to tell your level of blockage and where they are situated. This test is little expensive (about Rs 10000/-) but is almost as accurate as the catheter coronary angiography. This test can be repeated after one or two years to know the reversal of blockages. This test has come to India 12 years back and many people are not aware of this test. Until now the machines are available only in metro cities. This is one of the tests, which have n o s i d e e f f e c t s , a n d S a a o l

recommends this test to many. This test has created a major problem for the heart hospital cardiologists as it competes with their own catheter angiography. They are not happy with this test. The main reason is that the patient comes to know of the blockages without admission in the hospital and they cannot be pressurized for the angioplasty or bypass surgery. U s u a l l y, a f t e r t h e c a t h e t e r angiography, the cardiologists prevail upon the admitted patients; put pressure on them and the relatives and make them agree for such treatment, which gives lot of money. On public demand, many hospitals have installed this machine as more people want to know about their condition from outside. But you talk to the cardiologist in the same hospital, they will not encourage you to undertake this test.

The latest scanner as on 2018 are 320 slice scans and using dual source cameras the scanners have become 640 s l i ce CT scanners . The advantage of Dual source cameras are that they can scan the heart in half a second (less than half a heart beat) and are very useful when the heart rate is very high and patient can not hold their breath for long. Since heart disease is the most common

and most fatal disease of the world and India – and is detectable only after the blocks have crossed 70% by symptoms and TMT(Exercise Stress Test) – CT coronary scan is the best way to diagnose heart disease in very early stage. Millions of heart attacks and deaths can be prevented by using this test. It has already replaced the crude method of inserting a tube in the body to diagnose heart blockages by 70%.

Saaol Times, April, 2018 Saaol Times, October, 2019Saaol Times, October, 20198

My name is Md. Bilayet Hussain, age 45 years and I am from Bangladesh. I am a businessman. In 2017 I developed Chest pain on walking and went to the National Heart Foundation in Dhaka. I could not walk at all – as I would get chest pain on slightest exertion. They admitted me. After that they performed test called Coronary Angiography and informed me that all the three tubes of my heart were 100% blocked. They gave me time of only 7 days for undergoing Bypass Surgery. They told me beyond 7 days my life will be under danger – I may die anytime.

I came to know about Dr. Bimal Chhajer from You tube and internet. He was in Delhi. So I called Delhi and straight came to Delhi Saaol Heart Center in Chattarpur. After meeting him I became

thvery confident and from 5 November, 2018 I s t a r ted h i s t r ea tment o f Medicines and diet. I gave up Non veg food completely and also started cooking all my food without oil. I also started ECP or Natural Bypass and Saaol

Detox Treatment (BCA). The ECP was for 40 days and Detox was for 20 drips

thon alternate days. On 14 December, 2018 I came back to Bangladesh after completion of my treatment. I was completely normal by that time – the chest pain was gone.

In March, 2019 I went back to India and got another Angiogram dome (CT Angio) in AMRI hospital in Kolkata. To my delight two of my 100% blocks opened – 0ne completely and other by 10%. Collaterals have also developed in my LAD. But my symptoms were completely gone. Now I am able to walk 6-7 km per day. I am following all the instructions of Saaol regarding food and performing Yoga exercises.

Since then I have been recommending my fellow patients not to go for Bypass Surgery or Angioplasty and follow Saaol treatment. I have taken Oral EDTA after March, 2019 and feeling much better. I pray to Allah for a Long life of Dr. Chhajer.

Evolution of CT Angiography

C T s c a n o r C o m p u t e r i s e d Tomography or Computerised Axial Tomography (CAT Scan) makes use of computer processed combinations of many X ray measurements taken from different a n g l e s t o p r o d u c e t h r e e Dimensional images of the scanned area. In 1979 Nobel Prize for Physiology or Medicine was jointly awarded to Allan M Cormack and G o d f r e y N Hounsfield for development of CAT scan.The first CT scan was done

ston October 1 , 1 9 7 1 i n a H o s p i t a l i n Wimbledon in London and was developed by an electrical engineer (G N Hounsfield). It was done for the brain. By 1998 the CT scan machines gradually improved – but could do Scanning of the organs which do not move. The CT Scanners were 4-8 slices. The first 16 slice Machine was developed in 2002 – which make CT Scanners more fast and accurate. But with this scanner also Radiologists were not able to see the coronary blockages as the heart was moving in a high speed. The Scientists made 32 slice

Scanners in the year 2005 and this is when Heart's coronary arteries could be scanned by CT Scan. This was the first time that without putting a catheter inside the body the blockages of the coronary arteries could be seen. The technology further evolved and in 2006 the 64 slice scanners were developed which could detect and measure the coronary blockages

much more accurately. After the development of 128 slice CT scanner Cardiac Imaging became very easy and within 5 seconds the radiologists can detect very accurately the blockages in the coronary arteries with a high resolution picture. These images can be rotated, dissected and studied individually. With the use of advanced softwares the image qualities can be further improved in 128 slice scanners – making the machines equal to 500 slice scanners.

In catheter angiography a long tube called catheter is pushed blindly inside our body through a cut made in the thigh(femoral artery) or the hand (radial or branchial artery). The wire is somehow pushed towards the heart by trial and error method(similar to the pushing of the clutch wire in a scooter or motorbike). During this time the catheter scratches and damages the inside wall of the tubes it passes through. It may also puncture any tube leading to the death of the patient. Once the tip of the catheter reaches the heart, a dye is used and scanning photography is taken. During this whole procedure continuous X-rays are done. One such test exposes a person to a radiation equivalent of 70-100 X-rays. Complications like false puncture, infection, embolism (which can lead to Paralysis once it reaches the brain), allergy and

even Heart Attack can take place during this procedure. I know a person who remained in the hospital for 8 months as the leg artery was damaged and infected after angiography. Ultimately, the legs had to be amputated. If you ever see the report of an angiography you will find the blockages are all in round figure 70%, 80% and 90%. These are all by estimations. Blockages cannot jump from 70 % to 80%. They can also be 77.92% or 82.30% if you measure t h e m a c c u r a t e l y - w h i c h i s possible(quantitative angiography). Most of the hospitals only do a qualitative angiography and give the report roughly. The reports vary from cardiologist to cardiologist even if they see the same CD records of angiography. Many of the cardiologists knowingly estimate the blockages as more, to create pressure for the treatment.

Saaol Times, October, 2019 7Saaol Times, October, 2019

The most preliminary test for the heart is ECG or EKG. This electr ical recording is non invasive and tells us about any shortage of blood when the patient is at rest . I t shows abnormal results only when the Angina (shortage of Blood to the heart) is at rest or during heart attacks. When this ECG is taken during exercise on a tread mill it is called TMT (Tread Mill Test) or Exercise stress test. The TMT shows positive (abnormality) only when the blocks cross about 70% in any of the heart arteries.

If the blocks are in the range of 40-50% TMT still shows normal report. More accurate test to detect blocks are Coronary Angiogram – which we are going to discuss in detail in the next few pages.
