5 Things you need to knowabout Medicare1. Coding Reform. PTevaluation and reevaluationcodes will be made public bythe AMA on August 4th andwill become effective Jan2017. Ten other codes havebeen identified by CMS forreview. These are codes that

have a payment of 10 million or greater. Some of thecodes have not been reviewed in 19 years. In Mostcases “review” has equated to reduced payment

2. Alternate Payment Model (ACOs and bundledpayment systems). The Department of Health andHuman Services announced the goal to have 30% ofMedicare payment tied to value based systems hasbeen achieved nearly one year ahead of schedule. April1st elective hip and knee replacement mandatorypayment bundle is hitting 67 metropolitan areas acrossthe country. See APTA website for details.

3. Post acute care reform. The model will facilitate

and promote the use and reporting of standardizedpatient assessment instruments, quality measures,electronic health records and participation in nationalregistries to provide essential data. There is a push tocombine to one payment system. The first report tocongress is due by June 2016 and the first prototype by2022.

4. 90% of reimbursement is scheduled to be tied tooutcomes by 2018

5. Medicare Part B. The PQRS requirement forphysical therapists in private practice to report 9individual measures (or up to 8 in some cases) viaclaims or registry will have a 2% penalty in 2017 if notmet. Other providers are moving to merit basedpayment system in Jan 2017 and PT may be includedin 2019. This will included penalties up to 7% for poorpractices and bonuses for good practices

JOHNSON CITY – An East Tennessee State University student seeking her doctor of physical therapydegree through the College of Clinical and Rehabilitative Health Sciences has been selected to serve on acommittee for a national organization.

Melissa Buehler, a second-year physical therapy student, has been selected to serve on the American PhysicalTherapy Association National Student Assembly’s Committee on Interprofessional Collaboration.

The mission of the committee is to promote collaboration, education and networking between students andprofessionals across various healthcare disciplines and specialties. The committee also aims to advance physicaltherapy students’ understanding of and experience with providing patient-centered care as part of aninterprofessional team.

“I plan to do what I can to knock down some silos in today’s healthcare model by supporting one that canbetter meet the needs of the patient by using a collaborative approach to care,” Buehler said. “I hope that myexperience participating in ETSU’s Interprofessional Education Program will provide me with valuable insightin order to move the committee toward accomplishing its mission.”

Buehler’s appointment is for one year.

TPTA Day on the Hill 2016The 2016 TPTA Day-On-The-Hill was a rousingsuccess! Despite terrible weather, we had almost200 participants this year. Students from everyphysical therapy school in Tennessee were therewith large contingents from Tennessee StateUniversity and the University of Tennessee-Chattanooga. A large group from East TennesseeState University was scheduled to attend but evenworse weather in the Tri-Cities area kept mosthome, although 5 intrepid souls still made thedrive over. And new program South College alsohad 5 students in attendance!

And we set a new record for DOTH legislativevisits with at least 83 visits being made by those inattendance! Great job to all those that attended.

We were also privileged to hear from Senator BoWatson who is also a physical therapist and

Speaker Pro Tempore of the state Senate. Hiswords of wisdom are always a welcome andimportant part of DOTH.

DOTH is animportant function ofthe TPTA efforts,launching legislativeactivities, developingadvocates andadvocacy initiatives,and buildingrelationships that can support issues important to allphysical therapy professionals.

I would like to inform the TPTA board andmembership that the Advisory Council on WorkersCompensation met 5 times in 2015. I was present atall of the meetings, while also attending 3 MedicalPayment Committee Meetings, and the Forum on“The Future of Workers Compensation in TN,”which was presented by the TN Bureau of WorkersCompensation. The Bureau of Workers Compensationis a part of the TN Department of Labor.

As some of you know, while others may beunaware, a physical therapist is appointed by thegovernor every 4 years to serve as a Non-VotingPhysical Therapist Representative, on the TennesseeAdvisory Council on Worker’s Compensation. Oneof the roles of the Advisory Council is to hear andreview statistics related to Workers Compensationclaims/costs while also making recommendations/suggestions on pending TN Workers Compensationlegislation.

There are 7 voting members and 9 non-votingmembers on the Advisory Council, representingemployers, attorneys, health care providers, amongothers. My role on the Advisory Council is torepresent the Physical Therapy profession in TN onall issues or possible changes pertaining to WorkersCompensation law. Even though it is a Non-VotingPhysical Therapist Representative position it affordsour PT profession the opportunity to be aware of,and have a voice in any possible proposed changesin TN Workers Compensation Legislation.

Although there was a significant overhaul of the TNWorkers Compensation system in 2013/14 (with

many of those changes being judicial/settlementchanges; and not going into effect until July 1st,2014), in 2015 there was no proposed legislation/changes in workers compensation law or policy thathad any impact on our profession as a whole. Theonly exception to that would be that the TN Bureauof Workers Compensation did choose to switchfrom the ICD-9 to the new ICD-10 diagnosis codes.

Although, not all states chose the switch from usingICD-9, TN chose to do so, and in reality thatprobably makes it easier on us as clinicians since weare primarily using ICD-10 with all of our otherinsurance payers, including it being mandated byMedicare. My understanding from our last meetingis that there may be some pending legislation thatwill be brought before the Advisory Council at ournext meeting in late January 2015, but I don’tanticipate anything that will have impact on the PTprofession….but I will keep you posted.

On November 9-10, the TPTA LegislativeWorkshop was held in downtown Nashville. Ibelieve this is the 4th installment of this reallyeffective advocacy program.

Part of our Strategic Plan, the LegislativeWorkshop has evolved and changed through theyears.

We planned the program for 20 physical therapistsand physical therapist assistants and agreed to payall expenses for all attendees.

When we first met to discuss this year's version ofthe program, TPTA President charged the planningcommittee to make half of those Early Careermembers (usually defined as those that are lessthan 5 years out of school).

When it came down to the final list of attendees,all but 5 were from that most valuabledemographic. The group proved to be energetic,engaged, and fully committed to advocacy efforts.

We started with a social hour in the Sheraton hoteland then met Senator Bo Watson at the restaurant.If you didn't know, Senator Watson is anincredibly effective legislator, a respected leaderin the State Senate, and a practicing physicaltherapist.

Senator Watson not only met the call to join ourLegislative Workshop group and address thegroup, he drove up from his home in Chattanoogajust for that purpose.

Let me say that a little differently. Senator Watson,who had no other reasons for driving to Nashvilleon that Monday evening, gave up valuablepersonal time to join us.

Not only did Senator Watson share valuableinsight into the legislative process, he joined us fordinner!

Although planned as an informal gathering, wemet after dinner back at the hotel and continuedour discussions on issues, advocacy efforts, andpending legislation.

Tuesday morning was a more formal setting as ourlobbyists from Smith Harris & Carr kicked off theday. Estie Harris has been our lead lobbyist formany years and is one of the most respectedlobbyists in Nashville.

Let me interrupt this to add something here.

Whatever you have heard about lobbyists just isn'ttrue at the state level. Maybe in Washington butnot in Nashville, lobbying can be a bad word. InNashville, lobbyists are important components ofthe legislative process, serving as a valuableresource for the legislators. And Estie is the best!

Joining her were Meagan Frazier and LouAlsobrooks. What you might not know is that bothMegan and Lou are also attorneys. In other words,we have incredible leadership and advocacy fromour lobbying firm.

Your TPTA President Scott Newton and VicePresident Alan Meade provided insight into thelegislative process. Both have been integralcomponents to our advocacy efforts for well overa decade.

We ended Tuesday with a most interesting tour ofthe State Capitol provided by the staff of theTennessee State Museum, followed by a photo opin the Senate Chambers.

The post-meeting reviews were overwhelminglypositive and I truly believe we have openedavenues for effective advocacy for many years tocome for those in attendance.
