2015 was another year of accepting change as it came...some expected, some
not. We continued to see changes in the healthcare marketplace on a local,
regional and national level. We adapted to the changes and always kept the
healthcare needs of our patients as a priority.
We have always met our patients where their lives are at the moment of service.
Commitments are made to treat each individual with the same care and kindness
we would provide our own family members.
We do this day after day, one patient at a time. At the end of every year we look
at the numbers and are continually amazed with the outcomes.
Our great team of medical and dental providers, along with the Crusader
Community Health and Crusader Foundation boards, remained steady to their
commitment of the Crusader mission and provided high quality health and dental
care with respect and dignity.
During the past year, we’ve made significant upgrades to many of our facilities.
These improvements made the patient experience, as well as our employee work
environments, more enjoyable and safe. 2016 will see the building of our sixth clinic
in the heart of Rockford -- where patient need continues to grow.
As it has been for our entire existence, the environments in which many of our
patients are born, live, learn, work, play, worship, and age are unique. They affect
a wide range of health and quality-of-life issues. Social and economic challenges
that our patients face are often difficult for many of us to understand. Yet our
medical and dental providers successfully address these challenges every day.
Crusader continues to enhance the quality of life for our patients. We give patients
reassurance and hope. Our teams engage our patients in a unique manner. The
results of our most recent Patient Satisfaction Survey indicate that our patients
understand and appreciate the many benefits offered them by Crusader.
The Crusader management team and I continue to be optimistic about the years
ahead. We will tackle each future challenge in the manner that has worked well
for us for over 44 years. Plan well for tomorrow. Be present each day. Treat every
patient with dignity and respect. Work hard and have fun. It’s The Crusader Way!

Christina Emmert, was the recipient of the IFLOSS Coalition Oral Health Champion Award for 2015. Cristina’s dedication to Crusader’s mission and her commitment to oral health has been serving Crusader patients for over 17 years.
Nancy Halverson, VP received the 2015 Angel Award from Carpenter’s Place. The award was for Nancy’s contributions to the development of policies and practices at Carpenter’s Place; for both internal controls and services to homeless adults.
Sam Miller, CPA, CFO was the recipient of the 2015 Jeffrey T. Latman Leadership in Health Care Finance Award. The national award honors excellence, leadership and integrity in strengthening health center fiscal operations.
Shelton Kay, VP Community Relations, received the 2015 Ramona Lopez Community Health Center Employee Award for his commitment and leadership in improving access to the quality of care for Crusader’s most vulnerable patients.
Sharon Mulhall, RN, CDE, received the 2015 Crusader Advocate Award for her service to patients and co-workers. For over 21 years Sharon has encouraged Crusader diabetic patients to be advocates for their own diet and medical care.
Students from Winnebago High School and McNair Elementary School donated 3,400 books to be placed in the Crusader “Take-A-Book, Leave-A-Book” libraries outside Crusader on Broadway and West State Street. Books are meant to inspire reading for Crusader’s many pediatric patients.
For over 20 years the Cosmopolitan Club of Rockford has made a cumulative donation to Crusader of nearly $500,000. On left, a check for $20,000 was for Crusader’s 2015 patient supplies. Enthusiastic, young gardeners from Rockford Urban Ministries-Rockford Work Camp project helped beautify the Crusader Muldoon Grove community garden. Summer mission trip work teams complete various projects during their stays in the Rock River Valley. Patrons of the Crusader West State Street Muldoon Grove benefited from their hard work!

2015 saw significant upgrades to Crusader facilities. The lobby at Crusader on Broadway was reconfigured to ensure patient confidentiality and streamline the check-in process.
Crusader in Belvidere was remodeled to add more patient exam rooms.
The employee and patient parking lots at Crusader on West State Street were reworked for more efficient parking and lighting upgrades. The patient entry at West State Street was remodeled with automatic doors and carpeting.

CRUSADER ON ALPINEGrowing to meet the needs of the community.
During its 44-year history Crusader Community Health has responded to the diverse healthcare needs of one in five area residents.
Over the years Crusader has witnessed a steady growth in patients. In 2015, Crusader helped 48,234 individual patients who had 205,934 health care visits.
Today, Crusader has five facilities at strategic compass points located throughout our service area.
The construction of a 21,000 square foot single story facility is projected to serve 4,500 clients within the first two years. The new facility will improve clinical efficiency and relieve patient demand at other Crusader facilities.
This space will allow for future expansion as demand for care increases. The development of the new clinic fits well with Crusader’s mission to provide healthcare access to the medically underserved in our community.
At the core of the decision to expand Crusader are three important facts:
1. Crusader is experiencing an un- precedented demand for care that warrants the construction of a new clinic in the center of Rockford.
2. Crusader has stretched the capacity offered by its current locations.
3. Facility expansion at current Crusader sites is not an option as these locations are land locked or not suitable for expansion.
Adding on to, or upgrading existing Crusader facilities is not possible due to land constraints. Engineering studies reveal it is nearly as costly to renovate
an existing structure as it is to build a new one. An efficient, single story
structure will enable Crusader on Alpine to meet
community medical needs well into the
future.
A high density of low income individuals, not served by health centers, has been
identified for zip code areas in the new site
location.

at 1215 North Alpine...at the northeast corner of North Alpine and Guilford Roads.
The familiar, high-traffic location is on established city bus routes, has ample
parking and has been home to a medical clinic since 1972. People are accustomed to medical services on this familiar corner.
Because Crusader accepts most insurance, anyone can access medical care. In addition, a sliding discount scale is available for medical services.
This project will address need by offering preventive, acute and chronic disease management healthcare for:• Pediatrics• Family medicine• Internal (adult) medicine • Women’s health services
Special walk-in clinics for both children
and adults will be offered. The total cost of the new facility including land acquisition, construction and equipment is $7.1 million.
Crusader on Alpine is now scheduled to open early 2017.

Over 525 loyal Crusader fans filled the Giovanni ballroom to celebrate Crusader’s 43rd anniversary on Friday, October 16, 2015.
The theme TEAMWORK MAKES THE DREAM WORK brought out the competitive team spirit and creativity as attendees dressed in
their favorite team costumes and colors.
In the height of the football season, even Chicago Bears and Green
Bay Packer fans put team rivalries aside for one evening to support Crusader Community Health. Many other professional, college and prep teams were well represented. There were awards for most creative team display.
Besides sports teams, many chose famous teams from Hollywood, TV
or history. Lucy Riccardo & Ethel Mertz showed up to the
delight of everyone.
After a delicious dinner, co-emcees Mimi Murphy and Mike Robinson entertained the crowd with their own special Team-TV Anchor humor.
Net proceeds from the event, totaling over $209,000, will support the new Crusader on Alpine facility. The new 21,000 square foot facility will address the primary healthcare needs of Crusader patients within the center of Rockford.
A raffle with great prizes, along with a silent and energetic live auction entertained guests and
helped support the new clinic.

Crusader Community Health Foundation
initiated the Spirit of Caring Award in 1992 to honor special people in our community who have demonstrated outstanding efforts in caring for others.
In 2007, we added a Spirit of Caring award for an organization that exemplifies a mission of giving back to others in our
community.
Two years ago, the Spirit-of-the Future award was started to honor persons 40 years and younger for their caring efforts.
We salute all nominees who were nominated by family, friends and co-workers in recognition of their care and concern for others.
For 2015 one person received the Spirit-of-the-Future Award and three individuals and one organization received the Spirit of Caring Award.
JESSICA RIELLY 2015 Spirit-of-the-Future Award
Jessica Rielly was nominated by MELD in recognition of her dedication as a board member. She has helped MELD raise awareness of its programs and takes a leadership role in fundraising. In addition, Jessica is active in IGNITE Group for young professionals. She is also a member of the Rockford Area Realtors Young Professionals group who recently organized the Rock the Block Broadway beautification project to enhance the landscaping along specific blocks of Broadway.
KAREN BROWN 2015 Spirit of Caring Award
Karen Brown is recognized as a caring leader in the healthcare industry. While being a leader and role model to others she is also known for encouraging professional growth and development to the staff at OSF Saint Anthony Medical Center where she is a VP and COO. She has held numerous community board positions including the YWCA of Rockford, Byron Chamber of Commerce, and Discovery Center. In addition she is active in Transform Rockford, Alignment Rockford and the American Heart Association.
ROGER GREENLAW, MD 2015 Spirit of Caring Award
Roger Greenlaw has devoted his time and talents to the Rockford community through his involvement as a founder of Rockford Gastroenterology, and his long time support of the University of Illinois College of Medicine at Rockford, the Coronary Healthy Improvement Project, Fit Families and the Jeremiah Development Project. Thanks to his deep commitment to caring for the health of others, along with his passion for the community, several highly impactful community projects were developed and continue to thrive today.
KATHLEEN KELLY, MD 2015 Spirit of Caring Award
Over a long and successful career, Dr. Kelly has worked to improve the healthcare quality of people throughout northern Illinois. Through her leadership roles at SwedishAmerican Health System, Dr. Kelly implemented quality improvement initiatives, introduced holistic health programs and initiated wellness services. Her nominator stated, “she is an unsung hero who cares for many people selflessly and privately.” Dr. Kelly is a member of the Crusader Community Health Foundation Board and active community volunteer.
REMEDIES 2015 Spirit of Caring Award
Remedies has been providing substance abuse and addiction services within our
community for over 60 years. They also open their doors to victims of domestic violence. Remedies mission is to support victims of domestic violence as they begin the recognition and recovery process and plan for a future free of abuse. Remedies offers case management, education, advocacy, and emergency shelter for victims of domestic violence. Remedies’ shelter is open 24 hours a day, 7 days a week, 365 days a year.
