Cerebral blood flow autoregulationIn normotensives an increase in MBP over a certain range (60-125 mmHg) induces cerebral arteriolar vasoconstriction, thereby preserving a constant cerebral blood flow and an intact blood-brain barrier
A chronic intraluminal pressure stimulates the growth of smooth muscle cells and enhanced media thickness in resistance arteries that results in hypertrophic remodeling. Hypertension influences the autoregulation of cerebral blood flow by shifting both the lower and upper limits of autoregulatory capacity towards higher BP, while hypertensives may be especially vulnerable to episodes of hypotension

• Resultados NO CONCLUYENTES o CONFLICTIVOS

• Control estricto PA primeras 24h.

Reappraisal Guidelines 2009
Mancia G, et al. J Hypertens 2009
• No studies support a recommendation to initiate BP lowering treatment in cardiovascular patients with BP in the high normal range (benefits of treatment were seen only in individuals with a baseline SBP of 140mmHg and above, who often were on antihypertensive drugs already).
• Nor can the PROGRESS data be taken to support a SBP target below 130mmHg, as the average SBP achieved on more intense treatment was 132mmHg. However, the trial did show that an on treatment SBP of 132mmHg was better than an on-treatment SBP of 141mmHg (the average SBP of the placebo patients).

Summary of Main Longitudinal Studies on the Relationship between Antihypertensive Drug Use
and Risk of Dementia
• An association between mid-life HTN and late-life cognitive decline or dementia has been found in the majority of observational studies
• Observational studies point to some benefit of anti-HTN treatment on risk of AD
• The longer the duration of treatment, the stronger the preventive effect
• Treatment seems more effective in the youngest old than in the oldest persons
• Few studies suggest a greater effect of some classes of antihypertensive therapy, but evidence to date is limited and subject to bias

Summary Regarding Blood Pressure Lowering and Cognition
• Observational studies point to some benefit of anti-hypertensive treatment on risk of AD
• Few large blood pressure lowering trials have incorporated cognitive assessment or dementia diagnosis
• Meta-analyses neither prove nor disprove the efficacy of anti-hypertensive treatment on dementia risk
