Siddiqui Memorial Lecture by Prof. Khalid Masood Gondal, Senior Vice President, CPSP. Prof. Aamer Zaman Khan, Councillor, CPSP, was the Convener of this Conference. The conference was attended by heads of medical institutions including Lt. Gen. (Retd) Syed Afzal Ahmed, Principal, Army Medical College, Maj. Gen. (Retd) Prof. Muhammad Aslam, Vice Chancellor, University of Health Sciences, Lahore, Prof. Mahmood Shaukat, Principal, Allama Iqbal Medical College, Prof. Hamid Mahmood Butt, Principal, Services Institute of Medical Sciences, Councillors of CPSP, senior professors and a large number of distinguished College fellows.

Prof. Zafar Ullah Chaudhry, President CPSP thanked the Chief Guest for having spared his time to celebrate the Charter Day of CPSP. Prof. Chaudhry said that the genius of founding Fellows deserve to be paid compliments as they not only got the Charter of CPSP approved as an autonomous institution managed by its Fellows, but in developing the syllabi of various specialties when they had no model to follow. We now do have access to other models of Competence and can get access through internet and even video link while they had no such infrastructural facilities.

College has developed its own model of Competencies in the light of other models of competencies evolved by Royal College of Canada, ACGME and by Prof. Harden too. But the founding Fellows had no such access. Even access through phones took such long that it is hard even to conceive in the contemporary world of internet and Optic Fiber era. Yet they did it and did it wonderfully, said Prof. Zafar Ullah Chaudhry,

The President elaborately described the development of education systems as evolved in the West with the onset of Renaissance, the Protestant movement and explained the evolution of Word “College” itself.

He pointed out that College and Universities both conduct research but the difference is that the research conducted at the College is based on studies focused on clinical data collected in their specialty and is quantitative while research at universities is focused on innovations.

The College is now focused to train its trainees in keeping with its own model of competencies which is best suited to our socio-economic and social setup, said the President.

Prof. Chaudhry further elaborated the development of Competency Model of CPSP saying that the focal point of the Competency Model is Patient Care as is in all other models of competencies. But patient care demands that the specialist must have appropriate:

• Knowledge• Critical reasoning• Communication skills• Team work• Research

Prof Zafar Ullah Chaudhry added that CPSP demands

that its trainees should acquire following Meta competencies too, which are

1. Andragogy2. Professionalism 3. Advocacy

Prof. Chaudhry added that this framework articulates generic competencies enabling competencies to be outcome specific and align with both, Instructions and Assessment.

He further emphasized the role of SPECIALIST FACULTIES and the supervisors of each specialty to indentify enabling outcomes for each phase of training and strategies to achieve them.

President then focused on the Assessment tools for the CPSP model of competencies. He elaborated the Workplace Based Evaluation where on-site assessment is mandatory.

He added that WPBA tools are accompanied with structured feedback. These serve as formative feedback and instructional tool.

He further detailed with the help of an Assessment Blue Print of an orthopedic chart of Workplace Based Assessment tool. He went on to point out that Faculties need to further deliberate in keeping with their own specialty and develop the details of structured training

for all phases of training which should • Identify enabling competencies/outcomes• Strategies to attain the competencies • WPBA tools particularly Mini Cex and DOPS• Timeline for implementation.

President’s address was complimented by a presentation of Prof. Muhammad Aamer Zaman on Assessment in Competency Based Model. He described how CPSP Model of Competency was different from other models and how it best suited to the genius of our local socio-economic pattern.

He defined Competence entails more than the possession of knowledge, skills and attitudes; it requires application of these abilities in clinical environment to achieve optimal results. He told the audience that there are three models, Apprenticeship model, Process based model and Competency based model.

Prof. Aamir Zaman said that well known Competency models are of ACGME, Canmed and CPSP. Prof. Zaman went on to describe the main points of Assessment in Competency Based Model. He described Miller’s Triangle and how Cognition turns into behavior.

He said that the base is knowledge and Clinical part both, assessed through MCQs, essays and oral. When performance assessment is to be made, the tools are OSCE, Simulated Patients (SP) and the top of the Pyramid is assessed by undercover Simulated Patients, Videos and logs.

AdvAnced SkillS courSeATLS CourseAdvanced Skills Department of CPSP Lahore conducted an ATLS Providers Course running from 16th to 18th May with Prof. Mahmood Ayyaz, National Director while Course Director was Prof. Moeed Iqbal Qureshi and instructors were Prof. Ghulam Mustafa Aarain, Prof. Ahsen Nazir Ahmed, Dr. M.Waris Farooka, Dr. Asher Ahmed Khan, Dr. Tahir Hamid and Dr. Reema Sehar Ali. The Course was conducted for fifteen (15) participants.

PALS Providers CourseA PALS Providers Course was conducted for Twelve Participants by Prof. Kamran Khalid Khawaja, Dr. Col. Dean and Secretary elected

New Faculty of Medicine meets.

The newly elected and finalized Faculty of Medicine met on 8th May 2015 at the Academic Block of CPSP Head office, Karachi at 2 pm with Prof. Shoaib Shafi as the Council representative in the Faculty.

president’s addresses to Faculty:Prof. Zafar Ullah Chaudhry welcomed the Faculty and congratulated Prof. Jahangir Khan and Prof. Nayyer ul Islam on their election as Dean and Secretary. He told the Faculty that 70% of fresh trainees are applying for Medicine and Allied. This Faculty thus acquires the status of a parent faculty and has great responsibilities.

He urged the Faculty to take care and keep-up the standards at the time of induction of the trainees; use SVCD as tool of training as well as keep it updated at least every two (2) years. He stressed that E-Logbook should be considered as monitoring tool not only of trainees but of the supervisors too as it provides regular feedback of both. He urged the supervisors to keep themselves updated by participating in a workshop in which they wish to revamp themselves. CPSP has declared revamping of supervisors through participation in workshops free. They can participate in the entire workshop or any part of it. He also asked the Faculty to see that the supervisors do regularly participate in CME and CPD programs as it is the only method to keep themselves updated.President asked the Faculty to see that MINI CEX and DOPS are tools to assess the trainees at their workplace are used appropriately as it will help us improve the pass percentage in FCPS II. He further shared that College will be introducing online exams from January 2016 which will reduce pressure on logistics and also save time. President assured the Faculty that any of its members can have access to examinations as well as to MCQ bank to make necessary changes in existing pool of MCQs in keeping with the examination format.President stressed to make the research more meaningful for the trainees. Responding to the queries of Faculty members Prof. Zafar Ullah Chaudhry told that Faculty can present their point of view in College Council through its Dean and Secretary. He also told that if faculty wants to introduce changes in evaluation then please discuss it among faculty first and then propose it to the examination cell of the College. Prof. Khalid Masood Gondal asked the Faculty to add to the efforts of the outgoing Faculty by strengthening, empowering and delivering academically as well as administratively. He added that as trainees are increasing day by day, we do not want the training programs to get diluted.

Prof. Khalid Masood Gondal asked Faculty to focus on three important issues. • How to monitor the role of supervisors.• Trainees input should be continuously assessed whether

the training is satisfactory or not.• Does the faculty want to continue with Group A & Group B?

The Faculty elected Prof. Jahangir Khan as Dean and Prof. Nayyer ul Islam as Secretary of the Faculty.

Prof. Shoaib Shafi, Council representative in the Faculty said that CPSP believes in collective wisdom and faculty’s help is paramount. He emphasized on arranging Videoconference meetings at centers to involve greater number of supervisors at Regional Centers so that faculty can have their input too. Prof. Shafi added that curriculum is of great interest since this is guide for allied specialties too for the first two years until IMM. He felt that this area needs to be focused so that we may uplift the existing standards of fellowship training programs. He also felt that it is important to further improve relationship between supervisor and trainees. He felt that measures like regular journal club meetings could be of helpful. He felt that there should be tools to assess supervisors too. Prof. Shafi said that results of IMM have improved while the pass percentage in FCPS II has declined. Most failures are in short cases. He felt that examination pattern should be changed so that the bias factor, if any, could be further eliminated.

Prof. Shafi posed the question to the Faculty if they still want to continue with the groups setup. He added that once the candidate selected the specialty, is it necessary that we should ask the candidate to undergo with the mandatory Four (4) workshops. He also asked the Faculty to propose new examiners for the College who would perform the task willingly. He further asked the Faculty to deliberate upon the question that should we allow a candidate to proceed further in exams if he/she fails in TOACS.

Faculty under the Chairmanship of the newly elected Dean proposed that • There should be no Group A and Group B. One group

should be General Medicine and other being the specialty group. Students should be given a chance to come back to the General Medicine.

• Main objective of IMM training is to gain core knowledge of General Medicine so the rotations should be limited to four (4) out of which three (3) be taken in first year where as each rotation should be of 6 weeks and trainee should spend last 2 months in General Medicine.

• CCU and ICU rotations should be mandatory. CCU be taken before IMM and ICU after IMM.

• Faculty asked the College to help faculty evolve tools of WPBA.

• Faculty constituted Two Committees.

Prof. Khalid Masood Gondal, Senior Vice President and Regional Director Lahore gave a detailed briefing about the activities and programs of CPSP. He informed the visiting dignitaries that CPSP is keen to assist the Health department by providing specialist force of CPSP for District and Tehsil level Hospitals and healthcare units.

He gave a detailed program and made suggestions on behalf of CPSP. Prof. Zafar Ullah Chaudhry, President CPSP, Prof. Naqibullah Achakzai, Vice President CPSP, Prof. Syed Khalid Ashrafi, Treasurer CPSP and Prof. Mahmood Ayyaz, Prof. Aamir Zaman, Prof. Shoaib Shafi, Prof. Asghar But, Major Gen. Salman Ali, Prof. Mohammad Tayab all councilors, along with Dr. Abrar Ashraf Ali, Dr. Waris Farooka were also present at the briefing.

Khawaja Salman Rafiq said that CPSP is the pride of Pakistan and we all do feel proud of having such an institution.

He promised that the Provincial Government has and will provide all assistance to CPSP in its endeavor to provide Specialist doctors to the Nation.

He promised that he would urge the Chief Minister of Punjab to allocate appropriate plot to house CPSP. Provincial Secretary Health Mr. Rafiq Malik hailed the scheme to provide specialized doctors for District Healthcare Centers and urged CPSP office bearers and councillors to work on the plan so that specialists doctors could serve at District Healthcare centers too.

New Faculty for Surgery meets Surgery is the mother of many specializations

President Zafar Ullah President CPSP told the Faculty of surgery that there are many specializations of Surgery and therefore it is important that as parent faculty it should be keen and conscious in bringing any changes in the curriculum. President welcomed all the members of the Faculty on behalf of the College Council as well as in his personal capacity; being a surgeon of repute. The Faculty met on 9th May 2015 at the Academic Block of the College Head Office.

President informed that 65% of the trainees are in public sector where they feel scarcity of resources and paucity of proper management. Keeping that in view he felt that there are Four (4) elements of basic requirement that need urgent attention of Faculty. These are

1. Curriculum,2. Training,3. Stipend and4. MCQ bank

He emphasized that the faculty needs to work hard

in improving existing system. He further added that College has the tools of CME and CPD which helps keep supervisors improve and validate their knowledge and skills continuously. Now College is offering free access to all the workshops for supervisors and their participation in workshops will also provide CME credits to them.

introduction of Workplace Based Assessment:President further went on to explain the importance of Workplace Based Assessment (WPBA) since ongoing training can be assessed on realistic grounds which also helps to improve clinical skills of the trainees.

He informed that this assessment tool can be uploaded on E-Log system and to begin with you are being provided with Forms so that initially it can be run on quarterly basis in the institutions.

Prof. Khalid Masood Gondal added that most of the elected members of this Faculty have earned MCPS in HPE which will be an added advantage and the faculty would be richer with their knowledge and learning making Faculty stronger.

Dr Siraj, Director DME, in the light of the President’s discourse, suggested making a pool/registry of standardized patients for the examinations. He suggested making registry of:

The Faculty also proposed to meet on 5TH June 2015 to evolve a “Workshop on Construction of MCQs”.

Following points were kept pending for discussion in the next meeting of Faculty.

The Faculty concluded its meeting with a vote of thanks to President, Vice Presidents members of the Council and the Dean who gave their valuable suggestions to the Faculty.

CPSP invites applications from MCPS trainees for Qarz-e-Hasna for laptops.

CPSP Trust is inviting MCPS trainees to apply for Qarz-Hasna to purchase laptops. The Trust in the recent meeting noticed that MCPS trainees are not applying to avail this facility, while FCPS trainees are availing it.

The Secretary of the Trust has notified that all MCPS trainees registered with RTMC are eligible to avail this facility. Every applicant can secure an amount of Rs. 35,000/- to purchase a laptop. The amount is payable through seven (7) monthly postdated checks starting after three (3) months of securing loan. The applicant will purchase the laptop on his own and will bear the additional cost him/herself in case the cost of Laptop is more than Rs. 35,000/-The prescribed application form can be downloaded from CPSP website. Completed application forms be submitted to the Secretary of CPSP Trust through concerned Regional Director or Secretary CPSP (if the applicant is from Karachi).

Faculty of Ophthalmology is the eyes of CPSP (Zafar Ullah Chaudhry)New Opthalmology Faculty Meets President welcomed the new Faculty of ophthalmology. He said that all Faculties are ears and eyes of CPSP while Ophthalmology is the focused eye of College. He added that the work of this Faculty as the focused eye of CPSP should be a role model for other Faculties.

President explained the reasons for having ATLS as mandatory workshop for ophthalmology trainees and said it is beneficial for the trainees. President asked the Faculty to suggest a cost effective alternative of ATLS for the trainees of Ophthalmology in keeping with the benefits of ATLS. President suggested that Faculty should consider redesigning all workshops for trainees and supervisors and cut them to three (3) days; yet making them prolific for the trainees.

President discussed the usage of web TV among trainees and supervisors as video link for monitoring and supervision. He suggested compilation of a pool of programs for at least three (3) months so that programs are not exhausted. In this regard, CPSP is establishing two (2) studios; one (1) at Head office, Karachi and other at Lahore Regional Center.

President suggested the Faculty members to act as coordinators at their respective Regional centers and collect as many ophthalmologists as they can to work collaboratively. He also emphasized the role and importance of College’s “Integrated Research Training Of Clinical Skills” program and compared it with other competitive programs like USMLE and CANMED.

President went on to discuss the importance of Work Place Based Assessment (WPBA). He suggested that Faculties should devise tools for WPBA as it will not only strengthen the training programs but will also help assess trainees at their workplace. To begin with, Faculty can take help of MINI CEX and DOPS forms and run them in the institutions as a pilot project and observe the difference in training of trainees.

President said that there are sixty-five (65) institutions where trainees of Ophthalmology are being trained and out of these, 65% of the trainees are in public sector while the rest 35% are divided with 15% in private and 20% in Army institutions. He further informed that total number of registered supervisors in Ophthalmology is 136 out of which 79 supervisors are in public sector institutions whereas armed force and private sector have almost equal number of supervisors. There are a total of 474 trainees in Ophthalmology all over the Country; of which two third are in Public sector institutions while one third are in private sector and Armed forces institutions.

He said that upcoming trainees in ophthalmology are reduced by 40% which is alarming and to which Faculty should address. President added that Faculty will have access to MCQs bank to contribute or to make any changes in existing pool. He further added that CPSP is planning to shift its examination system online; it will not only save time but will also help in minimizing the logistics. We will also be able to allow maximum number of participants appearing in examination and would be able to tabulate timely results.

President Zafar Ullah Chaudry told the Faculty that College plans to develop a standardized patient bank out of which some of the patients are real ones with established chronic diseases while others will be simulators who have under gone training for the same. This would help examinations of the College.

President also informed that CPSP is under approval phase of accreditation of ACCME for awarding CME and CPD credits. Thus College will become a source of providing/sharing items to sister Colleges which will not only generate revenue but will also provide a platform for educationists to collaborate at international levels.

The Faculty elected Maj. Gen. Mazhar Ishaq as Dean and Prof. Zahid Kamal Siddiqui as Secretary of Faculty.

To review various curricula, committees were formed. Prof. Zahid Kamal Siddiqui was named as Focal person with Dr. Iftikharul Haq Tareen to look into curriculum of FCPS I.

For MCPS curriculum, Prof Muhammad Khalid bin Saleem was selected as Focal person with Dr. Iftikharul Haq Tareen, Ch. Javed Iqbal and Dr. Mir Ali Shah.

Similarly for FCPS II Brig. Muhammad Amer Ayub was selected as focal person with Prof Shahid Azeem Mirza and Prof. Shahid Jamal Siddiqui.

To look into and redesign the SVCD charts the task was assigned to Ch. Javed Iqbal.

To take care of TOACS, Dr. Muhammad Rashad Qamar and Ch. Javed Iqbal were selected while for Primary Surgical Skills, Prof. Zahid Kamal Siddiqui and Dr. Muhammad Rashad Qamar got the task. Ch. Javed Iqbal will review the MCQ Bank.

The Faculty suggested that video link meeting of the Faculty be held every three (3) months while in person meeting be held once a year. Faculty also felt that ATLS should not be mandatory workshop for Ophthalmology, while BLS workshop be mandatory for this discipline.

Standing Committee of National Assembly visits CPSP

The Standing Committee of National Assembly of Pakistan on National Health Services paid a visit to College of Physicians and Surgeons Pakistan at its Head Office on 9th May 2015.The Committee led by Mr. Khalid Hussain Magsi , consisted of Mr. Ramesh Kumar Vankwani, Ms. Shakila Luqman, Dr. Muhammad Azhar Khan Jadoon, Dr. Nikhat Shakeel Khan, Col. (retd) Dr. Amirullah Marwat, Federal Secretary National Health and Regulation Mr. Muhammad Ayub Khan Sheikh.

The visiting members of the Standing Committee were welcomed by the President CPSP Prof. Zafar Ullah Chaudhry, Senior Vice President CPSP Prof. Khalid Masood Gondal besides other office bearers and Members of the Council. President briefed the Standing Committee about the College through a detailed multimedia presentation. Prof. Khalid Masood Gondal, Senior Vice President CPSP briefed the Committee on the International aspects of the College.

The Standing Committee was highly appreciative of the contribution of CPSP in the field of higher and postgraduate Medical Education in Pakistan. The Standing Committee was especially impressed by the fact that College through its own means financed to develop Thirteen (13) Regional Centers in even far flung areas of the Country. All these centers are linked through the Video link with Head Office. They also appreciated the fact that College does maintain Two (2) Overseas Regional Centers at Riyadh (Saudi Arabia) and Kathmandu (Nepal). The Standing Committee members were highly appreciative of the amazing utilization of IT in the day to day reporting of all

the 21,000 trainees of College with their supervisors as well as to the College too.

The Standing Committee also appreciated the fact that the College’s Degrees and Diplomas are recognized by other Postgraduate Institutions and Healthcare centers in Saudi Arabia, UK, Singapore, China, Ireland and others. The Standing Committee was impressed by the management of College affairs through a highly transparent democratic manner by the Fellows of the College itself.

The National Assembly Standing Committee told that they would suggest all the Provincial Governments to involve College in various health related matters and also involve CPSP in expansion of Emergency Medicine Fellowship Program.

The Standing Committee also recommended the formation of a Five (5) members committee of CPSP which may act as an Advisory Body to the Federal Government on Health related matters.

The Committee also suggested CPSP to establish new sub regional centers especially in KPK and Baluchistan Provinces.

The Committee visited some of the departments of the College and saw the functioning of these departments themselves.

President thanked the Standing Committee for their visit and also thanked for their valuable suggestions to CPSP as well as to the National and Provincial Governments.
