Main components:
- Community Engagement: AHSC offers a continuum of social and support services in a culturally sensitive set-ting by reaching out to Asian immigrants, reacquaint-ing them with the arts and culture of their homeland, providing opportunities for socializing, and fostering a sense of community. Our culturally relevant and linguis-tically appropriate community outreach includes our popular ethnic meal program for seniors and short term case management program.
- Health Integration: AHSC provides series of public health education projects and activities, health educa-tional workshops and psychosocial support programs which include: Health and Wellness Groups, Asian Wellness Connection, Chronic Disease Link, Living Well with Chronic Disease Training classes, Powerful Tool for Caregiving training classes, Women’s Health and mam-mogram referral, Emergency Preparedness and Healthy Kids application assistance.
- Health Care Services: AHSC has a state-certified outpa-tient Behavioral Health clinic specializing in serving the Asian community. Culturally and linguistically appropri-ate clinical services are provided by a team of bilingual professionals and psychiatrists. Collocated in the same facility, clients may also access onsite, low-cost Naturo-pathic and Chinese Medicine clinics. The Annual Asian Community Health Fair and the OHSU Global Health Center: interdisciplinary Community Health & Education Exchange (iCHEE) aims to encourage better access and preventive care.
Specific integration efforts already underway at AHSC are as follows:1. Asian Wellness Connection (AWC) provides coordi-nated care for clients from partnered community clinics with chronic disease management and wraparound so-cial services, support groups and coordination between the mental health clinician and primary care providers.
Over 500 individuals and families benefited from this project ( Three years funding support from UW, NWHF just ended in June 2012).2. Enhanced ethnic outreach, including ethnic meals for Asian seniors twice a week. AHSC provides case management to clients, referral for community re-sources and connect them with their primary care providers. 45,445 meals were served since the start of the meal program in April 2009 (Serving Mult. Co residents with funding support from Mult. Co).3. AHSC has partnered with the OHSU School of Nursing, whose student nurses provide a number of screenings onsite including blood pressure, weight/Body Mass Index (BMI), and blood glucoses checks to patients in need of close monitoring. Starting this Oc-tober, partnering with the OHSU Global Health Cen-ter: interdisciplinary Community Health & Education Exchange (iCHEE), AHSC will expand services aim to encourage better access and preventive care. (On go-ing partnership).4. Tri-County Mental Health Connection for Asians: a one year pilot project started in July 2012. Team up trained community health workers with our mental health clinicians to outreach, educate the Asian com-munity on mental health/illness. Basic screening will be provided and seamless enrollment to our out-pa-tient mental health clinic. The pilot project will benefit over 100 people (Funded by UW for 12-13).
Highlight of AHSC’s output on Integrated & Holistic Services Model (from Jan to Dec 2011): 1. Provided 59,538 service encounters, including 4,674 hours of case management, 1,111 hours of interpre-tation, 1,739 hours of information and referral, 491 hours of consultation and counseling to community members, and 3880.33 clinical hours of mental health services. 67% female and 33% male. 2. Accumulated 6,516 registered clients; large per-centage of them are low income with low English pro-ficiency. About 70% are OHP clients and more than 20% are uninsured.3. AHSC’s clients’ ages: approximately 11% of 19 to 29, 25% of 30 to 49, 40% in the range of age 50 to 69 and 22% of 70 to 80 & above.4. Most clients are emigrated from China, Korea, and Vietnam, majority of them are resided in Multnomah County, Washington County and Clackamas County.

AHSC provides community engagement services to help
community members live happier and healthier. We found that people
who are engaged actively in our center’s services and activities are
more able to maintain better health. They have a sense of belonging and
identity, they are able to expand friendship, they are more willing
to help others, they know how to connect to arts and culture, they
feel more secure and stable, they express more laughter and joy…….

AHSC provides culturally and linguistically appropriate public health and social services to help community members receive bet-ter care. 11th Annual Asian Community Health Fair
We are excited, yet humbled to share that this year’s health fair was a great success! The Asian Health and Service Center (AHSC) hosted the 11th Annual Asian Community Health Fair on Saturday, August 18, 2012 in Southeast Portland. Together with our 283 community partners who range from health care professionals and sponsors to bilingual volunteers, we attracted 1200-1500 to the fair and served over 505 people, and provided 2,049 free health screenings and health services. The array of screenings provided this year include blood glu-cose (diabetes), blood pressure, hearing, dental, vision (i.e., glaucoma), cholesterol, mental health, naturopathic health, women’s health, and various OHSU Let’s Get Healthy screenings. Other invaluable health services we provided at no cost include body mass composition measurement, Chinese medicine consultation/acupuncture, and immunization vacci-nations. Compared to last year’s health fair, there was a 21.6% increase in registration for needed services, and 808 more screenings were administered. We are pleased to have served the Asian communities in the Portland metropolitan area with efficacy thanks to the volunteers who assisted with interpreting and ensuring that community members were well taken
care of. With a greater service provider to community member ratio, we were able to provide faster, quality service to those who needed assistance. The health fair this year was a stunning collaborative commu-nity effort to make the AHSC vision a reality, reduc-ing health inequity and increasing access to quality health care to the Asian communities. We would like to give special thanks to Comfort Care Dental, Oregon Lion’s Club, OHSU Let’s Get Healthy, Pacific Univer-sity, OHSU School of Nursing, OHSU Richmond Clinic, Oregon Adult Immunization Coalition, Division Medi-cal Clinic, Dr. Suk Ki Lee, Dr. Sung Jin Cheon, Ms. Eva Kim, Dr. Shane Kim, Dr. Kenny Lee, Dr. Tracy Doll Lee, Dr. Maureen Becker, Ms. Sarah Yu, and Ms. Zora Tu, and National College of Natural Medicine for their generous support of volunteerism, and medical sup-plies. Yu Miao means “careful-ly planting and nurturing young sprouts.” Give them good nutri-tion, and they will grow into great plants. Children are like young sprouts, nurturing them with good quality of education will help them become great people. AHSC started Yu Miao Chinese Immersion Preschool in 1999. It is for all who want to ex-pose their children to the Manda-rin language and Chinese culture. Our goal is to provide quality edu-cation for children between ages 3 and 5. It is our desire to provide the students a quality education in a nurturing and caring environ-ment. We seek to help children fa-miliarize with Chinese customs and culture through Chinese children’s songs, stories, folk dance, games, and arts and crafts. Yu Miao has grown steadily with our preschoolers. Since 2007, we have moved to the current ad-dress at the former school building of Our Lady of Sorrows Parish Cath-
olic School on SE Woodstock Blvd and 52nd Avenue. In September 2007, the school was certified by the State to be the first Chinese Immersion Childcare Center in Oregon. We have three spacious classrooms, each room is about 25’x25’. We have a huge indoor gymnasium, fenced outdoor play-ground and courtyard. Our faculties are well trained early childhood education professionals with experience and credentials from mainland China, Taiwan and United States. Our program includes hands on learn-ing, Chinese literature and poetry, singing, dancing, arts and crafts, afterschool activities, and more. We believe in active engagement and learning through play and stimulating activities. Our preschoolers and afterschool program students work to become well-versed in performing arts, and their efforts are well recognized by community. They re-ceive invitations to per-
form at various events, including the Chinese New Year Fair at the Oregon Convention Center and other cultural activities. School tour is available by appointment.
