NMDA receptor mechanisms are involved in the development of hyperalgesia/sensitization (as are opioid metabolites)
NMDA receptor is also involved with opioid tolerance (desensitization).
Dilemma: Increasing the dose of opioid can either improve or worsen pain
Weaning at times may actually improve analgesic effect.
