Visioning Assessment for the KCAP Climate Adaptation Project Consent to Participate in a Questionnaire Project Investigators: CALP invite your consent to participate in a research study, as follows: Consent: You agree to participate in a research project conducted by CALP The project is part of a postdoctoral research project that examines how interactive landscape visualization tools such as GoogleEarth can improve communication and understanding of climate change impacts and strategies. The research will also help to refine the visualizations used in local community climate change projects in the future. Your participation would take place within the KCAP Climate Adaptation Project Working Group meeting on June 9. Your participation in the research, which includes completing 2 questionnaires (before and after the visualization presentation), will take approximately 20 minutes in total (not including viewing the presentations which will take approximately one hour, plus time for questions) . The research project part of the meeting will include: 1. An overview summary of the research study and its aims as part of the Kimberely project; 2. A Powerpoint presentation by the CALP team during which you will be asked to view and, if you wish, discuss images of possible future scenarios that illustrate different adaptation and mitigation measures for your local community; 3. An open session where you can, if you wish, read the prepared posters and explore interactive visualizations of climate change impacts and possible future scenarios in GoogleEarth at a computer station. The research depends upon the following questionnaires. Each questionnaire takes about 5-10 minutes to fill out. The research results will be presented in a summary document that will be made available to you upon project completion over the summer. Your participation in the research project is subject to the following conditions: Your participation in the project is entirely voluntary. If you are a member of the Kimberly Climate Adaptation Working Group, it does not in any way affect your standing within the working group. You may refuse to participate or withdraw from the questionnaire at any time without jeopardy or repercussions of any nature. While the Working Group meeting occurs as an open forum where participant identities are known, your anonymity and right to privacy will be protected in any publicly accessible reports, papers, and presentations resulting from this research. No personal identification is attached to any of the questionnaires.

Visioning Assessment for the KCAP Climate Adaptation Project

Consent to Participate in a Questionnaire

Project Investigators: CALP invite your consent to participate in a research study, as follows:

Consent: You agree to participate in a research project conducted by CALP The project is part of a postdoctoral research project that examines how interactive landscape visualization tools such as GoogleEarth can improve communication and understanding of climate change impacts and strategies.

The research project part of the meeting will include:

1. An overview summary of the research study and its aims as part of the Kimberely project;

2. A Powerpoint presentation by the CALP team during which you will be asked to view and, if you wish, discuss images of possible future scenarios that illustrate different adaptation and mitigation measures for your local community;

Your participation in the research project is subject to the following conditions:

• Your participation in the project is entirely voluntary. If you are a member of the Kimberly Climate Adaptation Working Group, it does not in any way affect your standing within the working group. You may refuse to participate or withdraw from the questionnaire at any time without jeopardy or repercussions of any nature.

• While the Working Group meeting occurs as an open forum where participant identities are known, your anonymity and right to privacy will be protected in any publicly accessible reports, papers, and presentations resulting from this research. No personal identification is attached to any of the questionnaires.

• The investigators guarantee access to all recorded data will be limited to the project investigators and will be kept secure in a locked faculty office.

If you have any questions or concerns about the procedures used in this research, CALP have agreed to address these and any other inquiries that you may have.

We would be pleased to explain the methods employed in the study in greater detail after you have participated today, or you may contact --------. If you have any questions or concerns about your rights or treatment as participants in a research project, you may contact the Research Subject Information Line in -------.

With these questionnaires, you have also received a copy of this consent form for your own records. By filling in the questionnaire, you consent to participate in this study pursuant to the above conditions.
