Ready beyond tomorrow with Dual Source CT

With its special 3D imaging abilities, CT imaging provides robust and reproducible assessments. By measuring annular and aortic root morphology and dimensions in their true plane, it delivers critical incremental information. CT-based sizing may also lead to better procedural results, with lower incidence of paravulvular regurgitation, because it estimates the true dimensions of the (non-circular) aortic annulus better than 2D tools, which tend to underestimate. CT imaging is the only modality that enables you to simultaneously assess iliofemoral access and angio g raphic procedural angle prediction, which may enhance the success and efficiency of your procedures. And this holds true not only for established procedures such as TAVI but also for future applications such as mitral and tricuspid minimally invasive interventions.

Be ready for the future today with Siemens unique Dual Source CT and share its benefits with all of your patients. Frail patients in particular benefit from the ultra-fast acquisition speed and low kV imaging: Dual Source CT can provide all relevant information in one single scan with one single injection of contrast agent as low as 38 mL1, thereby reducing the risk of impaired renal function.

Focus on what you need to see

Although MRI imaging is still under-represented in structural heart disease therapy, there is growing evidence to suggest it can be equivalent – or even superior to – echocardiography and CT imaging for specific elements of the TAVR patient workup, as well as for post-implantation evaluation1. And thanks to new technology, even arrhythmic and dyspneic patients can now benefit from MRI.

Free-breathing, high resolution Cardiac MRI

Compressed Sensing Cardiac Cine is the first clinical application based on our disruptive speed technology, which makes MRI acquisition up to ten times faster, with no loss in image quality. Instead of almost six minutes, you can now perform a high-resolution Cardiac Cine scan in just 25 seconds2 – without breath-hold. How does it work? We acquire just a fraction of the image data, then use iterative reconstruction to generate high-resolution images based on our award-winning algorithm.

• Acquire free-breathing, high-resolution Cardiac Cine images

• Capture the whole cardiac cycle for precise quantification

• Expand your eligible patient population for cardiac MRI

For a PURE® experience in angiography

For diagnosis and treatment of cardiovascular disease, you demand crystal-clear images of the moving heart and of challenging cardiac anatomies in any angulation. To spice up the challenge, dose has to be kept to a minimum even during complex procedures. Our Artis systems deliver images in excellent quality at the lowest reasonable dose – without compromising on image quality or clinical outcomes. And this is valid for our complete portfolio – in all detector sizes and also ranging from floor, ceiling mounted, or biplane systems to highly flexible multi-axis systems with robotic technology.

Tailored to your needs

Heads-up display: Stay focused with context-sensitive onscreen menu.

Diagnosis Therapy

The more complex angiography procedures and system interaction become, the harder it can be to use interven-tional systems to the full. On Artis zee, Artis Q, and Artis Q.zen systems, our PURE® platform makes Siemens Healthineers smart technology supremely easy to use. As with the 3D Wizard, choose the desired image result from a pool of possible cases and let the system guide you through the acquisition. All required parameters for a 3D scan inclu ding protocol recommendation are provided. This supports definition and establishment of clinical as well as depart mental standards for clinical studies, quality assurance, etc. The result? Higher process efficiency, better diagnostic information – and enhanced patient treatment outcomes.

eSie Valves Advanced Analysis Package• Advanced machine learning technology for

highly standardized assessment

• Comprehensive static modeling of the mitral valve in less than five seconds

• Dynamic analysis by calculation of a moving valve model throughout the cardiac cycle

Ultrasound can be a powerful alternative to CT imaging during preprocedural planning. With advanced tools such as True Volume TEE and eSie Valves advanced analysis, our ACUSON SC2000 system analyzes aortic valve anatomy and function accurately for fast, analysis-based decisions.

True Volume TEE visualizes real-time volume and volume color Doppler in a 90° x 90° sector. This means no risk of stitching artifacts in patients with arrhythmia, eliminating the need for ECG control and waiting time.

eSie Valves modeling software helps you size TAVI devices more accurately, even for patients with renal insufficiency. Based on the valve model it creates, this machine-learning-based program takes about 50 measurements automatically – for efficient, reproducible assessment of the aortic valve.

See what lies ahead

Aligning the valve prosthesis accurately in the aortic root is essential during TAVI. In less than 30 seconds, syngo Aortic Valve Guidance recon structs and segments the aortic root automatically – and indicates anatomical landmarks – based on preprocedural CT scans or DynaCT cardiac images. To adjust the C-arm projection for the best view, you simply press a button.

By overlaying segmentation results and landmarks on live fluoroscopy, you can then guide the valve prosthesis more accurately, reducing the risk of paravalvular regurgitation.

Fusion imaging provides continuous guidance throughout the deployment phase of the valve without additional contrast media usage1. A preoperative CT image can be co-registered to angiography with only two fluoro shots from different angles.

Your guide to the paravalvular leakage

Although new valve technology is reducing the incidence of paravalvular leaks (PVL), they still pose a significant risk during and after TAVI. To help you treat them effectively, our syngo 2D/3D Fusion Package fuses preoperative CT, MR or PET images of an identified leak with live fluoroscopy for precise guidewire navigation. Two single fluoro shots are all it takes for co-registration.

One rotation to the LAA

Left atrial appendage closures (LAACs) are now an established method of reducing stroke risk in patients with non-valvular atrial fibrillation (nvAF) as an alternative to long-term anticoagulation drug therapy. Since no two LAAs or surrounding structures are the same, our advanced imaging tools can help you plan and effectively perform occlusions with reduced risk of complications. In addition to echo-based 3D imaging support as ICE, TEE, and the fusion of TEE with live fluoroscopy we also offer guidance solutions based purely on angiography.

syngo DynaCT Cardiac creates 3D visualizations of the LAA with its current blood-fill status during the procedure. With rotational angiography, you can obtain high quality CT-like images in just 5 seconds.

Mitral valve replacement – a new frontier

While transcatheter repairs of degenerative mitral valve regurgitation are an established alternative for patients with increased risk related to comorbidities, transcatheter valve replacement (TVR) is a new and highly challenging field.

Due to the complexity of the mitral valve anatomy, integrated multimodal imaging will be key to support successful implantation of future devices.

From CT assessment to fusionAdvanced 3D CT imaging helps you assess valvular anatomy and calcification, select and size your device, and plan the best access route. Our unique Dual Source CT technology helps to ensure robust data acquisition even at high heart rates, reducing the need for beta-blockers. It scans fast, providing clear images even when patients cannot hold their breath.

From TEE assessment to fusion

Treat complex valves efficiently – with automated measurements and comprehensive 3D visualizations using TEE. The eSie Valves advanced analysis package can help you plan your patient’s therapy by quantifying the diastolic mitral valve orifice area in advance. Having been trained on a large database of annotations covering multiple diseases, the eSie Valves algorithm can detect the leaflets automatically. After just minor editing, they provide a comprehensive 3D model of the diseased valve and more than 50 key measurements based on this modeling.

TrueFusion lets you fuse eSie Valves models with live fluoroscopy. The Artis C-arm can be adjusted auto-matically to the optimal angle, based on the visualization of the mitral valve annulus.

True Volume color Doppler on TEE lets you assess the success of your valve implantation with confidence by providing real-time imaging of the complete mitral valve apparatus and flow information.

Tricuspid valve imaging options

Unlike transcatheter therapy of aortic and mitral valve disease, intervention strategies for tricuspid valve (TV) disease are still in the early stages. The large dimensions and fragility of the valve, the lack of valve and annulus calcification, and the close proximity of the right coronary artery are just some of the factors that demand a unique approach.

The anatomy also presents challenges for TEE imaging. Intracardiac echocardiography in 2D – but especially in 3D – can be an alternative, and requires no general anesthesia.

Our ACUSON AcuNav V ultrasound catheter is the world’s first real-time Volume ICE catheter. It provides superior visualization of anatomies and devices at a volume size of 90° x 24° and can be guided by a single operator.
