My25 Choice Start-Up Specifics Really Important To Know • Changing up longstanding habits/routines and poor health will not happen overnight. There will be bumps in the road within the first 2 to 3 months—count on it. But we get to smooth sailing pretty quickly when staff and house managers are responsive to us and proactively reach out on a frequent basis to request menu changes, so we can get to the best—most maximizing, person-centered and budget sensitive—menus possible . . . quickly. We routinely create menus for people with diabetes, GERD, lactose intolerance, dysphagia, allergies, gluten free, end stage renal disease, and more. We have a 98% retention rate over 6 years. So please know that we understand how to smooth out the bumps if you engage with us. • Submit menu and census change requests often to: [email protected] • If there are reluctant consumers regarding eat-better habits, let us know—we’ve seen and heard a lot, and been helpful in hundreds of instances along these lines. High on our list of tried-and-true suggestions is engaging people supported in menu planning and mealtime prep. “When you make it, you own it.” • Please provide consumers’ weights on a routine basis to the My25 Support team, when requested. The following pages contain more about weekly deliverables and menus that cycle new every 35 days and resources that will help you maximize My25 Choice to the fullest.

Menus

Based on information—collected during the intake process and on an ongoing basis from DSPs and house managers—My25 menus, emailed each week, are customized for each individual’s food likes/dislikes and dietary needs—no matter how complicated, such as for diabetes, GERD, lactose intolerance, dysphagia, allergies, and more—and routines of each setting. We also address staff needs related to preparation; as one example, many settings ask for slow cooker crockpot meals to ease prep time and take advantage of one-pot meals. We subscribe to The Dietary Guidelines for Americans and a moderation versus deprivation approach—which leads to sustainable, eat-better habits. You will often see initials on menus that outline all meals, snacks and beverages for each day of the week, calling out for specifics that staff should note regarding individual needs. Grocery Shopping Lists

My25 Choice grocery shopping lists sync with the menus and are also sent via email on a weekly basis. The grocery shopping lists are organized by aisle (as best we can—understanding that grocery stores do differ a bit). We also provide an estimate of total cost, which is useful for management oversight and staying within budget. House managers often compare the grocery shopping list total to the grocery store receipt. Accounting for regional differences, there is a match if the two are no more than 10% apart. Grocery expense is contained when My25 menus are followed and shopping is in line with the menus. Also, it’s important to check what is on your shelves and in your refrigerator/freezer before heading to the grocery store, so you don’t overbuy and go over budget. We’re often asked about fresh fruits and vegetables versus frozen/canned options. Fresh fruits and vegetables typically cost more than frozen and there is often more spoilage with fresh. The nutritional component is similar between fresh and frozen. But if you prefer fresh, we’ll certainly include that in your menus and on your grocery shopping lists.

Recipe Preps Steps

Corresponding recipe prep steps are emailed each week along with the menus and grocery shopping lists. Every effort is made to keep prep quick and easy for busy staff. Please note that highlighted areas above note consumers’ initials to call out specifics for individual needs in the prep process. Remember: Involve people supported in prep. “When you make it, you own it.”

Outcomes Progress Report

At least twice per year, My25’s Support team compiles a progress report overiew for each provider’s management team. The information is based on the routine, proactive outreach we engage in with staff and house managers on an ongoing basis, and at least every few months. In this report, we summarize information related to health improvement, the ways in which person-centered choice is being accomplished, grocery budget specifics, and recommendations for improvement. Responsiveness from staff and house manaaers—including consumer weights—is all vital information to outcomes progress and our reporting to provider leadership that has made an investment in the program for the benefit of people supported, staff, and the organization as a whole.
