Our Community Services deliver a range of personalised support in Stockport, Greater Manchester and the surrounding areas. We provide person-centred, creative, fun and professional support services to children, young people and adults with complex needs including:

We work to enable individuals to break down barriers and become more independent, to develop new and existing skills and to provide the opportunities to engage with their peers

Outreach

The Hub is a community space in Salford, at the Humphrey Booth Centre, which has been opened, thanks to a significant grant from Booth Charities, to help support and bring together the local communityof Ordsall and Langworthy.

The refurbished space boasts a safe outdoor play area, specialist bathing facilities, a high-tech IT suite and commercial kitchen. The Hub provides short breaks, community and support services for young people. The facilities can be used by local residents for scheduled activities throughout the week and can be rented out at very affordable rates by residents and local community groups.

Together Trust runs a number of vibrant adult day services in the North West, which offer person centred support to adults with a range of disabilities and complex needs. Each centre organises group activities, both in the day service and out in the local community, providing opportunities for the individuals to develop their independence skills as well as the chance to socialise.

Newbridge - Stockport Newbridge offers a diverse range of exciting and varied activities for young adults aged 18 to 30. There are opportunities to develop vocational and independence skills as well as the chance to socialise. They have their own on-site restaurant, ‘Café Xperience’, where individuals can gain new skills making and serving lunch five days a week. Those who use the centre regularly get out and about in specially adapted vehicles, accessing the local community, including the leisure centres for swimming, football, basketball and the gym.

Adult Day Services

SK30+ - Stockport SK30+ is a service for adults aged 30+, which explores an individual’s interests and ambitions to tailor make a unique support, care and activity plan. SK30+ has a large sensory space and art studio and users of the service regularly get out and about to participate in a range of community activities in and around Stockport.

New Visions - Oldham New Visions is a day centre for adults aged 18 years and over, which creates tailor-made programmes to suit the needs of individuals. New Visions offers a wide range of activities, including access to learn touchscreen computing, sensory equipment, art and games and regular supported trips into the community.

WaTT offers young people aged 18+ the chance to gain catering industry experience and qualifications in a professional, safe and supporting environment. They are involved in every stage of a catering business from helping to prepare and deliver high quality food, to learning new skills including good customer service, food hygiene and safety.

WaTT helps improve the confidence, skills and self-esteem of individuals through working and training within a team.

A whole world of opportunityTogether Trust has a number of supported living services across Greater Manchester and Stockport that enable adults with a physical or learning disability, autism or complex health needs to live in their own homes, rather than in residential care homes. The aim of the service is to enable people to live full and enriched lives within their local community, to experience a broad range of relationships and build support networks.

Bolton Supported Living Bolton Supported living is a new Together Trust Service, which offers support to individuals with autism and complex needs to live within their own home, in Bolton, by providing person centred support. Using Positive Behaviour Support and Active Support models staff help enable individuals to develop independence and improve their daily living skills.

What is Active Support?• A method of supporting people to engage

in meaningful activities and relationships as active participants.

• Using ordinary everyday activities to encourage choice and engagement, and to increase independence.

• Working with people to promote optimal engagement, irrespective of the degree of disability.

• Addresses the way staff teams are or ganised.

• Active Support addresses the complementary relationship between person-centred action and person centred planning

The ultimate aim is to support people to live the lives they want to, doing the things they want to, following their agenda and respecting their decisions

“Our son has been supported by the Together Trust in his own home, in his home town since he was 17. He is now 20. Prior to the Together Trust, Mark attended many out of borough schools for children with autism, travelling considerable distances. His life has changed considerably for the better over the last three years primarily due to the dedication and care of the staff team supporting him within his home.

“We do not feel there is enough paper available in the world to document how complicated a young person our son is. We always said ‘If you get his support package right, he will be a delight to work with, if you don’t, it could have disastrous consequences.” So far, the staff team have got it right, never being complacent and always looking for improvements.

“The staff have had to make sacrifices by changing their working patterns to enable Mark to have a person-centred support package that works better for him. Our son now lives around the corner from us, not hundreds of miles away. He is settled and happy and interacting with other service users. He now has a social life! He is supported by a fantastic staff team: dedicated and non-judgmental.

“We as a family are more settled and happy because our son is. It certainly is not a lot to ask for, but certainly a lot for a staff team to deliver, especially with the complexities our son presents. The Together Trust staff team are doing a fantastic job and we cannot put into words our appreciation and admiration of them. To put your trust and care of your vulnerable child, however old they are, into the hands of others is very difficult, but with the team at the Together Trust it is certainly a whole lot easier. Many, many thanks to them.”

What is Positive Behavioural Support?

Positive Behaviour Support (PBS) is a function-based approach to eliminate challenging behaviours and replace them with greater social skills.

PBS does not focus exclusively on the individual but also includes changing their environment.so it can be successful with a wide range of individuals, in a wide range of contexts, with a wide range of behaviours.

The most crucial part of devising PBS plans is the Functional Behavioural Assessment (FBA), which identifies the contexts (events, times, and situation) when behaviour may and may not occur and identifies the consequences

At the Together Trust we are committed to supporting people to achieve their full potential at home, in the community and in education.

Our learning support service, currently based a Bolton College, supports individuals aged 16 – 26 years, who may have been prohibited from attending further education due to complex needs and/or disability, to access education and achieve their aspirations.

Our team works closely with staff at Bolton College to offer person-centred support to individuals with complex medical needs, safeguarding concerns, a physical or learning disability and behavioral difficulties.

We work to empower students and give them the confidence and ability to learn new skills that help them to achieve their educational goals, negotiate and manage everyday challenges, make informed choices and make positive change.

These skills include:

• Communication

• Team work

• Decision making

• Respect

• Conflict resolution

• Problem solving

• Managing emotions

• Accessing the community

Steven’s Story…

For three years, Steven had been told by his Local Authority that he could not attend college as they thought no one could accommodate his needs. He had an end of life plan and needed extremely complex and continuous medical support. However, Steven was extremely passionate about continuing with his education. So when Bolton College asked us at the Together Trust if we could support him we said yes, with no hesitation. Within a few weeks the appropriate support staff had been matched with him and sent on additional training to ensure they could meet his specific needs and he was in the classroom enjoying college life. Staff worked hard to support him everyday and with their support Steven was able to enrol on a level 1 IT qualification and made many new friends.

Learning Support Service

Developing skills for life

We provide a range of support services to complement holidays and short breaks for families, children and young people at three Together Trust owned, specially adapted holiday venues.

All our support workers have undertaken in-depth specialist training to support children, young people and adults who have autism, complex needs, acquired brain injuries or a learning disability. All services meet a high standard and are flexible to provide day support, sitting services or activities.

We own three fully accessible holiday venues, which are furnished to a high standard and are fully inclusive of utilities and site passes.
