The primary goal of our program is to provide opportunies for individuals who have a disability to develop recreaon skills and explore inter- ests while providing a break for the caregiver. Although we are connuing to offer limited group and family-oriented oungs, we encourage parents to take advantage of the respite service so that caregivers can have a break through our recreaon programs. Programs that are in the secon “Programs for Individuals” are intended to be respite in nature and count toward waiver parcipaon. Individuals must ulize at least one individual program annu- ally to remain acve on CP Rochester’s roster. This program is community-based and parcipant-driven. We encourage families and parcipants to make suggesons and provide feedback on the programs that are offered. Please note that the only program where we offer a staffing rao of 1:1 is for the Individual Swim Lessons and rock climbing; otherwise staff and volun- teers provide support and oversight. Individual assessments are used to deter- mine whether seng and acvies are appropriate for each individual to ensure a fun and safe environment. Most of our acvies take place locally, however, others explore venues outside of Monroe County. We are always open to working with new partners and ven- ues, so if you know of a great resource, please feel free to share them with us! Ages: 3 and up

Instruc+on is provided by our Adapted Physical Educa+on teacher and swim-

ming instructors and is designed for individuals who have special needs. Stu-

dents will receive 30 minutes of instruc+on and will work toward developing

independent swimming skills and increased safety awareness in the water.

Space is limited!

Our bowling league is a great recrea+onal and social opportunity. We bowl weekly

and celebrate at the end of the league with a pizza party & award ceremony. No

previous experience is necessary just the excitement to try bowling. Individuals

will be grouped into teams and scores will be kept each week. Bumpers & bowling

ramps are available so people of all abili+es can par+cipate!

Programs for Individuals

Jump, run, flip, and be crea+ve in this gymnas+cs class

at The College at Brockport that is supported by stu-

dents in the Physical Educa+on program. Par+cipants

are kept moving from sta+on to sta+on and are

matched up with a volunteer throughout the class.

Individuals can try out the balance beam, roll and flip

on the mats, hop over the horse, bounce on the

(small) trampoline, and (everyone’s favorite) hop into

the big foam pit to relax at the end of class!

Program #4: Star+ng in February, Dates TBD

Time: 9am-9:45am

Ages: 6—21

Cost: No cost to par+cipant

Group Size: Limit 8 individuals

Deadline: Please register by the end of January and an email will go out with

dates as they become available.

1:1 Fun, Fitness, and Physical Activity

F A L L 2 0 1 5

Our fun fitness provides the opportunity for increased physical ac-

+vity levels and skill development through a 1:1 instructor. Ses-

sions will be geared toward the individual's age and interests with

a focus on aerobic, muscular strength and endurance, and flexibil-

ity components. Sessions will be 30 minutes long. Ages 5-9 and 10-

17.

Descrip+on for individuals ages 5-9: The sessions will include skill development and op-

portuni+es for increased physical ac+vity through par+cipa+on in ac+vi+es/games.

Fitness levels will be assessed by observa+on.

Descrip+on for individuals ages 10-17: The sessions will involve fitness tes+ng with the

Brockport Physical Fitness Test designed to assess each individuals specific needs.

ANer assessment, target areas of fitness will be selected by the individual, games and

ac+vi+es will supplement the session.

Shared Ski Adventures is an adap+ve Learn to Ski Program that helps

people with disabili+es to develop independence in alpine skiing or

snowboarding. SSA at Swain registra+on is now closed, however we

are s+ll taking applica+ons on a first come first serve basis to fill last

minute sports.

We will be offering a new local ski and snowboard op+on this year at

Northampton park through a partnership with the park and SWAIN.

We are currently assessing the best days and +mes so please send in

of the RMSC during your visit.

This unique form of rock climbing goes sideways rather than ver+cal. Therefore no ropes or

harnesses are required. These three events will be a great way to get youth exposed to the

world or rock climbing in a safe quiet environment. Students will receive 30 minutes of instruc-

+on and will work toward developing coordina+on and balance.

2. Complete an Intake Packet for Respite / Recrea+on

3. Submit your completed Intake Packet to the Intake Coordinator

4. Obtain approval for Recrea+on Respite services

In order to par+cipate in Respite Recrea+on Programs, individuals must:

• have Medicaid

• be enrolled in the HCBS waiver

• have received approval for the service from CP Rochester and DDRO

• par+cipate in a Respite Recrea+on Program annually

How to Register for Programs All required intake documenta+on must be received and processed prior to register-

ing for an event or program.

registra+on, staff must ensure that all the required documenta+on is in place for each

person, and at +mes we must follow up with your Medicaid Service Coordinator.

The confirma+on email will include payment details. Sending payment in with your registra+on will not

Program Fees We try our best to offer low or no-cost recrea+on programs, however, some ac+vi+es do incur expenses

beyond basic staffing resul+ng in fees which must be paid aNer successful registra+on. This cost applies

to both HCBS/Case at home waiver enrolled AND non-waiver enrolled individuals.

If you are enrolled in the HCBS Waiver or the Care at Home Waiver, you s+ll have to pay the fee that is

listed for that ac+vity.

If requested, we will provide a receipt for payments made to CP Rochester for Recrea+on Programs. You

may choose to submit your receipt for Family Reimbursement—please contact your Medicaid Service

Coordinator for further informa+on.

Payments or Cancellations Payments or cancella+ons must be made by the program’s registra+on deadline. If we have not received

payment one week prior to the event’s date or start of the session and you have not contacted us, we

will assume that you are not going to par+cipate in this program anymore. Past due payments will affect

registra+on for future recrea+on programs. In the event that a program is cancelled, we will issue a

voucher that can be used toward future recrea+on programs; we do not issue refunds.

Project Access Transportation Project Access is a grant supplemented service that provides, low cost transporta+on to individ-

uals with disabili+es living with family in Monroe county.
