After the courtesy call with the Barangay Officials, the group proceeded to the Creek Site for their courtesy call with Mr. Noli Rapsing, the President of the Muntinlupa Creeksite Neighborhood Association Inc. (MCNAI). The group explained the purpose and objectives of their community exposure.

Information Dissemination

The group of health team disseminated information for the meeting of the core group, visited their houses and discussed briefly about the continuation of the Health teachings.

The team had a problem with the time and the availability of the members of CHO.

The team set a specific time for the members that can fit in their schedule.

Formulation of Concept Paper and Plan for Core Group’s Meeting

The group formulates their concept paper which is about the reinforcement of basic vital signs monitoring and bandaging. The group had a brainstorming on what activities should be done for the entire stay in the area and assigned the people for certain parts of the activity. The group also started rehearsing for the said assembly.

1. The group has to proceed with plan B which is to reschedule the meeting on the next day.

The key holder was not around. Because of this, the group had to post pone the meeting.

The group proceeds to the Creek Site for the continuation of their activities.

The activity was cancelled because the Multipurpose Hall was not opened as the key holder was not around. The group had a pre-conference to discuss their plan B.

The group goes to the houses of each participant to inform that the activity was rescheduled.

Coordinate to the officers or members of the organization for them to get the key to Mr. Rapsing.

House to House Visit:

- Actual Demonstration Operation Timbang and computation of BMI

The group, together with the participants started weighing the children in Creek site.

The group was divided in two groups to maximize manpower and time. The two groups were not able to attend to all the children because some of them were at school during our visit.

The group together with the participants went back to the multipurpose hall to compute the BMI and the students assisted the participants in computing.

Some of the children in the Creek Site are afraid to get their weight.

1. The team had some difficulties that caused them to slow down their data gathering due to the people living in blocks 1,2 and 3 had many questions about the purpose of it and what would it benefit them.

With that the group decided to include in the survey those residents who weren’t able to finish their studies regardless of their age.

The team just asked the people to still answer the form even if they would not like to continue their studies.

The students decided to finish everything the next day.

Some of the residents declined to answer the form because they said that studying Is no longer their priority in life since they already have their job and own family.

Due to time constraints the students wasn’t able to finish the documentation and data tallying.

The group went back to blocks 5 and 4 to get the data of those people 25 years old and above who weren’t able to finish their studies regardless of their age.

The group was able to finish all the blocks and collected 11 survey forms.

The students stayed in their classroom and started to do all the documentation on the activities that they conducted in the community.

The students also started to do their tallying of data regarding the people 25 years old below and above who weren’t able to finish their studies.

NARRATIVE REPORT OF HEALTH TEAM

The Level III students of Adamson University – College of Nursing started their

community duty for the second semester of academic year 2014-2015 in Purok 6, Creeksite,

Barangay Sucat, Muntinlupa City on January 26, 2015 with their clinical instructor, Ms. Ma.

Flordeliza U. Donato, RN, MSN.

The first day, January 26, 2015 from 7:00 AM – 12:00 NN started with a flag ceremony

at the Barangay Hall. We were warmly introduced and welcomed by Chairman Mamerto

“Metong” Sevilla during his speech. One of the students, Ms. Sha Yaoru (Anna) was able to talk

in front of the people and introduce the nursing students. We then went to Creeksite and initiated

an ocular survey. The students were divided into two groups; the health team, which will

determine the current health status and improve the previous findings and utilize the action plan;

and the ALS team which will determine the out-of-school youth in the area and will coordinate

with some educational officers for the Alternative Learning System. After walking around the

area, we started making our documents which will be used for our duty such as the Gantt Chart

and letter of invitation for the general assembly.

On the second day, January 27, 2015 from 8:00 AM – 12:00 NN, the health group started

to make the letter of invitation for the general assembly that will happen on January 29 at the

Multipurpose Hall at Block 10. Then it was distributed to block leaders and community health

organization officers. The group was also able to talk to the president of the Home Owners’

Association, Mr. Noli Rapsing and share the development of our activities for Creeksite. We also

introduced some activities for them. The other group started to interview out-of-school youth in

the area and gathered their basic information. The two groups also started to make concept

papers for the next activities.

On the third day, January 28, 2015 from 8:00 AM – 12:00 NN, the health team continued

giving invitations for those who were not in their houses during our rounds. They also started

making their concept paper which is about the reinforcement of basic vital signs monitoring and

bandaging. The group had a brainstorming on what activities should be done for the entire stay in

the area and assigned the people for certain parts of the activity. The group also started

rehearsing for the said assembly.

On the fourth day, January 29, 2015 from 8:00 AM – 12:00 NN, the General Assembly

regarding the introduction of activities for the next two weeks took place. Ms. Donato and the

students discussed with the participants some proposals such as creating a herbarium and

formulating a DBHS for the area. The participants agreed on having a reinforcement of basic

vital signs and BMI taking for the next day in the same venue.

On the fifth day, January 30, 2015 from 8:00 AM – 12:00 NN, unfortunately, the activity

was cancelled because the Multipurpose Hall was not opened as the key holder was not around

when we arrived. The hall was supposed to be used also by the Senior Citizens for their meeting,

with the same time as the planned time for our activity. Instead, we informed the participants that

the activity will be moved on Monday, February 2 through sending a text message and by going

to their houses. After that, we made little changes on our concept paper for our activity and

started rehearsing and reassigning the students for their tasks. Then, we helped the other group in

their data gathering.

On the sixth day, February 2, 2015 from 7:00 AM – 12:00 NN, the group, together with

Ms. Donato joined the weekly flag ceremony at the Barangay Sucat Hall. After that, they went to

Creeksite and prepared early for the day’s agenda. The Health Team, together with their clinical

instructor Ms. Donato conducted a reinforcement of the lecture about vital signs and body mass

index or BMI to the participants. The participants did a return demonstration about what they

have learned like taking the blood pressure, temperature, respiration rate, pulse rate, weight and

height taking, and solving for the BMI for both children between 0-6 years old (using the Gomez

Chart) and the adults.

On the seventh day, February 3, 2015 from 8:00 AM – 12:00 NN, the group, together

with the participants prepared the materials for the Body Mass Index taking, like the weighing

scale and tape measure. Then they started weighing the children in Creeksite. The group was

divided in two groups to maximize manpower and time. The two groups were not able to attend

to all the children because some of them were at school during our visit. After that, we went back

to the Multipurpose Hall to start computing for their BMI. The students assisted the participants

in computing.

NARRATIVE REPORT OF EDUCATION TEAM

The level three student nurse of Adamson University had two weeks of exposure at Purok

6, Creeksite, Sucat, Muntinlupa City started on January 26, 2015 to February 5, 2015. On the

first day, the group had courtesy call to the Barangay Captain Engr. Mamerto “Metong” Sevilla

at Sucat, Muntinlupa City led by Mr. Iman Patrick Kiamco. The purpose of exposure of BSN

level 3 in the area was discussed, which is the continuation of the activities conducted by the

same group of students, the BSN level 3. These activities aim to aid identified problems by the

community people at Purok 6, Creeksite, Muntinlupa City. Moreover, Ms. Flordeliza Donato,

RN, MAN, the clinical instructor of the group discussed about the barangay resolution which

includes health training skills such as vital signs taking, herbal medicines and diabetes mellitus.

The group was divided into two namely, the health training skills group and the education

group. Health training skills group aims to reinforce vital signs taking and first aid preparation

that were previously taught to the group of participants who attended last semester. In addition

to, this group imparted about diabetes mellitus and herbal medicines. The education group aims

to identify the number of out of school youth and undergraduate older adults in the area and

formulated a questionnaire as a tool. Moreover, the group had ocular survey to determine if there

are any changes in the area guided by the spot map that was updated last exposure to the area

before they conduct the survey.

On the next day, the group had a short discussion about plans for the day and started data

gathering in the area with the target respondents of twenty-five years old and below who are out

of school youth or undergraduate. During data gathering the group had easily gather some data

because most of the people were in their house and very cooperative. They willingly respond to

the interview and answer the questionnaire appropriately. The group finished blocks 10 and 9

and collected ten questionnaires. Hence, the group also encountered problems during data

gathering such as some determined respondents who are undergraduate were not at home during

daytime because they are working while others are busy doing their household choirs, with this

matter the group decided to left the questionnaire on their relatives so the respondents can

answer and collect it the following day. Second day, the group continued the survey but before

that the two groups had a quiz first before they conduct the data gathering. Blocks 8 and 7 were

finished and collected fifteen questionnaires. Time is also a factor that somehow became a

barrier to the group in collecting data easily and promptly because some respondents had

difficulties answering the questionnaire and it took time to finished a questionnaire, with this the

group elaborate the questions to the respondents and had short interview to validate the data

gathered to them. On the third day, the education group participates in the health training skills

group with their activities and continue their data gathering on the fourth day which they finished

blocks 6, 5 and half of block 4 and collected eleven survey forms. There were also respondents

who had difficulty reading due to blurred vision and difficulty writing due to pain in their hands.

Their relatives help them in writing and reading together with the education group. Fifth day of

data gathering, some houses have two floors with different family living on the first and second

floor, others are still asleep during the day because they work at night. According to their

relatives even though they are already working, some are still willing to be part of the survey of

the group conducting. The remaining houses in block 4 and almost half of block 1, 2, 3 were

surveyed and collected nine questionnaires. On the sixth day, Ms. Flordeliza Donato, RN, MAN,

raised the concern of the Creeksite community to the education group regarding the adults who

are above twenty five years old who wants to be part of the survey. The group decided to include

those who are willing to participate the data gathering regarding of their age. The group finished

the remaining houses in blocks 1, 2, 3 and collected eleven questionnaires this time the

respondents are those who are undergraduate regardless of their age. On the seventh day,

Wednesday, the group went again on the blocks 10 to 4 to survey again if there were anyone who

are undergraduate or out of school youth. Due to time constraint the group only finished the

blocks 4 to 7 and collected 17 questionnaires. On the last day of data gathering, Friday, the group

was able to finish the remaining blocks and collected 81 questionnaires.

We had two weeks of exposure to Purok 6 Creeksite, Sucat Muntinlupa City. In this

exposure I had a lot of realizations in life that opened up because of the experience I got from

our two weeks duty. Those people living in the area wherein we had our duty were so hospitable

to us. That on our first day when they saw us they it was like we have been their neighbor for a

long time, it was like they really close to us already. Maybe because they know that we came

there to try to help them improve their life especially their health. I also notice that those people

despite of their situations in life they are happy and seemed like they don’t care that they were

poor. For them life must go on. Those people who were affected by the fire last year most them I

noticed were already built new and improved houses. Some actually built 2 floor houses were in

fact before of the fire they told us that their house was only bungalow.

Our group were also been divided in to half. The first group was given the task to

continue our activities last semester. And then our group which was composed of Junina and

Patrick, the three of us were given the task to survey the community to know the population of

those out of school youth or those people who were not able to finish their studies. I had a great

time doing that because I learned that most of the people who answered the form were not able to

finish their studies due to financial reasons and that if they would be given a chance to pursue or

continue it they would do so. For me this is a good sign that these people despite of their

economic status in life they still have dreams and believe to achieve it once they finish their

studies.

During two weeks duty I become more thankful to God in everything that I have. I

learned to appreciate more of my studies because it is true that there are a lot of people who

would like to study but weren’t able to study due to financial reasons and here I am studying in a

Catholic school. I should not complain because I already have what others are dreaming of and

that is to be able to study.

As we all know, education is one of the most important things a man should have

nowadays. For education, as we know it, shapes up a man and helps him become successful

in the future. Looking into the community we have served and dined with this past two week,

all I can say is that I really had fun. It was an enjoyable, unforgettable yet hard and

exhausting experience. I enjoyed a lot not because of the warm smiles the people brought and

gave us during our stay, not because of the green jokes they tell us and the stories they share

which gives a great lesson in life, not because of the low cost and cut-off or discounted price

we received whenever we buy something in bulk, it is all because of the simple smile and

hope that shows in their eyes that made me realize how much our education is important.

That, on the latter part, will help us in the future for me and my family. Never the less, they

inspired me to be a better man than what I am today. They showed me that no matter how

hard the trials may be, if there is ever a little or a single strand of hope left, you should give it

your all and try to achieve what you have in mind. After all, ”Ang edukasyon ang pinaka

mahalagang bagay sa lahat kaya pagsikapan nyo.” If given another opportunity, I would

deffinately go back and serve them in the way I can

MALVAR, JUNINA ROSE I.

Two weeks of exposure at Purok 6, Creeksite, Muntinlupa City. With this time frame I

had observed and learned a lot from this place. I was able to know more the place and met the

people who are very active and participative in every activity our group had. Then, I realized that

my school has established rapport in this area that is why they are all welcoming and happy

whenever they see the group of Adamsonian students.

On my exposure in this area I was assigned to be part of the education group. This

group aims to identify the total number of out of school youth and adult or older adult in this

place. When our group (the education group) conducted the survey, we thought that it is

impossible to finish the whole area with just three members because it has 10 blocks. But, our

group was able to finish it on time and had some talked with the respondents. I found out to the

interview with the community people that most of them though they are undergraduate they

finished vocational courses through TESDA. I was glad to hear stories from them on what they

finished and how it helped them. There were also sad stories about those out of school youth who

chose to stop their studies because they are no longer interested, some are bullied by their

classmates, lack of financial resources, had accident, and enjoyed the company of friends than

going to school, got pregnant and the need to work. There were also adults who want to continue

their studies but the problem is that nobody will take care of their children if he/she’s not home.

Some do not want to continue and focus their attention to the studies of their children.

I also observed in the area that their houses are already furnished after fire

accidents burnt down their homes. This area is really a flexible community. There are lots of

factors that hinder the community to progress one of this is the education that is mentioned

above. Studies are important aspect in human lives for this will tell your future what are you will

be. I hope that there are other sectors that could help them to be a community of prosperity and

peace and not just bring hope to them but make their dreams come true.

OPOLINTO, JOHN MICHAEL R.

It was our second time in Creeksite, Barangay Sucat, Muntinlupa City for our community

duty, and I was already familiar with the place so I was more comfortable in communicating

with the people there and coordinating with the officials of the barangay. We returned with the

continuation of our “service” for them, which has started last first semester. On our past visit

here, we focused more on data gathering and teaching them how to take the vital signs. For this

semester, we were assigned to continue teaching some aspects of the health assessment, which

are the Body Mass Index and the utilization of Gomez Chart. Our clinical instructor also gave us

the task to gather information from the out-of-school youth and the adults who weren’t able to

finish elementary.

For me, I was again challenged, not only because I was the group leader, but also due to

the division of labour, since we were less in number compared to last semester. We had to work

harder and endure the striking rays of the sun and the pollution around the area. As student

nurses, we have to go through all kinds of areas, be clean or untidy. We met again with our

previous participants and we were happy because they appreciated our presence again. They said

that they were very thankful for us coming back to their area because they really wanted to learn

the basic nursing skills so they could use it with their loved ones or with their neighbourhood

when need arises. I felt so glad and blessed as well because I proved that they really need our

help.

After that, we continued with our activities and as the days went by, I became closer with

the residents and the participants. I became more sociable and more vocal with what I want to be

heard. At the back of my mind, I thought that being a community nurse was not bad at all, that

you just have to be equipped with the strength, passion, and social skills to go through it. This

rotation was really a game-changer for me. I learned so much and somehow I improved some of

my imperfections. How I wish I could stay there for a few days longer and know them more. I

will never forget the things I’ve learned, the people I’ve met, and the great help I’ve given for the

residents. A simple act of kindness, appreciation, and love could reach far.

The BSN-301 Vitae Custodes had their 2nd semester community duty for 2 weeks from January 26 to February 6, 2016 at the Creeksite, Sucat, Parañaque together with their Clinical Instructor Ms. Flordeliza U. Donato.

Our duty was actually a continuation of what we had for the last semester. We implemented the things that we planned last semester. Our group was divided into two, which was composed of the Health Team (John Michael R. Opolinto, Sharamina U. Rivera, Fenella Camille C. Toyco and Sha Yaoru) and the Education Team (Carmi P. Fernandez, Iman Patrick L. Kiamco and Junina Rose I. Malvar). We, the Health Team had them reviewed about the Vital Signs Taking and glad to say, all of the participants still remembered it. They can take the vital signs independently, though, some needs a little bit of guidance. We also reviewed them about the BMI taking. And again, all of them can do it already. We also thought them on how to use the Gomez Chart which was absorbed by them easily. The following days we had the Operation Timbang from Block 1-10 together with the participants. After the activity, the results were made as the participants' homeworks for them to know if the children in their blocks are underweight, normal or overweight. And of course using the Gomez Chart. Proud to say again, the participants had very good results on their homeworks. As for the Education team, they had a survey from Block 1-10 about the Educational status of the people in the community.

The experience in the community was good. Everytime we go in there, it makes me appreciate more the kind of life that I have. I am very thankful of everything that I have, indeed. Also, I was very touched with the participants' participation through our 2 weeks exposure. They were really very active in our activities, though some were not, because of some instances and their very busy schedule which we understood, some were really there showing their effort and showing interest wholeheartedly. It was really heart-warming to know that every detail you had taught them was really used in their everyday living. Some even shared their experiences for the past months and how the learning we imbibed in them was really applied. They thanked us so much for the knowledge that we shared to them. It is really a lot of tickle in the heart how they showed their gratitude towards us.

I really am beyond very happy to see in my memories and remember their precious smiles and appreciation. It's like a photographic memory that reminds you how worthy you are as a human being and a sibling to God's other children. Thank God for allowing me to touch someone's life and imbibing knowledge in our own little ways. One of the grateful experiences a student nurse can have, indeed.

SHA, YAORU (ANNA)

2015 /1/26 is the first day that we went to the SUCAT for our community duty. The first

day we attended the morning meeting with all SUCAT committee members, and in this meeting

we also discussed the things during our 2 weeks of stay, and also we wanted the people who

lived in the SUCAT to help us give us the cooperative.

Last semester in the SUCAT community duty, we taught people there that vital sign,

herbal medicine, the colds, also the dengue. So for this 2 weeks duty in the SUCAT we will

continued the vital sing taught, we will help members to review the skills and also taught them

the compute BMI. In the first week we invited the all the members that last time who attended

our class, but finally just 6 people attended, so our student nurse help them to review the skill of

how to measure the vital sign also review the indication for the 10 herbal medication, also our

student nurse we also review and have the quiz about those herbal medication.

After the first week we done taught vital sign, herbal medication, and the compute the

BMI, our student nurse divided 3 groups, the 2 groups with the 6 member went to every block, to

find children to get the birthday, weight and height. After this, we lent member go back home

and compute, the other day we check their homework, and let them explain those children which

is normal, overweight, underweight and obesity .check wrong answer and give them correct

explain .and the 3rd group they still go to house to house do the serve during these days, we all

no classmates upset, we all do good and offer the foods for people every day. The very glad thing

is during the last day all people leader very appreciate our duty, and wait our next duty in

community.

When finished this 2 weeks duty in SUCAT. I find that even the people are very poor

there but they still feel happy and everyone have good relationship, the main problem for them

need to change i think is environment, even you are poor, but people have clean place to live

according to Nightingale rule they will more chance to decrease the disease incidence.
