Key Definitions

Community Health Needs Assessment (CHNA)– a systematic process involving the community, to identify and analyze community health needs and assets in order to prioritize these needs, and to plan and act upon unmet community health needs. Differs from community needs assessments used to identify physician needs.

Implementation Strategy (IS) – hospital’s plan for addressing community health needs, including health needs identified in the community health needs assessment. The implementation strategy is often considered the hospital’s overall community benefit plan.

Conducting a Community Health Needs Assessment

A community health needs assessment is a systematic process involving the community to identify and analyze community health needs and assets in order to prioritize, plan and act upon unmet community health needs. This process results in a product: a summary report.

1.1. Determine who in the hospital (MBO or region) will participate in the needs assessment process

Step 1: Plan and prepare for the assessment

• Determine who will lead process and participate in the process• Form an internal assessment team • Engage appropriate internal departments - community benefit, strategy, tax, finance,

population health management, clinically integrated networks, mission, communications, clinical areas, others as appropriate

• Be certain everyone understands their role in the process

1.2. Plan for community engagement

Step 1: Plan and prepare for the assessment

• Preference is to conduct the assessment as part of a community partnership effort• Be sure to include representatives from populations that are minority and vulnerable • Involve community coalition as appropriate• Questions to consider:

o Are you currently collaborating with any community partners as a part of a Mission and Ministry Fund grant?

o Is there a community planning process already in place?o Who was involved in the initial assessment three years ago?o Are active community groups in place?

1.3. Engage hospital board and executive leadership

Step 1: Plan and prepare for the assessment

• Involve executive leadership from the beginning• Within CHI, determine what governing body will adopt the implementation plan for your

hospital• Implementation strategies must be adopted by “governing body authorized under state law

to adopt the implementation plan”• Plan assessment timeline to allow for governing body adoption prior to June 30, 2019• Determine how assessment process and progress will be reported to Board and executive

leadership

1.4. Determine how the CHNA will be conducted

Step 1: Plan and prepare for the assessment

• Conduct in partnership with others in the community • Determine if you will conduct your own assessment or collaborate with others in

conducting a joint assessment• Determine if similar health assessments have already been conducted by your local health

department, federally qualified health center, or school of public health• Determine if you will use a consultant and their respective role in this process

1.5. Identify and obtain available resources

Step 1: Plan and prepare for the assessment

• Identify existing resources and infrastructure o Your hospital resourceso Other hospitals or collaborative partners o Local colleges, universities, schools of public health o Individuals with assessment experience o Others already conducting assessments o In-kind participation and support

1.6. Develop a preliminary timeline

Step 1: Plan and prepare for the assessment

• Expect to spend approximately 6-18 months planning and conducting the CHNA• Final IRS deadline is June 30, 2019• CHI will require an acknowledgment form to be signed to ensure compliance with IRS

requirements• CHNAs may be required to be submitted to CHI prior to IRS deadline for audit/review of

requirements• Internal timeline should consider use of consultants, board approval and meeting dates,

and other internal time limiting factors
