Introduction
The Patient Protection and Affordable Care Act, enacted March 23, 2010, added new requirements for nonprofit hospitals to maintain their tax-exempt status, including a requirement to conduct a Community Health Needs Assessment (CHNA) once every three years to gauge the health needs of their communities and to develop strategies and implementation plans for addressing them.
Additionally, following the CHNA a Community Health Implementation Plan (CHIP), which defines specific goals on how the prioritized needs will be addressed, is required. A yearly Community Health Action Plan (CHAP) will provide the specific actions and metrics for each goal.
The CHNA, CHIP, and CHAP were conducted in compliance with these new federal requirements and as an opportunity for [Hospital Name] to fulfill our commitment to our organizational mission to “extend the healing ministry of Christ by caring for those who are ill and by nurturing the health of the people in our communities.”

In our community, 22.5% of adults don’t get the recommended leisure time activity, 20.4% of adults smoke, and 83.4% of adults eat less than 5 fruits and vegetables per day. Over 43.6% of adults are overweight, and 34.5% are obese. These numbers are much higher than state averages, and point to our need to improve healthy eating/active living in our community in order to fulfill our mission to extend the healing ministry of Christ by
caring for those who are ill and by nurturing the health of the people in our communities.
Partners and Inputs: LiveWell Finney County, Kansas Health Foundation-Healthy Communities Initiative,Garden City Com-munity Health Advisory Board, WorkWell Kansas, Kansas Department of Health & Environment
Utilizing evidence-based programming, we will provide diabetes prevention classes and diabetes screening to the public. Classes will be offered free-of-charge in the community and classes will be held at St. Catherine Hospital for patients referred by hospital and community providers.
ACTIVITY 1: Deliver Diabetes Prevention (DPP) and Screening classes to the community and hospital clinic patients.
METRIC: Continue partnership with Garden City Community College to deliver courses once per month beginning January 1, 2017. Using in-house and community provider referrals, offer monthly DPP classes at St. Catherine Hospital Siena Medical Clinic.
ACTIVITY 1: WorkWell Kansas Programming provides worksite based programs in lifestyle changes including weight management, active living, healthy eating and stress management.
METRIC: A trained St. Catherine Hospital associate will host a WorkWell Kansas Mental Health and Stress Management Training for Worksites.
ACTIVITY 2: We will continue ongoing wellness work with employees of Finney County through their Employee Wellness Programs.
METRIC: We will deliver a series of 30 minute stress management classes for Finney County employees.

In our community, almost 16% of adults report heavy alcohol consumption and roughly 23.4% of adults report a lack of social or emotional support, compared to 15.7% in Kansas.Additionally, the rate of violent crimes reported by law enforcement is 384.5 per 100,000 residents, higher than the Kansas rate of 363.6 per 100,000.

Health Equity/Access to Care
By increasing access to healthcare, we seek to both prevent illness as well as reduce costs to the community by preventing more serious complications through consistent care and medical attention. According to Small Area Health Insurance Estimates, in 2012 26.3% of adults were uninsured and 10.2% of children were uninsured in our community. This is higher than the Kansas rates of 17.6% and 7.1% respectively.

Create a case management-linked program for bi-directional referrals and continuity of care across the community.
Provide free asthma screening to all fifth grade students in Finney County.
METRIC: Case manager will work with inpatient staff, patients and providers to develop all post discharge continuity of care planning, especially ensuring that patients admitted through the Emergency Department are assigned a primary care provider upon discharge. The case manager at Genesis will be in touch with case managers at St. Catherine Hospital to provide upstream referrals and continuity of care. This new partnership will improve access to care and appropriate follow-up.
METRIC: During Quarters 3-4, 2016 provide asthma screenings and education to all fifth grade students in Finney County through organized outreach and partnership with school personnel. Some children will be newly identified as asthmatic or at risk and will be referred to primary care providers. Teams may also discover children who do not have a designated provider, or perhaps do not have insurance coverage. These children will be referred to the navigator and case managers listed in this section.

METRIC: The majority of the employees at Tyson Foods are members of the refugee or Latino communities. These groups have been identified as having limited access to healthcare. While all employees are provided health insurance after 90 days, many do not understand that they have the ability to enroll their families. Likewise, many do not understand how to navigate the healthcare system in the community. The navigator(s) will be able to assist with enrollment and setting up a relationship with a primary care provider in the community.
Develop and implement a work-site benefits enrollment and navigator program in partnership with the primary employer of minority and refugee members of the community.
ACTIVITY 1: Develop and deploy a partnership with Tyson Foods Human Resources staff to provide a culturally appropriate health navigator to attend employee orientation and open enrollment.
