date. To be signed by Managing Director / General Manager

3. Chapter 2. HSE organization structure is not mentioning top management hierarchy and organization chart is

separate document subject to revision.

4. Chapter 7. Responsibilities of staffs appear to be generic and are neither specific nor measurable in some

cases. Responsibilities of staff indicted on safety org-chart are not provided.

Blank copy of JHA was provided in the attachment. Details of risk assessments will be available

as per Work Method Statement submitted.

7. HSE Incentive Scheme/Program and Environment / Waste Management Program needs to be specified.

HSE Incentive Scheme/ Program is attached. Waste Management Program is available in

manual.

8. HSE plan shall be reviewed periodically and Safety Manager must be available on site full time basis.

9. Contractor shall submit weekly and monthly HSE performance report in Hill - International provided format.

10. Contractor shall report all Near miss incident/ dangerous occurrence, Fire incident, Accident/ Incident

immediately via telephone to Hill International. Near miss incident/ dangerous occurrence, Fire incident,

Preliminary Accident and Detail Accident Report must be provided within 24 hrs and 72 hrs respectivelyincorporating Hill International provided format.

NOTED

11. HSE monitoring and enforcement for the project is a contractor sole responsibility. NOTED
