Guidelines• Utilises telephone, mail and consultation• Provides individualised care by Nurse• Works in partnership with the patient’s GP• Provides structured 12 month follow up
From our web page Cardiac CoachingResearch has shown patients who have recently had a heart attack , cardiac surgery or coronary angioplasty are more likely to have a healthier, better outcome if they participate in Cardiac Coaching in addition to their normal GP and specialist follow up. Berwick Medical Centre has introduced a cardiac coaching program in 2007. The program is delivered by Sister Sue Bradley . Sue has post graduate qualifications specifically in cardiovascular disease prevention from the University of Melbourne and the Anti Cancer Council in addition to her aged care and diabetes education skills. She has many years experience from working in Cardiac Rehabilitation courses. Patients will be enrolled by their GP. Sue will contact them by telephone at designated intervals over the 12 months after their cardiac event and will then arrange an appointment with herself to discuss goals, treatment and self help.

4 sessions quarterly following event
How does it work
• Determines the patient’s knowledge of their risk factors
• Establishes targets for those risk factors• Educates patients in their practice of
lifestyle measures• Assists with adherence to drug therapy• Reinforces a chest pain emergency plan
Parameters• Smoking: Cease smoking. Avoid passive smoking• Nutrition: Reduce trans fats to < 8% of the diet• Alcohol: 2 or less std drinks/day males, 2 alcohol
free days. Max 4/day 1 or less std drinks/day females, 2
alcohol free days. Max 4/day. Maximum 6 on exceptional celebration days.
• Physical activity: 5 x 30 mins/day moderate activity. May be taken in 3 x 10 min sessions.
