MISSION STATEMENT “Our mission is to serve God in fulfilling Christ's Great Commission by raising up and equipping

disciples out of the ranks of those afflicted by substance abuse and its related emotional disorders."

PROGRAM PHILOSOPHY It is the vision of the founders of His House Recovery Residence, Inc. (HHRR) to provide quality, effective, residential substance abuse treatment, at an affordable cost to the Client, while maintaining integrity with other treatment professionals and the criminal justice system.

INTRODUCTION The founders of HHRR believe that in order for the treatment of addiction to be effective it must be holistic. In addition to the treatment of the primary illness of chemical dependency or substance abuse it must also address the psychological and social behaviors, which precipitate substance abuse and are exacerbated by prolonged, chronic use. The overt manifestation of addiction is characterized not only by compulsive use of mind altering and/or mood changing chemicals, but is also associated with other self destructive intra -personal and interpersonal patterns of behavior. Long-term recovery is dependent upon changing these self-destructive behaviors. Addiction is progressive. Left untreated, and addict’s physical and psychological dependence upon their drug of choice continues to escalate. The negative impact upon an individual’s physical and mental health increases as substance abuse escalates. The potential outcomes of untreated addiction are incarceration, impairment of physical and/or mental health and in extreme cases, death. Addiction is chronic in nature. The best outcome a Client may hope for is remission. Attempts to return to a "controlled” or “recreational” use of any mind altering and/or mood changing substances will reactivate the compulsion to use and bring about a return to behavior patterns that existed prior to treatment. The onset of symptoms returns more rapidly than when the behavior was first acquired. The belief in total abstinence from any mind altering or mood changing substance is the foundation of the HHRR philosophy. HHRR holds the belief that addiction is treatable providing the addict is willing to work a daily recovery program. It is further believed that treatment of an addict’s family and significant others enhance the probability of long-term recovery and reintegration into the family structure and community. Left untreated, addiction is believed to be a terminal illness. Death may occur from primary physiological symptoms of alcohol or drug use (e.g., cirrhosis of the liver) or the secondary effects (e.g., cardiovascular disease) Death may also be the result of alcohol and drug related accidents or suicides. Additionally, there are affective implications in long term, untreated addiction. Feelings of guilt, shame, and unworthiness lead to social isolation and failure of interpersonal relationships. Addiction impairs the ability of the Client to delay the need for gratification and to interact appropriately with others. Decisions are made based on the need for immediate gratification. These decisions are usually self-serving and self-centered, indicating a lack of or an arrested spiritual development. His House Recovery Residence Page | 1 Revised: September 15, 2014

PRINCIPLES OF TREATMENT HHRR adheres to the following beliefs about the treatment that it provides to clients: 1. Effective treatment of addiction requires a multidisciplinary approach. Skilled professionals trained in the psychological, social and spiritual aspects of recovery work together in planning each phase of a Client’s treatment. These include, but are not limited to: the Executive Director, Counselors and Ancillary staff. Consultation with outside medical and mental health professionals is sought when requested by the Client or determined to be necessary by the treatment team. 2. HHRR encourages the Client’s family and significant others to become involved in the treatment process. This involvement takes the form of referral to family support groups and family/marital therapy as indicated. The primary goal of outpatient and recovery residence treatment is to reintegrate the Client into his/her family and community systems as soon as possible. 3. The structured and supportive environment provided by HHRR makes it possible for clients to address the emotional changes brought about by long-term addiction. Shame, guilt, low self esteem, fear and anger are common psychological themes which clients are able to explore. Special issues of men and women are addressed in gender specific groups. 4. HHRR believes that an essential component of a Client’s treatment program is the development of a spiritual life including an active relationship with God through Jesus Christ. Regular attendance of a 12-step program is required and participation at God’s Rolling Thunder Church is mandatory. On rare occasions attendance at the Client’s home church will be allowed when approved by the Director and accompanied by family. 5. HHRR believes that it has an obligation to clearly define its treatment goals to both the Client community and the referral community at large. These goals are:

a. The Client will abstain from all mind altering and/or mood changing chemicals; b. The Client will verbalize an understanding of the disease concept of addiction; c. The Client will identify and express feelings in an appropriate, healthy manner; d. The Client will become involved and participate in a 12-Step fellowship regularly. The Client will get and use a sponsor regularly; e. The Client will perform daily chores, daily meal preparation, weekly food purchases and other living skills as assigned; f. The Client will obtain employment and assume financial responsibility for all treatment fees. Disabled clients may substitute volunteer work for employment; g. The Client will participate in leisure activities and therapeutic leaves, using these leaves to repair damaged family and significant other relationships.

His House is a supportive, structured living environment for addicts who are new in their recovery. Clients are subject to nightly curfews, random urine screens, and are required to attend Alcoholics Anonymous or Narcotics Anonymous meetings on a regular basis, in addition to weekly groups at His House. Clients are required to work, unless there is a physical or mental disability, which prevents them from doing so. It is strongly recommended, especially in parole and probation cases, that a Client remains for completion of the entire program, which is approximately eighteen months. The actual length of time for completion of the program is determined by the Clients’ participation and progress in the program, not a predetermined amount of days in the program. We strongly believe that the Criminal Justice System is overrun with cases that are directly related to an individual’s addiction problem. We have found that the treatment provided in the prison system isn’t enough. While providing education to the addict, they do not afford the person the opportunity to practice the necessary life skills they have learned. At His House, the Client is allowed to implement the skills in which the recovery lifestyle requires, while still under structure and supervision. We feel that our program is an effective alternative sentencing strategy to truly rehabilitate addicts whose disease has led them into legal problems. The formation of the His House program is a culmination of ideas and experiences of several people actively participating in 12-Step programs of recovery. His House staff has found that the ideas and theories taught in the 12-Step fellowships implemented in a structured living environment greatly enhance the chances of recovery. His House recognizes that the disease of addiction affects the addict physically, mentally, emotionally, and spiritually, and that the success of any program of recovery lies in daily commitment to a 12-step program. His House believes that regular attendance and application of the principles of these programs are the foundations for that success. The His House staff is made up of people who are professional treatment providers and/or recovering addicts, themselves. We are available to clients to help in teaching the twelve steps and the practical application of these principles in daily living. The programs of A.A. and N.A. teach us that there will come a time when nothing will stand between the addict and a drink or a drug but his relationship with his Higher Power. We, therefore, encourage spiritual growth.

ADMISSION of Clients His House staff will conduct a personal screening interview with all prospective Clients and appropriateness for admission will be determined on an individual basis. Only those who are willing to seek recovery shall be admitted. Upon approval for admission, Clients are requested to have the following recent treatment records forwarded to His House, if available: diagnostic summary (with Multi-Axial Assessment), social history, treatment plan, and aftercare plan and medication list, if applicable. State law requires a current (within the previous 6 months) PPD and RPR on the day of admission; Clients will not be admitted without these vital medical records. On the day of admission to His House, staff will conduct an entry interview with each Client to include a personal data history, fee contract agreement, rules and regulations agreement, other applicable paperwork and unit placement. COMMUNITY LIVING Life in a residential treatment community is a new, and sometimes confusing, experience for most clients. To minimize the stress involved in early recovery, His House has developed a structured program that exists to enable the total community to function in a constructive and supportive atmosphere. At the time of admission to His House, each Client is fully briefed concerning all rules and regulations of the residential requirements. The His House staff provides supervision of the residential community to include individual, group and family interactions. Staff members are available on a 24-Hour basis. Each unit is under the direction of a Senior Client, then the Case Manager, and finally the Program Director. Each member of the Community is required to attend specific groups each week. Part of the Community Meeting function shall be to introduce new members to the community, to positively reinforce responsibility in the clients, and to educate clients in areas directly related to substance abuse. In the Peer Group Meeting, the Senior Clients have the responsibility of confronting irresponsibility and giving consequences, reviewing meeting attendance, and reviewing Therapeutic Leaves. Staff members will be present at these Peer Group Meetings in an advisory capacity; however, staff reserves the right to make final decisions. Clients will be required to attend weekly Group Therapy, Educational Groups and Individual Case Management/Counseling sessions, as scheduled. All clients will be admitted into Orientation Phase before moving into any other Phase.

TRANSPORTATION For liability purposes, His House Recovery Residence does not provide transportation. Clients may bring their personal vehicle to His House if they provide a copy of their valid driver’s license and a copy of current proof of insurance card on the vehicle to be driven while in our program; however, Clients may be subject to surrender of their vehicle as a consequence for various rule infractions. Public transportation is available to all Clients. Clients without personal vehicles are encouraged to seek transportation from other Clients, sponsors, or appropriate contacts in the 12-Step fellowship (transportation not supervised by His House staff). Our rules require that all passengers in a private vehicle must be with all males or all females, unless prior approval for co-ed transport is obtained by the staff.

HIS HOUSE'S RELATIONSHIP WITH OTHER AGENCIES His House Recovery Residence places a high priority on treatment. Our staff maintains constant liaison with other treatment providers and criminal justice agencies regarding Client progress via telephone consultation, on-site Client review conferences and written reports. If His House is a secondary treatment provider to the primary outpatient program, counselor, doctor or other healthcare provider, we require that Clients follow their primary care givers treatment plan. If a Client fails to follow through with outside treatment commitments while at His House, the Client will be subject to discharge. His House Recovery Residence Page | 4 Revised: September 15, 2014

There is a 3% processing fee for paying online. We use Paypal and are charged a 3% fee for using the Paypal

service. The 3% processing fee is only charged to cover this cost. Online Payments need to be made by the

appropriate payment deadlines. If processing fees or late charges are not included in the online payments,

they are still due and will be collected from the clients.

COURT SERVICES Jail screening and court appearance fees may vary from one jurisdiction to another,

depending on distance of travel. The Metro Atlanta area includes City of Atlanta, Fulton County, DeKalb County,

Cobb County, Clayton County and Gwinnett County. If your jurisdiction is not listed, please contact our office for

a fee amount. Metro Atlanta Area Fees or jail screenings are $100.00. The defendant's attorney should make

arrangements with the appropriate authorities for a contact visit with our staff member to conduct a Mental

Health Screening for Substance Abuse. This correspondence should be forwarded to our office as well. Metro

Atlanta Area fees for Court Appearances are $100.00 for a half day, then $50.00 per hour thereafter. Our Staff

needs as much advance notice as possible to schedule a Court Appearance. Fees for Jail Screenings and Court

Appearances are to be paid in advance. Be mindful that the Court is not obligated to take our recommendation.

Court Appearance Fees are non-refundable regardless of outcome in Court. Note: Fees for Services not listed

will be determined by the Executive Director. Fees are subject to change. Please consult with the Executive

Director to ensure current Schedule of Fees.

His House Recovery Residence Rules & Regulations

INSTRUCTIONS: Read and initial in the blank provided for each rule. Do not sign the Rules Agreement without a staff member present. It is reasonable to expect and encourage Clients to assume reasonable responsibilities. Greater individual involvement by Clients in their care increases the likelihood of achieving the best outcomes and helps support a quality improvement, cost-conscious environment.

Those responsibilities include: 1. Clients have the responsibility to be a full participant in the formulation of their Holistic Recovery Plan. 2. Clients, have the responsibility to follow their Holistic Recovery Plan. 3. Clients have the responsibility to provide the service delivery staff with `all required information to

maintain proper and correct records. 4. Clients have the responsibility to attend required group/individual counseling sessions and be on time

to groups and individual counseling sessions. 5. Clients have the responsibility to treat their therapist or paraprofessional with dignity and respect. 6. Client’s performance during the treatment will be reported to the referral source, when it is required

for treatment compliance with court orders. 7. Clients will not possess any drug without prescription, or alcohol including mouthwash and medications

containing alcohol. 8. Clients will be required to take random drug / alcohol tests. Refusal is grounds for immediate

termination from the program and premises. 9. Clients are responsible for any keys issued them. Replacement keys are $5.00 each 10. No Pets Allowed. 11. Clients will be fully dressed in the common areas at all times 12. Violence, or threats of violence, emotional or physical, will not be tolerated. Conditions requiring

contacting authorities will result in the immediate dismissal of the person(s) involved. 13. No weapons are allowed on premises or in vehicles. 14. All vehicles must be approved with the submission of proof of insurance and ownership. Vehicles must

be in operating condition. 15. NO gambling of any kind. 16. Clients are not permitted to frequent environments detrimental to their recovery such as, bars, lounges,

liquor stores, taverns, etc. 17. No smoking in the house 18. No pornographic materials or media or secular music is allowed in ministry houses or vehicles. 19. Common areas are for relaxation and community interaction. No sleeping on the couches. 20. Laundry hours are 7:00 A.M. until 10:00 P.M. daily. 21. Clients are not permitted to loan money or automobiles to other clients. 22. All Clients will participate in chores. Chore List will be posted. Initial______

Schedule of Program Fees

1. Payment for participation is $200.00 per week or $800.00 per month. A $30.00 Late Fee will be assessed if not paid by 12:00 Noon the day following their due date.

2. There will be no pro-rated refunds for any dismissal for rule violation. (This means if you pay on Friday and are dismissed on Saturday there will be no money given back. No Exceptions)

3. Urine Drug Screen, $3.00 4. Transportation Fees, $2 within two miles, $5 within twelve miles, greater distances will be set on a

case by case basis. A wait time charge of $10/hr will be assessed to all trips except the “Walmart Run” Saturdays. Trips to the office, meeting and church are free. However, small donations for gas

Recovery

1. All Clients must have a counselor or sponsor whose name and number will remain on file. Clients are expected to have regular contact with their counselor or sponsor at least once a week. House personnel are not admissible as sponsors.

2. All Clients are required to attend recovery meetings: two weekly 12 step programs, church and therapeutic counseling are required. All meetings are required. Outside Meetings are documented with the signature of the chairperson. Forms are available.

3. If the Client is transitioning from a treatment facility all aftercare recommendations must be followed. 4. If on probation with a legal agency, the resident must adhere to all requirements made of them. This

includes informing their probation officer of their current participation at His House. 5. Group House Meeting, Wednesday's Bible Study, Sunday morning Church, Emergency House Meetings,

Scheduled Recovery and Process Groups and at least 2 outside recovery meetings are mandatory and attendance is unconditionally required unless an exception is made by a staff member. Sunday attendance at God’s Rolling Thunder Church is mandatory. Failure to attend a mandatory meeting will result in a $20.00 sanction for each missed meeting and will be due with the following week's program fees. Failure to pay sanction fees when due will result in a $30.00 Late Fee. Initial______

Urine Drug Screen Policy

1. Random urine drug and alcohol screening is a significant aide in the recovery process and is practiced at His House for the benefit of everyone participating here. Random screens are administered and supervised by staff. There is a $3.00 fee for the screen. A list of banned substances is available from the Director.

2. A positive screen may result in a minimum 5-day expulsion from the program. During which time all fees must be kept current and no refunds will be given. Client will be referred to an agency for evaluation and counseling.

3. The cost of a Alcohol and UDS at Cherokee County's Lab is $35.00 and is your responsibility. 4. Re-admittance to the program may require you to report to the Cherokee County DUI / Drug Court

Lab no later than the following business day and submit to a drug and alcohol screen. Then after 5 days you must return to the same Lab for a follow up screen and the 2nd screen must indicate a lower presence of the banned substance before you will be allowed to return to the premises.

5. Failure to pass 3 urine drug screens at anytime will result in immediate termination from the program.

6. Re-admission to His House will be considered after 7 days and will require an additional $150.00 Relapse Assessment and Re-Entry Fee.

7. All failed screens will result in the forfeiture of off premises privileges, except for home, work, church, and supervised recovery meetings, or accompaniment by a house member with at least 30 days of sobriety. Failed drug / alcohol screens are reported to probation or the court when applicable

Off Premises Policy

1. All off premises passes are allowed after 30 days and are subject to a UDS before and after the pass is granted.

2. House Members, during their 1st 30 days are not allowed to leave the premises except for home, work, church, and supervised recovery meetings, or accompaniment by a house member with at least 30 days of sobriety.

3. Weekend passes are allowed after 30 days with 3days prior approval and must include at least 1 recovery meeting while on pass. Initial______

4. Phones and Computers

1. Office phone and computer are available for Client’s use. Common courtesy is expected when others are waiting. No calls longer than 15 minutes.

2. Cell phones are permitted. Conversations should be restricted to common areas out of respect for your neighbor. All ringers must be off by 10:00 P.M. Calls after lights out should be kept to a minimum out of courtesy to others.

3. The Office computer is available for Client’s use in job search activities and checking email. No game playing browsing adult websites, chat rooms etc.. No Downloading! Initial______

Personal Living Area

1. Bedrooms are the sole domain of its occupants. 2. No guests are allowed in bedrooms. 3. All rooms are to be kept clean and beds made daily prior to leaving the house. 4. His House is not responsible for losses or theft of personal property, including money, jewelry, clothing,

etc. 5. No storage space is available for personal belongings beyond that which can fit in one’s bedroom,

however, bicycles and work tools may be stored with prior approval and is contingent upon availability of space.

6. Bedrooms are subject to daily inspections and search. 7. Staff Members Rooms are Off Limits, except by the expressed permission of that Staff Member. 8. A minimum $10.00 Fee will be assessed for each Sanction. Multiple sanctions can be assessed for

failure to keep one's area in compliance with rules. 9. House Chores are assigned weekly and must be completed to Staff's satisfaction 10. No swimming pool privileges or off premises passes will be allowed until your area is in compliance and

chores are satisfactorily completed. Failure to satisfactorily complete weekly chores by Sunday will result in posted sanctions:

Kitchen – Hours 5:00 A.M. until 10:00 P.M.

1. Paper towels cleaning products, light bulbs and trash bags are provided. 2. All Clients are expected to clean up after themselves when cooking. Pots and pans are to be cleaned

and put away. Plates, glasses, and silverware are to be put in the dishwasher. 3. When available, each Client will be assigned a separate cabinet for personal food storage. 4. Refrigerated items are to be labeled with Client's name. Unidentified items are fair game.

1. Guests of the opposite sex are not allowed on the premises between the hours of 10:00 P.M. and 7:00A.M. This includes sitting in a car talking in the driveway.

2. Visitors are allowed in common areas only. No visitors are allowed in bedrooms. 3. Clients are responsible for their guests and must ensure that they behave in accordance with the same

rules as the Client. 4. House management must be informed of all overnight leave from the house.

Significant Others

One of the most frequent causes of relapse is getting involved or reconnecting with a significant other early in recovery. Because of this we strongly suggest that Clients do not date while in recovery at His House for a minimum of 1 year. If, however, the Client is married or engaged prior to occupancy, one is required to identify that person and the status of the relationship to house management by the time of acceptance into occupancy. Employment

1. All Clients are required to work unless disabled. (30 hrs. minimum). 2. Clients must inform house management of job changes. 3. Unless arrangements with Staff have been made all Clients will be out of the house between the hours

of 8:30 A.M. until 3:30 P.M. and must be either at work, volunteering, or seeking employment. 4. Evening and Night Shift work which interferes with mandatory requirements is not permitted.

Warnings and Eviction

1. Violations of any of the following will result in a warning. Accumulation of more than two warnings in a four-week period will be grounds for termination from the program.

a. Failure to do assigned chores. b. Illicit use of computer or phone. c. Failure to maintain personal living area. d. Disturbing others after lights out. e.
