Check that all providers are listed and then click on Next

Confirm you have all of your providers listed then click on OK

Type in the number of well child visits you had and the number of books distributed between January 1, 2015 and June 30, 2015.

Type in the name and email address of the person responsible for ensuring that new providers are trained in the program

The #1 spot should say MDAAP. If you have received donations/used books, you can put their names on the other lines. (For examples, staff, Girl Scouts, etc.)Check any that you provide and then give the number of literacy referrals you made. It is OK to put in 0.
