What you need to know….

• No action required if you don’t want to make changes.

• If you do nothing, your current coverage will continue in the new year except FSA – FSA requires active election each year.

• The same plans will be offered in the year effective October 1, 2104 at rates shown in the Benefit 101 Enrollment Guide.

• You can make changes or enroll online 24/7 until August 22nd.

• Call Center is available for making changes or enrolling in Accident/Injury, Whole Life, and Critical Illness coverage.

• Enrollment Period: July 29th – August 22nd

Annual Enrollment Highlights• Passive enrollment = July 29 – August 22

• All current elections roll over into 2014-2015, except FSA.• Employees must re-enroll and make new FSA elections

• PCP required for NEW HMO enrollees• Without PCP designation, one will be assigned• If currently enrolled in the HMO, current PCP will remain on record

• Supplemental Life Insurance Increase • Increase existing Employee Supplemental Term Life insurance by $10,000 (one

increment) without EOI• New Elections for Employee and Spouse Life insurance, and increases in

excess of $10,000 will require Evidence of Insurability (EOI)

• Changing from Plan B to Plan A • Guaranteed approval for up to $30,000 in Supplemental Life Insurance and

Disability plans without answering any medical questions

• Update Beneficiaries for Life Insurance5

Notify the Insurance Department so your enrollment is processed correctly.You may choose which of you will have deductions taken from your pay

You may not elect Spouse Life Insurance because under the Life Insurance policy, you may not be covered as an employee and a dependent.

Only one of you may cover your dependent children.

The spouse who needs medical coverage will be Plan A.

The spouse who does not need medical coverage will be Plan B.

• Employees may increase Voluntary Life by $10,000 without submitting Evidence of Insurability (EOI) during Annual Enrollment

• New elections and increase more than one increment require EOI• Plan A may elect up to a total of $180,000 • Plan B may elect up to a total of $150,000

• Employee and Spouse rates are age banded based on Employee’s age as of October 1, 2014

• Child flat rate for $5,000 or $10,000 • Children who are married and/or disabled are ineligible

• Evidence of Insurability required if electing coverage for the first time or for Employee Life in excess of $10,000

How to Enroll• See page 8 in your Benefits 101 Guide

• Log On to www.benefitsgo.com/claycounty

• Enter your Social Security Number as your User ID

• Enter the last four digits of your SSN as your PIN#

• Enrollment should take 10-15 minutes

• Review your current elections

• Validate your dependents for eligibility

• Click “View Cost” to see how much a new plan selection will cost before clicking “Continue”

• Make sure your confirm your elections

• Print your Confirmation Statement

• Make changes until August 22nd by clicking “Start a new Enrollment”. Benefits selected on your last confirmation shown will be effective October 1, 2014.
