ZANGWILL From t h e PsychologicalLaboratory, t h e National Hospital, Queen Square, London I n 1935, Curran a n d Schilder drew attention t o t h e incidence o f a peculiar language disorder i n cases with acute cerebral disturbances o f traumatic o r toxic origin. This disorder comprised misnaming, paraphasia, discursive a n d incoherent speech, a n d i n some cases bizarre forms o f verbal expression reminiscent of schizophrenia. Although t h e phe- nomena were ascribed i n part to focal dysphasia, t he authors commented on their similarity t o certain types o f language derangement occurring i n t h e primary psychoses. Following Kleist (1914, 1934), they were le d t o place emphasis o n t h e probable role of cerebral dysfunction i n t h e genesis of lan- guage disorders i n general. I n studies of post-traumatic confusional states undertaken during t h e war, Paterson (1942, 1944) w a s likewise impressed b y t h e frequency o f para- phasic speech disorders. H e pointed o u t that although n o t a l l confused patients present a disturbance o f speech, i n those w h o d o t h e picture differs i n n o essential w a y from that shown i n cases o f focal lesion without confusion, e.g., i n jargon aphasia. Indeed i n several o f h i s cases clear-cut dysphasic residua long outlasted t h e acute confusional state. A t t he same time, i t w a s suggested b y Zangwill (1945) that t h e speech disorder i n certain o f Paterson's cases appeared t o transcend a n ordinary dysphasia a n d t o involve a n underlying derangement o f t h e thought processes. More recently, Weinstein a n d Kahn (1952) have devoted paper t o paraphasia in organic disease. Their observations a r e b as ed on a series of 30 cases i n which misnaming a n d other peculi- arities o f language were prominent. These dis- turbances, they point out, never occurred in isolation a n d were always associated with disorientation, denial of illness, a n d related symptoms o f general organic type. T he intracranial lesions consisted mainly o f deeply situated o r bilateral tumours, a n d aneurysms with subarachnoid haemorrhage; i n nearly a l l cases t h e E.E.G. showed bilateral diffuse delta wave activity. T he authors argue that t h e language disorders observed i n their patients differ in important respects from those ordinarily m e t with i n aphasia. F o r instance, words a r e n o t garbled a d t h e rhythm o f speech i s normal. Further, mis- naming w a s commonly referable t o concomitant disorders o f perception a n d orientation, a n d did not necessarily presuppose derangement within th e o f language itself. Emphasis i s also placed o n t h e role o f motivational factors, more especially those presumed t o govern denial o f illness. In con- clusion, t h e authors argue that paraphasia i n cases o f organic brain disease should no t b e classed a s a n aphasic disorder referable t o a focal lesion o f t h e dominant hemisphere. I t i s t o b e regarded a s t h e manifestation o f a more general change i n be - haviour, i n which altered patterns of perception a n d motivation play a n important part. I n t he analysis o f confusional states it i s obviously important t o ascertain whether, a nd i f s o t o what extent, focal signs a n d symptoms contribute t o t h e overall picture. Although t h e attempt made b y Paterson (1944) t o explain confusion a s a mere aggregate o f focal signs m a y well appear t o o atomistic, t h e conception of a global organic reaction-type advanced by Weinstein a n d Kahn (1952) undoubtedly goes t o o f a r i n t h e opposite direction. A s Paterson has pointed out, t h e con- fusional state i s always an individual affair an d shows very considerable variation from case t o case. I n particular, by no means a l l confused patients exhibit paraphasic speech a n d t h e latter does n o t appear obviously related t o t h e depth o r severity o f confusion. Further, defects o f orientation (Paterson, 1944) a n d denial o f disability (Critchley, 1953; Brock and Merwarth, 1957) a r e often par- ticularly prominent in cases in which t h e lesion principally involves t h e parietal lobes. I t might therefore seem probable that both general a n d local changes in cerebral function contribute to t he con- fusional state a n d that these, i n combination, deter- mine t h e incidence o f paraphasia. A t a l l events, o n e m ay suggest that fuller analysis o f language a n d i t s disorders ma y throw light on t h e psychology o f confusional states.
