We are delighted to present our 2013 Annual Report. Though we continue to face challenges, many related to changes in the healthcare system, CJE SeniorLife has achieved numerous goals in the past year by utilizing sound strategic and business practices while staying true to our values of Respect, Advocacy, Compassion, Intention, Innovation and Accountability. We are dedicated to serving the growing and diverse needs of our community in alignment with our mission to enhance the lives of older adults.

This year, we took a slightly different approach in deciding which information to highlight in CJE SeniorLife’s Annual Report. Often, as Board and staff members, we are surprised to learn that even though CJE has been around for 41 years, people are not aware of everything that we do for a wide spectrum of adults, from all socio-economic backgrounds, who live throughout metropolitan Chicago. We are frequently told that CJE is one of the “best-kept secrets around,” often a destination of choice and hope when a family member is experiencing a crisis.

To broaden the community’s knowledge about how CJE is proactive as well as responsive in finding solutions related to the transitions of aging, we asked our staff to provide us with what they feel people need to know about CJE’s comprehensive network of care. This Annual Report, therefore, depicts the Top 20 programs and services that can help older adults and their families resolve a problem, provide enrichment or plan ahead for the future.

We offer our thanks for the hard and dedicated work of our Staff and our Board of Directors. We are honored to have earned the trust of our residents, clients, family members and community partners. There have been many instances when a community member seems astonished to learn that CJE SeniorLife—a nonprofit, diversified and innovative health and human services organization—provides more than 40 programs and services to enhance the lives of older adults. Therefore, this annual report is a compilation of the Top 20 facts that community members need to know about CJE. These 20 items just scratch the surface of what CJE provides to people of all ages—from those who may be considered adult children, to those who may be entering their tenth decade of life (at the present time, CJE has more than eight residents who are 100 or older!). Our range of services continues to swell as we strive to meet the diverse needs of over 19,000 older adults and their family members annually through a complex and integrated network of life enrichment, supportive resources, healthcare and applied research. In addition, the professional community continues to benefit from our well-respected educational and internship opportunities.

1. CJE has a full time rabbi who is a Board Certified Chaplain. As part of the Religious Life Department, Rabbi Dr. Michael Schorin provides Pastoral Care services at both Lieberman Center and Weinberg Community and leads weekly Judaic classes as well.

2. CJE is launching a Skill Based Volunteer Program for experienced individuals who want to keep their professional skills sharp while making meaningful contributions to one of the largest nonprofit health and human services organizations in Illinois...CJE SeniorLife! We seek out skill based volunteers to help with projects like creating newsletters and connecting with synagogues.

4. CJE is at the forefront in finding ways to connect homebound seniors to the larger community. Our Virtual Senior Center (VSC) pilot program, developed in partnership with SelfHelp Community Services of New York, provides opportunities for Chicago’s isolated older adults to stay in contact with their families, friends and community using touch-screen technology. Seniors are able to participate virtually in real-time programs from community centers in four test cities. An evaluation component will measure the effectiveness of the VSC in reducing isolation and improving the quality of life for selected individuals.

5. CJE was one of the first area agencies to use Creative Arts Therapy when working with older adults. Art, music and dance therapy for therapeutic purposes are evident throughout the organization, including Lieberman Center, Weinberg Community, as well as Adult Day Services. This year, CJE and The Art Institute of Chicago collaborated on a program, Art in the Moment, in which older adults with early to mid-stage dementia view art pieces with their caregivers and use special techniques to spark dialogue and engage in hands-on art making. Artwork produced during these visits was displayed in the Museum’s Modern Wing.

Fact: More than 4,000 people attended Shabbat morning services at Lieberman and Weinberg this year. The services are conducted by a rotation of two rabbis, three cantors (one who is both a rabbi and a cantor), three cantorial soloists and one lay person. In addition, classes on various Judaic topics are taught by a number of Jewish educators and lecturers from the community. Our rabbi also arranges for people of other faiths to receive the supportive services of their clergy.

Fact: In the past year, a total of 3,895 individual and group volunteers dedicated over 73,300 hours, amounting to a value of $1,623,249 in services provided. CJE’s oldest volunteer, a Lieberman resident, recently passed away at age 106.

Fact: CHL held over 50 health education/screening events with over 700 attendees in the past year. One of the most popular lifelong learning classes, Learning How to Use an iPad™, helps CJE residents and community members become more comfortable using technology to communicate with friends and access their favorite websites.

Fact: An Art in the Moment iPad “app” and the web-based program were developed as a new technology that simulates the community program using traditional works of art from the Museum. Visit www.ArtInTheMoment.org to learn more how it can help engage caregivers and older loved ones with cognitive disabilities.

3. Our Center for Healthy Living (CHL) helps individuals maintain life balance by fully engaging mind, body and spirit. It actively promotes health and wellness through a wide range of lifelong learning classes and community workshops, including preventative health screenings.

6. Food is one of our specialties! Weekly, 90 volunteers deliver hundreds of hot meals to homebound and frail adults. Also, people in the Chicago area can order delicious, fresh-frozen entrees, side dishes, and breakfast items from Kosher to Go™. Many hospitals, schools, synagogues and organizations also utilize CJE’s extensive range of kosher food services, which includes a full catering menu that is great for the Jewish holidays, family celebrations, or special events.

Fact: Last year, we delivered 100,785 Kosher to Go meals to 558 clients.

7. Your Eldercare Consultants is a new, special division of geriatric care managers. These professionals are trained to provide ad- vice and support to families when they see signs that an older loved one’s situation—such as their health, memory, or ability to manage their own finances—are changing or declining. This year, CJE also launched the YourEldercareConsultants.com website to help families learn the benefits of contracting with a geriatric care manager who can find solu- tions to the transitions of aging. Your Eldercare Consultants provide consultations, ongoing care management and bill paying assistance to maximize an older adult’s independence and quality of life.

Visit YourEldercareConsultants.com for free informational resources for caregivers to explore. “The Caregiving Coach,” a monthly blog on the Jewish Federation website (www.juf.org/news/blogs.aspx), offers practical advice about the everyday challenges that caregivers face.

8. Our Adult Day Services are offered in Evanston, Deerfield and downtown at Sinai Congregation where people with dementia, other cognitive issues or frailty can enjoy socializing by parti- cipating in stimulating activities in a structured setting. While loved ones are at Adult Day Services enjoying creative arts activities such as designing unique hats for an upcoming fashion show, current events discussions or chair exercises, caregivers are provided with respite which enables them to attend to their own needs.

Our award-winning CJE Culture Bus™ provides organized excursions for older adults with early stage memory loss to unique cultural sites and events in the Chicago metropolitan area. Trips have included hands-on experiences at a glass blowing studio, a therapeutic horse academy and Fermilab, and are designed to provide participants with a normalizing social experience while visiting culturally interesting places.

9. CJE sponsors numerous Support Groups. These are facilitated by licensed, certified social workers and provide emotional support, information, resources and coping strategies for different life stages.

10.Weinberg Community for Senior Living in Deerfield was a “first” when, in 1995, it received one of Illinois’s first assisted living licenses. Today, Weinberg Community’s Gidwitz Place for Assisted Living and Friend Center for Memory Care (for people with early- to mid- stage Alzheimer’s and related dementias) are still considered a shining model for other assisted living residences. More than 200 residents receive help with activities of daily living. They are encouraged to participate in fulfilling and innovative life enrichment opportunities and look forward to art therapy, visits from certified therapy dogs, fitness classes, manicures and massages—all under one roof. The annual Weinberg BBQ, Farmers’ Market and Mah Jong tournaments—designed for fun and friendship—bring out many community members, too.

Fact: Hundreds of locations have been visited by Culture Bus in 10 years of operation. This year it traveled over 800 miles on 47 excursions.

Fact: Over 100 support groups were held this year to help caregivers of people with dementia; people with adult children with disabilities; people who have lost a loved one; caregivers of persons with Parkin-son’s Disease; and family caregivers of Adult Day Services Clients.

Healthcare

11. Lieberman Center has heart! Lieberman Center has a new Center for Heart Health in Haag Pavilion for Short-term Rehabilitation. Providing innovative rehabilitation therapy for patients with congestive heart failure or its underlying conditions, it is under the direction of Dr. Caesar DeLeo, board-certified in Cardiology and Internal Medicine and a member of the Cardiology Division at NorthShore University HealthSystem.

Our Center team—attending physician, cardiologist, physiatrist, nurse practitioner and specially trained multi-disciplinary staff—monitors patients, promotes lifestyle changes and teaches physical, emotional and psychological skills to facilitate a successful transition home.

Fact: The Center for Heart Health has treated 30 persons with a primary diagnosis of heart disease since it opened in mid-2013 and has achieved the remarkable readmission rate of zero percent in its first six months.

12. Our new Infusion Therapy Program sets Lieberman Center apart from other skilled nursing facilities. Staff can now administer specific medicines and fluids intravenously to care for patients with clinically complex needs. Antibiotic therapy, nutritional therapy (TPN) and pain management are just a few examples of infusion available at Lieberman Center.

13. CJE has a commitment to people who have Parkinson’s disease. Most of CJE’s clinical staff throughout its network of care is specially-trained to address the needs of people with Parkinson’s. We offer a full range of exercise classes, support groups, art therapy and other rehabilitative services.

14. We are introducing technological advances in our delivery of healthcare. Lieberman Center for Health and Rehabilitation in Skokie is one of the first skilled-nursing facilities in the area to introduce electronic medical records enabling the coordination of care with the local health care system. Lieberman Center is the first skilled nursing facility in Illinois to participate with a hospital (NorthShore University HealthSystem) in the Illinois Health Information Exchange, allowing sharing of patients’ full hospital records for the provision of seamless, coordinated care between the hospital and a skilled nursing setting.

15.Our organizational goal is to improve client and resident outcomes. Staff is closely measuring patient outcomes by analyzing the functionality of patients at the time of entry and at the time of discharge, and is successfully reducing its hospital readmission rate.

16. CJE was one of the first agencies awarded a grant by Centers for Medicare and Medicaid to launch a Community-Based Care Transitional Program. In our resulting demonstration project, designed to reduce hospital readmissions, CJE nurses identify patients at Northwestern Memorial Hospital, Presence St. Francis and Presence St. Joseph Hospitals and, with social workers, follow them for 30 days after returning home. Our staff provides coaching so seniors can understand their illness and maximize the recovery process. Through the care management component, nurses assess each patient, and, if assessed as “high risk,” he or she is referred to our care manager, who contacts the patient or family to suggest additional services to help him or her prevent another hospital stay.

17.Counseling, launched over 41 years ago when CJE was founded by the Jewish Federation, provides individual and group mental health counseling at the Bernard Horwich Building and Weinberg Community. Our licensed clinical social workers for older adults and family caregivers have over 170 collective years of experience.They help individuals who are experiencing issues related to life transitions, like isolation or losing a spouse. Linkages, a program even more important as our community ages, helps older adults who have adult children with disabilities.

Fact: Over 60 people have participated in regular exercise classes designed by licensed physical therapists. These classes focus on flexibility, return to regular movement, and rhythm control.

Fact: This past year, Lieberman Center’s Haag Pavilion for Short-term Rehabilitation achieved a 15% readmission rate to acute care within 30 days of hospital discharge—much lower than the national rate of 26%.

Fact: More than 3,500 hours of face-to-face counseling have been provided this past year.

Fact: To date, more than 1,000 individuals were admitted to our Transitional Care Program. 10% of them were assessed to be at high risk and referred to our care manager.

Research and Education

The original version of the above website, MyPainManagement.net, was developed and tested as the Self Care Pain Management Research Project of the Leonard Schanfield Research Institute.

18. CJE is one of only a handful of social service agencies in the nation that supports an applied research division. The Leonard Schanfield Research Institute (LSRI) is well-regarded for its investigation, development and recommendation of practices and procedures that have later been adopted for use by CJE program managers. CJE’s MyPainManagement.net website and the Parkinson’s training program were initiated at CJE by the LSRI. Led by Dr. Madelyn Iris and a team of special investigators, significant progress has recently been made in the development of a system that standardizes the way elder abuse is reported. The Elder Abuse Decision Support System (EADSS) is the first web-based decision model designed specifically for use in the field of elder abuse.

19. We are a destination of choice for social work and nursing students. Interns are mentored by and work closely with CJE staff members. These interns gain important skills and the valuable experience of working in real life situations.

20. CJE provides dozens of special presentations for professionals and community members to advance knowledge about healthy aging issues. Transition to Wellness, an annual educational series held at Weinberg Community, features speakers on topics such as the impact of Alzheimer’s disease on family members, advanced directives, sub-acute rehab and financial planning. Social workers and nurses receive continuing education credits for attending these CJE-sponsored programs.

Fact: This year alone, CJE has provided a total of 11 internships that fulfill social work and nursing students’ requirement for the completion of practicum’s in the community. In addition, four Lewis Summer Interns from JUF, who are undergraduates interested in Jewish Communal Service, come to CJE to attain some real work experience in the Jewish community.

Fact: In the past year, more than 12 continuing education programs have been presented by CJE staff, many in partnership with other professional organizations, and a total of 44 social workers received CEUs for their participation.

Social Accountability Our employees have volunteered for at least 60 outside organizations and participate in fundraising drives for nonprofit organizations like Alzheimer’s Association and American Lung Association. This year, over $2,700 has been distributed to employees in crisis through the Employee Emergency Fund, which is funded by donations from employees.
