A. BASIS OF INITIATION OF TOS Amendment ＃可
Amendment 剝削II provide for wo『k identified under Stage 2 of the remediation se『vices under TOS SN妞， in support of a settlement between the City of Los Angeles (City} and Los Angeles Unified School District {lAUSD). This remediation work falls under the oversight of the Regional Water Control Board {RWQCB) and requi阻s their accep抽nee and approval. As a 時suit, it is difficult to specify a project end date.
B. SCOPE OF WORK
Brown and Caldwell shall provide technical services for the site remediation at 2110 N. San Fernando Road as described in Brown and Caldwell’s proposal submitted on August 12, 2015. The scope of work for Stage 2 includes b叫 is not limited to:
1. Continue to o悍rate the Soil Vapor Extraction (SVE) Sy剖開• Provide a qualified expe的 familia「 with the geographical area, the m司or contaminants
on site and SVE concepts and operation • Perform all operations and maintenance on the SV E system • C。nduct quarterly sampling, analyzation, and reporting
2. Install one additional Groundwater Monitoring Well at either 45 ft below ground su巾ce（旬s）。r65 ft bgs
3. Semi-Annual Groundwater Sampling and Collection • Prepare 間po前s and upload to Geotracker for the RWQCB app『oval
4. Prepare and submit Final Closure Report 5. Prepare and submit Final Human Health Risk Assessment (HHRA) 6. Participate in final meeting with the RWQCB to discuss soil cl。sure
C‘ SCHEDULE FOR COMPLETION OF WORK
The completion date of this p叫ect is dependent on any follow-up requirements/requests by the RWQCB to the stated testing/reporting.

E. COST REIMBURSEMENT
Invoice and Cost Reimbursement:
The work to be performed under this Task O吋er Amendment 削 is on a lump sum basis not to exceed $124,200. The to個I amended budget shall not exceed $272,200. All of the Consultant’s costs including Consultant’s employee sala吋師， overhead, other di時ct cos趟， subcont悶ct expenses, and prof社 a陪 included in the Lump Sum Amount (Attachment #3).
The Consul個nt wm submit invoices upon completion of the following milestones as prescribed in the expenditure t.able at the end of this document (Attachment 位）．
