I hope this will give you the information needed: The application form is part of the Database. I have included a sample of what goes on the application form. The dash lines should be boxes where applicants can check each one. There should be links included that take them to that subject/information. The tax payment section will take them where they can enter their bank information. All of this information will be stored in the Database. A seven (7) digit number will be assigned to each application. They must agree to the oath, and then a message will appear. (See message)
