I. FILING OF CRIMINAL COMPLAINTS FOR PRELIMINARY INVESTIGATION AND INQUEST

Police, NBI, and other law enforcement agencies, offended party

Complaint Affidavit, Affidavit of Witness, Police Blotter if any, and any other Relevant document necessary for the crime complained of such and Medical Certificate if the crime complained of is Physical Injuries. 7 copies each. Each additional respondent requires an additional set of copy. Every Additional crime complained of requires an additional set of 3 copies.

1. Present for assessment the complaint affidavit and all the supporting documents (2 minutes)

2. Fill out the Investigation Data From and the OCP-Naga City Records Checklist, Route and Case Status if the complaint documents are complete (5 Minutes)

3. Proceed to Prosecutor on Duty for Administration of Oath on the affidavits. The affidavits are explained to the Client (5-30 minutes)

II. ISSUANCE OF PROSECUTOR’S CLEARANCE

III. ADMINISTRATION OF OATH
