    is form is designed to assist you in gathering information, completing the Supplement to Registrant Bar Application,d submitting the appropriate forms and information required to convert your student registration or CLI registration inrsuit of admission to The Florida Bar. Click the various links to access information or printer-friendly forms. Applement to Registrant Bar Application submitted with accurate and complete item responses and all requiredppleme ntal documents can be proce ssed more expeditiously. Processing will not be initiated on defective applicationsd an additional defective fee may be charged. You must also submit the Re-Examination/Conversion Application toke the General Bar Examination in order to indicate which examination you intend to take. Timely filing of thisplication is necessary to avoid late filing fees.

    EP 1 - PRINT AND USE THIS CHECKLIST

    ___1. Print this Checklist.___2. Check each item when completed to ensure that your submission includes all required supplemental documents

    and information.

    EP 2 - GATHER INFORMATIONAL MATERIALS

    s suggested that you gather this information and have it available when you complete the Supplement to Florida Bar plication.

    ___1. A copy of the Registrant Bar Application and all amendments previously filed by you. If you did not retain acopy, you may request a copy by writing to the address below and submitting the $50.00 copying fee. However you do not submit a copy of your application with your supplement.

    ___2. Read and review each item of the Registrant Application and amendments you previously filed as a student toassure that each response is correct and current. Update any item that has changed since your initial filing thathas not been reported in an amendment.

    ___3. Five personal references who have known you well within the past 5 years, including names and currentmailing addresses.

    ___4. Information to update your residence addresses.___5. Information to update your employment, including employers' names, addresses, dates employed, position,

    type of business, supervisor, and reason for leaving. Include self-employment or association with any

    occupation, business, enterprise, or profession, either part-time or full-time; employment as a law clerk;positions in clinics, internships, externships, or other similar non-paid positions.___6. Information regarding any delinquent credit, defaulted credit, unfiled tax returns, and all student loans.___7. Information to update all arrests, charges or accusations (including traffic violations) reporting dates, law

    enforcement agency, explanation of event and final disposition. If your arrest records are sealed, you mustpetition the appropriate court to unseal those records.

    ___8. Information about all bar admission or registration applications filed in any other jurisdiction.___9. Fingerprints (unless previously submitted as a Florida Bar registrant or CLI registrant)

    All applicants for admission to The Florida Bar must have their fingerprints scanned for electronic submissionto the Florida Department of Law Enforcement and to the FBI by MorphoTrust USA. Instructions for this stepare in the answer to How do I submit the required fingerprints? in our FAQ. Confirmation of your compliance

    with this step must be received by the board before processing of your application can begin. If your fingerprints have already been taken by MorphoTrust USA for the purpose of applying to The Florida Bar, youare not required to be re-fingerprinted.

    ___10. Retrieve or obtain your NCBE number.You will be required to report your NCBE number on the Examination Application under Step 4 below. Go towww.ncbex.org/ncbe-number to either retrieve your NCBE number or request an NCBE Number. Your uniqueNCBE Number will be used for identification purposes when you take the Multistate Bar Examination and theMultistate Professional Responsibility Examination and may be used as an identifier for other bar-admission-related purposes. It is recommended that you copy and paste your number into the "NCBE Number" field onyour Examination Application. Therefore, you should have that window open when you are completing theapplication.

    EP 3 - COMPLETE THE SUPPLEMENT TO FLORIDA BAR APPLICATION

Proof of citizenshipIf you are a citizen of the United States, submit with your Bar Application a certified copy of your birth certificate, or provide a photocopy of your certificate of naturalization, or certificate of citizenship for submission to the United States Citizenship and Immigration Services for verificationof authenticity.If you are not a citizen of the United States, provide a photocopy of the immigration document thatdocuments your status for submission to the United States Citizenship and Immigration Services for verification of authenticity.

    ___8. Copies of letters of request for law school and post law school transcripts___9. Certificate of Dean (PDF - 171k)

    Complete the top section and submit the Certificate of Dean to your law school dean after your graduation. If attending a Florida law school, do not provide the form to your school.
