Please read this checklist carefully to ensure that your application can be processed as quickly as possible. Please do not submit your application if any of the documents listed below are missing. If you do, we may return the application to you without processing it. Please print this Document Checklist, and attach to the front of your application form. Please do not use folders or plastic sleeves when submitting your application and documents. We do not return the original copies of translations of foreign language documents. We recommend you keep copies. When you submit original documents you must provide plain copies behind the originals. Tick each section of the checklist to show your application is complete. Can I submit just plain copies of my documents? No. Original documents or certified copies are acceptable only. Certified copies must be stamped or endorsed as being true copies of the originals by a person authorised by law to take statutory declarations in the applicant's country or in New Zealand. Examples: a lawyer, notary public, Justice of the Peace, or court official. Residents of Iran: Certified copies of qualifications must be stamped by international office of universities. All other certified documents must be stamped by judiciary offices or the Ministry of Foreign Affairs Registration and Personal Status Department. We do not accept copies certified by the South African Police. Please note that we may request the original documents. Please ensure that you complete the relevant tick box above the declaration on the declaration page. Full birth certificates Please provide original full birth certificates or certified copies for everyone included in the application. Birth certificates must show the names of both parents and the place and date of birth. Passports Original passports, or certified copies of passports, for everyone included in the application. If passports are required for urgent travel please send in certified copies. Please note that we can only issue a resident visa into a passport which is valid for a further 12 months. Relationship evidence (Married) You must provide evidence that you have been living together for 12 months or more in a partnership that is genuine and stable. You must provide evidence of living together at the same address as well as evidence that your relationship is genuine and stable. A marriage certificate is evidence of the nature of your relationship, but is not considered evidence of living together. You should seek to provide a variety of types of evidence with the application and you should focus on providing evidence that is produced officially (e.g. bills, bank statements etc). For examples of documentation that you should submit please see the Application for Residence Guide. Offer of employment If you obtain an offer of employment prior to lodging your application please inform us as soon as possible and then enclose your full employment agreement. Your employment agreement must be signed by you and the employer and will need to demonstrate that your employment meets the minimum statutory requirements, e.g. holidays, occupational health and safety and minimum wage Qualifications You must supply your original qualifications or certified copies. Please also include full transcripts. If you have claimed points for a postgraduate qualification you must provide the original undergraduate qualification or a certified copy. NZQA International Qualification Assessment You are required to provide a full International Qualification Assessment (IQA) from the New Zealand Qualifications Authority (NZQA) to be eligible for the qualification points you have claimed. If you have a Preliminary Assessment Report (PAR) please note this will not be sufficient at application stage. Visit www.nzqa.govt.nz for more details. Work experience evidence This evidence must confirm the work experience you claimed in your EOI. References must be on company letterhead and show the start and finish dates of your employment and what role you held with that company and average weekly hours worked (especially if part-time). - Contracts (including full job descriptions) - Payslips - Salary increment notifications - Tax documents - Training undertaken as part of your work (such as industry certifications)

If you have been self-employed you must provide evidence from your accountant showing the number of years of your self-employment, together with financial statements to cover the years of your claim. You must also provide evidence of the nature of your self-employment such as contracts, invoices, or evidence from clients.

Visa evidence If you are claiming points for work experience or qualifications gained in a country where you do not hold citizenship, please provide evidence to demonstrate your visa status in that country.

Partner’s qualification See the requirements listed above.

Partner’s NZQA International Qualification Assessment Your partner is required to provide a full International Qualification Assessment (IQA) from the New Zealand Qualifications Authority (NZQA) to be eligible for the qualification points you have claimed. If he/she has a Preliminary Assessment Report (PAR) please note this will not be sufficient at application stage. Visit www.nzqa.govt.nz for more details.

English language proficiency You must provide evidence for all applicants aged 16 years and over. Please see the Application for Residence Guide.

Partner’s English language proficiency As you have claimed points for your partner’s qualification, he/she must meet the English language requirements for a principal applicant; equivalent to IELTS 6.5 or higher. If you do not provide IELTS, It is best to provide recent evidence (no more than 2 years old at the time the application is lodged).

INZ 1007 Medical and X-Ray Certificates for all those included in the application. The Medical and X-Ray Certificates should be no older than 3 months old at the time that you submit your application. The Radiologist’s report must be attached to the x-ray certificate. Please refer to www.immigration.govt.nz/paneldoctors for a list of panel doctors. You must have the Medical Certificate completed by one of the panel doctors listed. Please note that there are no panel doctors in the USA and those applicants from the USA must have their Medical Certificate completed by a registered doctor to whom you are not related. Please remember to enclose copies of your laboratory results. Children under the age of 11 do not require X-Ray Certificates.

Police certificates for all applicants aged 17 and over. You must provide original Police Certificates from all countries of citizenship and from any country where you have lived for 12 months or more in the last 10 years. This includes any country where you have been for 12 months in total including short, non-consecutive visits. These certificates must be no more than 6 months old at the time that you submit your application. For information on how to apply for a police certificate please visit: www.immigration.govt.nz/migrant/general/formsandfees/formsandguides/policecertificate.html If you have ever been known by another name (such as a maiden name), you will need to demonstrate that you have requested the certificates in every name you have been known by. On most occasions we require national police certificates rather than provincial or state police certificates. Applicants who require a UK police certificate should contact ACRO. Applicants who require a police certificate from the USA must supply an FBI police certificate.

Please note that your Expression of Interest has undergone a preliminary credibility check only. A thorough assessment of your eligibility for points will be undertaken at application stage. However, it has been noted that you have claimed the following points for which you will need to provide additional documents as outlined below to establish your eligibility for these points.

New Zealand Qualifications Authority Assessment

You are required to submit a full NZQA International Qualification Assessment (“IQA”) of your qualification when you submit your application in order to be eligible for qualification and / or bonus points in an area of absolute skills shortage. Your specific NZQA requirements are: NZQA Level: Bachelors with Honours level 8 or Bachelors level 7 Comparable qualification: A qualification compared to one of the qualifications listed under the Long Term Skill Shortage List, as below.

You are required to show evidence that you meet the requirements as outlined on the Long Term Skill Shortage List (“LTSSL”) when you submit your application.
