Dear Exporter:
The following represents the process by which the San Francisco Chamber of Commerce Issues Certificates of Origin.
The Chamber must rely on your statement that the goods you will be shipping constitute domestic exports of the United States. Therefore, we ask you to sign the Affidavit of Origin statement printed on the San Francisco Chamber's Certificate of Origin form (enclosure "A"). By signing the Affidavit or Origin, you state that the goods exported are domestic exports as defined by the U.S. Department of Commerce. This must be signed with each shipment.
The San Francisco Chamber of Commerce Certificate of Origin form, containing the required affidavit, is available at the Chamber and may be xeroxed at your convenience for future use. In the event a particular country, or your company, require a specific Certificate of Origin form be used, the separate Affidavit of Origin (enclosure "B") must be signed and accompany each shipment. These may also be xeroxed at your convenience.
In addition, because the Chamber is relying on your statement, we ask you to sign a copy of the Certificate of Origin Indemnification Agreement (enclosure "C"). By signing the Indemnification Statement, you agree to indemnify the Chamber of any liabilities incurred by the Chamber as a result of its issuance of the Certificate of Origin. These must be filed with the Chamber annually, before any certificate will be issued. Both of the documents are legal agreements. If you have any questions, consult with your lawyer.
Attached is the schedule (enclosure "D") which states the hours someone will be on duty to process Certificates of Origin.
The cost for processing each Certificate of Origin is $15.00 for members and $40.00 for non-members. There will also be a filing fee of $5.00 for the annual Indemnification Statement. For our member's convenience, coupon books are available for purchase at the price of $150.00 per book of ten coupons.

Affidavit of Origin
(To be signed if any other than the San Francisco Chamber of Commerce's Ce11ificate of Origin form is used)
• The undersigned exporter certifies to the San Francisco Chamber of Commerce that allgoods for which a certificate of origin is requested (which goods are to be shipped by
exporter to one or more customers in one or more shipments) constitute "domestic
exports" of the United States of America, as determined in compliance with the definition
of "D" (Domestic) exports promulgated by the U.S. Department of Commerce, Bureau ofCensus, for the purpose of the Shippers Export Declarations and in accordance with otherapplicable federal, state, or foreign law.
• The San Francisco Chamber of Commerce (the Chamber) states that, based solely on theaffidavit of the exporter, and without any independent verification by the Chamber, the
Chamber believes that the goods described above are products of the United States ofAmerica. The Chamber makes no warranty, expressed or implied, concerning the goods,
or their origin, affidavit or any other documents relating to the goods, and assumes noresponsibility for the truth and accuracy of any statements contained in the affidavit or anydocuments relating to the goods.

PROCESSING HOURS
Certificates are processed Monday through Friday, during
morning hours of from 9:00 a.m. to 10:30 a.m., and during
afternoon hours of :from 2:00 p.m. to 3:30 p.m.

FOR PROCESSING
CERTIFICATES OF ORIGIN
1. Check to see that the Certificate of Origin is filled out correctly and completely;
all names and addresses are spelled out in full.
2. The Affidavit of Origin statements on the Certificate of Origin must be the
following authorized statements:
"The undersigned exporter certifies to the San Francisco Chamber of Commerce
that all goods for which a certificate of origin is requested (which goods are to be
shipped by exporter to one or more customers and in one or more shipments)
constitute "domestic exports" of the United States of America, as determined in
compliance with the definition of "D" (Domestic" exports promulgated by the
US Department of Commerce, Bureau of Census, for the purpose of the Shippers
Export Declarations and in accordance with any other applicable federal, state or
foreign law. "
"The San Francisco Chamber of Commerce (the Chamber) states that, based
solely on the affidavit of the exporter and without any independent verification by
the Chamber, the Chamber believes that the goods described above are products
of the United States of America. The Chamber makes no warranty, expressed or
implied, concerning the goods or their origin, affidavit, or any documents relating

3. The Affidavit of Origin section of the Certificate of Origin must be signed either by the
exporter or the freight forwarder acting as agent for the exporter.
4. A fee of $15.00 for the Chamber members or $40.00 for non-members must accompany
each Certificate of Origin
5. Exporters and freight forwarders acting as agents on behalf of exporters must sign an
indemnity agreement. The original is to be filed annually with the San Francisco
Chamber of Commerce before any certificates are issued ($5. 00 filing fee).
