0 FOREWORD

This procedure is established in accordance with the requirements of the identical standards, European prEN 4179: P5 2014 and American NAS 410 Revision 4 standards relating to the certification on non-destructive testing agents operating for the aeronautic and aerospace industry.

On the behalf of:

- National Aerospace NDT Board (NANDTB), independent from every and all employer as defined in prEN 4179:2014 P5 and NAS 410 Revision 4 standards.

- Aerospace Sectorial committees of French Confederation Nondestructive testing (COFREND), COSAC works in accordance with this document in France, which is therefore applicable for nondestructive

testing agents who belong to foreign companies bound by specific contracts to constructors, repairers and users of aerospace material.

COSAC's organization and operations are described in the CER/COSAC-PR-002 procedure.

Limited level 1 certification is described in the CER/COSAC-PR-013 procedure.

Hereafter, unless otherwise mentioned, in this procedure the word "certification" is used according to its meaning as mentioned in EN 4179 (generic term for the last EN version).

The qualification and certification process described hereunder includes the following steps:

1 GENERAL

1.1 Purpose

This procedure describes the way COSAC applies the requirements of the EN 4179 standard for qualifying and certifying personnel involved in nondestructive testing (NDT) assessment operations in aerospace industries which fabricate new material, supply services, maintenance and repairs. The qualification and certification process includes the following steps: - qualification by COSAC - certification by employer

1.2 Applicability

This procedure applies to personnel using NDT methods to test and/or accept materials, products, components, assemblies or sub-assemblies This standard also applies to personnel directly responsible for the technical adequacy of the NDT methods used, to NDT employees, to personnel who write NDT procedures and/or work instructions. Individuals who are certified according to the previous revisions of the EN 4179 (procedure CCAPP 94001) don't need to be recertified regarding the requirements of this procedure until their current certification expires.

1.3 Nondestructive testing methods

1.3.1 Common methods

This procedure contains detailed requirements for training, experience and examination applicable to the following NDT methods:

1.3.2 Other methods

When needed, COSAC may implement an emerging NDT method not provided hereunder. In these cases, COSAC has to:

- Consequently change its procedures in order to include the new testing method. - Consult the general examinations data base questions for Level 1, 2, and 3 validated by COFREND. - Create and approve the data base questions specific to Level 1, 2, and 3.

- Create and approve practical examinations texts and corresponding keys. - Set up key tables. - Write the training programs. - Approve the examination centre(s) for this new method.

- Appoint the level 3 personnel responsible for the implementation of the new qualifications. Their number shall be limited to the minimum required.

These individuals shall comply with the requirements stated in § 5.4, in particular as regards their general knowledge and industrial experience in the method and enforcement area.

3 DEFINITIONS

3.1 Examination centre: COSAC-approved centre where the qualification examinations take place. 3.2 Training centre: entity who offers training services preliminary to the NDT certification.

3.3 Certification: document validated by the employer, stating that the agent complies with the requirements

of EN 4179.

3.4 Documented: in written or electronic form.

3.5 Activity field: field in which the personnel involved in the qualification or certification works, selected in the following list:

- Production (foundry, metallurgy, etc.) - Processing (forge, rolling, etc.) - Fabrication (machining, forming, assembly, welding, heat treatment, surface treatment, surface

treatment, coatings, composite, etc.) - Maintenance

3.6 Testing sample: part or data to be assessed, containing one or more discontinuities, flaws (natural or artificial) or known and documented set up, used during the practical examination in order to demonstrate the candidate's ability to use a particular method. The testing samples can be actual parts, parts fabricated for the purpose of the examination or, when necessary, control data of the parts, like radiography scans or ultrasonic maps.

3.7 Written: stored in an electronic or paper format. 3.8 Employer: entity which employs (or contracts the services of) one or more individual(s) who carry/carries

out NDT operations. Individuals who carry out NDT operations and are their own employer are included in this definition.

3.9 Assessment: analyse performed after interpreting the instructions observed during an NDT, in order to

define if they comply with the stated acceptance criteria or to inform about the product quality. 3.10 Examination: formal, controlled, and documented test performed in accordance with this procedure, in

order to verify the candidate's visual ability or non-ability in the applicable NDT method. 3.11 "Closed books" examination: an examination administered without access to any reference materials. 3.11 "Open books" examination: an examination administered with access to specific reference material

provided during the examination. 3.13 General examination: a written examination addressing the basic principles and theory of an applicable

NDT method. 3.14 Practical examination: an examination to demonstrate an individual's ability to perform an NDT method as

used in the employer's premises. Questions and answers are not necessarily written, but a checklist shall be used. Observations and results shall be documented.

3.15 Specific examination: a written examination to determine an individual's understanding of operating

procedures, codes, standards, product technology, test techniques, equipment and specifications for a given method as used in the aerospace industry.

3.16 Examiner: Level 3 COSAC-certified agent with inspection option or Level 2-certified in the particular

technique for which candidates attempts the examination. 3.17 Experience: actual performance of the NDT method tasks, performed in the work environment and

resulting in the acquisition of knowledge and skills. This does not include formal training, but may include on-the-job training (see § 6.2).

3.18 Instructor: an individual who provides training for NDT personnel. 3.19 On-the-job training: training in the work environment to gain experience in instrument set up, equipment

operation, testing method application, and recognition, interpretation and assessment of instructions under appropriate technical guidance.

3.20 Formal training: an organized and documented program of learning activities designed to impart the

knowledge and skills necessary to be qualified to this standard. Formal training may be a mix of classroom and practical activities in a training centre. Self-instruction is not included.

3.21 Instruction: apparent response of a condition resulting from an NDT inspection that requires interpretation.

3.22 Work instruction: document containing a detailed description of the NDT technique, as well as the

parameters used to test the specific part or group of parts (for example: extruded aluminium part) or assembly. The work instructions are written in compliance with the procedures (§ 3.27). In this document, work instructions can be referred to as Data sheets.

3.23 Interpretation: operation consisting in determining whether instructions are relevant or not Relevant

instructions shall be assessed. 3.24 Method: one of the disciplines of nondestructive testing (e.g. radiography) which could include various

techniques (§ 3.31). 3.25 Responsible Level 3: Level 3 EN 4179 appointed by the employer who has the responsibility and authority

to act on its behalf and to warrant that the requirements of EN 4179 and NAS 410 standards are complied with as regards the employer.

3.26 Direct observation: direct observation is when the observer is able to immediately assist the trainee and

keeps within a distance that permits uninterrupted, unaided visual and verbal contact with the trainee. 3.27 Procedure: a written general “how to” instruction for performing a given process. Procedures are used

when writing the work instructions (§ 3.22). 3.28 Internal application procedure: employer's procedure describing the qualification and certification

management rules according to EN 4179 for the NDT personnel (see § 4.1). 3.29 Qualification: skills, training, knowledge, examinations, experience and visual capability required for

personnel to properly perform the functions of a particular level. 3.30 Subcontractor: as regards the principals, entity responsible for the aerospace products fabrication or

maintenance. In the context hereunder, this includes NDT suppliers and providers. 3.31 Techniques: category of a method.

3.32 Special processes: special processes defined in the table below require particular prerequisites, along with a complementary training under the responsibility of the employer and of the Responsible Level 3.

3.33 National Aerospace NDT Board (NANDTB): an independent aerospace organization representing a nation's aerospace industry that is chartered by the participating prime contractors and recognized by the nation’s regulatory agencies to provide or support NDT qualification and/or examination services in accordance with EN 4179.

4 GENERAL REQUIREMENTS

4.1 Internal application procedure

Employer shall write and update an Written Practice procedure for qualification and certification of its NDT personnel according to EN 4179. The applicable procedure must be sent to COSAC, together with a letter appointing the Responsible Level 3. The Written Practice procedure must comply with requirements of EN 4179. For qualifications in accordance to this procedure, the letter must state:

the methods, techniques and levels for which the employer assigns the qualification to COSAC. the contributors' functions and responsibilities. the training and experience conditions. the conditions of the vision control. the field of activities.

Employer must sign and affix the company's stamp on the ethic commitment, according to the model in the Annex 3 hereunder.

4.2 Conformity and responsibilities

The employer is the only responsible of the certification according to EN 4179. COSAC, in quality of NANDT BOARD, defines a document called "certification document" which must be validated by the employer in order to deliver the certification.

5 QUALIFICATION AND CERTIFICATION LEVELS

As a reminder, skills and responsibilities of the trainees and Level 1, 2, and 3 individuals as defined in EN 4179, are the following:

5.1 Trainee

An individual who participates to a training program for an NDT method and is in the process of becoming qualified for certification to Level 1 or directly to Level 2 (without first obtaining the Level 1). The trainees shall: be documented as trainees and be actively participating in a training program for a stated NDT method

for a limited and specified period of time. obtain experience under the direct observation of a Level 2 or Level 3 individual in the same method or an

instructor only when approved by the Responsible Level 3. not make accept or reject decisions. not execute test or assessment tasks of products which include or imply the acceptance or rejection

without direct observation. not perform any other NDT function without direct observation. be subject to the vision controls as stated in § 7.1.1.

5.2 Level 1

In the test method in which he/she is certified, Level 1 agent shall:

be able to follow work instructions. have the skills and knowledge to process parts, document results and perform equipment standardization

in accordance with approved work instructions. have the skills and knowledge to carry out any necessary preparation of parts before or after inspection in

accordance with approved work instructions. have the skills and knowledge to conduct system performance checks in accordance with the applicable

process standard. receive guidance or supervision from a certified Level 2 or Level 3 individual in the method when

necessary. have the skills to perform interpretations and evaluations of specific product or product type for

acceptance or rejection in accordance with approved work instructions.

5.3 Level 2

In the method in which he/she is certified, a Level 2 agent shall:

be thoroughly familiar with the scope and limitations of the technique/method in which he/she is certified. be familiar with the codes, standards, and other contractual documents governing the method used by the

employer. have the skills and knowledge to set up equipment, process parts, interpret and assess acceptance or

rejection, and to document the results. have the skills and knowledge to conduct system performance checks in accordance with the applicable

process standard. be capable of providing the necessary guidance and/or supervision to trainees and Level 1 agents. have a basic knowledge of relevant product manufacturing and inspection technology. be capable of developing work instructions from an approved general procedure. Such work instructions

shall require final approval by a Level 3 COSAC-certified individual in the method. when necessary and specified in the internal application procedure, have a basic knowledge of aircraft or

vehicle maintenance

5.4 Level 3

In the test method in which he/she is certified, Level 3-agent shall:

have the skills and knowledge to interpret codes, standards, and other contractual documents that control the NDT method used by the employer.

be capable of taking over the technical responsibility for the NDT facility and personnel. be capable of selecting the method and technique for a specific inspection. be capable of preparing and verifying the adequacy of procedures and work instructions. approve NDT procedures and work instructions for technical adequacy. have a general knowledge of other NDT method(s) and product technologies used by the employer and

of the associated flaws. when necessary and specified in the internal application procedure, have a basic knowledge of aircraft or

vehicle maintenance be able to inform the projects office about the limits of the methods and techniques, the geometry of the

parts which are better adapted to an efficient industrial assessment. be capable of providing or directing training, examinations (according to the requirements described in the

following provision), and certification of personnel for the method in which he/she is certified. conduct NDT for the acceptance of parts, only if the proficiency in this ability is proven (§ 7.1.4.3 and §

8.6b). conduct Level 1 and 2 examinations in accordace with this procedure, only if he/she holds a Level 2

certification or the inspection option. When required by the internal application procedure, be capable of auditing outside agencies to ensure

the requirements of the internal application procedure are met.

6 TRAINING AND EXPERIENCE

6.1 Training

6.1.1 General requirements.

As a reminder, general requirements, as defined in EN 4179, are the following:

Candidates for certification to Level 1, Level 2 or Level 3 shall complete sufficient formal training to become proficient with the principles and practices of the applicable test method and techniques and be capable of carrying out the duties specified in Section 5. Formal training shall be conducted prior to, or in conjunction with, on-the-job training. All completed NDT training shall be documented.

General (§ 3.20), specific and practical training shall always be supplemented by a documented practical on-the-job training with the employer (§ 3.19).

The minimum training hours for Level 1 and Level 2 agents are provided in Table II and Table IIA for the specified NDT methods and techniques. If a further method is added, Table II shall be supplemented by specifying the quantity of hours on the basis of the requirements for a method with a similar complexity.

6.1.2 Training outlines

Formal training shall cover the minimum requirements of the COFREND and COSAC trainings, which are available on their Websites. As a reminder, in accordance with EN 4179, the training shall cover at least: - basic theory, - test principles, choice of methods, assessment settings and relevance to different materials and part and test variables, - products, materials and defect (formation and characterization), - equipment standardization and operation, - the importance of process controls, - the importance of appropriate processing steps and parameters, - health and safety aspects, - applicable techniques and the advantages and disadvantages of each, - capabilities and limitations of each method and technique, - operating procedures, standards and general specifications, - interpretation and evaluation of the instructions and documentation of inspection results.

The training outlines shall include a list of the references on which the training material is based

6.1.3 Reorientation training

A reorientation training shall be provided to personnel non EN 4179-credited within 12 months of their training. This previous training shall have been determined to be acceptable by the Responsible Level 3. As a minimum, reorientation training shall cover products, equipment set up and operation, specific operating procedures, applicable techniques, interpretation and evaluation of NDT results, safety, applicable standards and specifications. The development level of each individual shall be assessed by the Responsible Level 3.

The reorientation hours shall be ≥ 25% of the minimum requirement indicated in Table II.

6.1.4 COSAC and employer responsibilities

COFREND and COSAC define the minimum training outlines. If an employer reckons that the provided training (other than training to the workplace) is not sufficient for his particular needs, he/she shall complete or make it completed.

6.1.4.1 COSAC-approved training centre

Regarding the CER/COSAC-PR-014 procedure, COSAC ascertains compliance to requirements of the EN 4179. The training centre shall provide COSAC with a nominative list of the contributors, their qualification and, as the case may be, their certifications. This list shall be available for the employers. Upon request, employers can transmit this list to their principals, their auditors and to the regulating authorities.

6.1.4.2 Other cases

If the employer performs a training in a training centre which was not ascertained by COSAC, the Responsible Level 3 shall: - check it complies with EN 4179 requirements - check it complies with training outlines as defined in § 6.1.2.

6.2 Professional experience

As a reminder, pursuant to EN 4179, candidates for certification to Level 1, Level 2 or Level 3 shall have sufficient practical experience to assure that they are capable of performing the duties of the level for which certification is sought. The minimum experience requirements for Level 1 and Level 2 are provided in Table II, those of Level 3 in Table IV. If a further method is added, Table III shall be supplemented by specifying the quantity of hours on the basis of the requirements for a method with a similar complexity. The training specific to their work place can only be considered as experience if the following conditions are present: - training was performed by certified personnel pursuant to EN 4179. - internal application procedure accepts this practice.

6.2.1 Previous experience

As a reminder, according to EN 4179, when a candidate gained his/her experience with a previous employer, this experience must be documented and approved by the Responsible Level 3.

6.2.2 Equivalent experience

As a further reminder, according to EN 4179, personal who is qualified through other programs shall document this previous experience, and the latter shall be overseen by the Responsible Level 3 according to requirements indicated in Table III or Table IV.

7 EXAMINATIONS

The appliance documents shall be prepared by the candidate according to CER/COSAC-PR-004 procedure and revised by the employer. The candidature shall be transmitted to COSAC within the prescribed terms. The following elements must be enclosed:

- employer's internal application procedure, or reference to it if already supplied to COSAC, - vision certification obtained not more than one year before the examination, - ethic commitment.

COSAC approves the candidature upon examination of the application documents.

French is the official language, but foreign trainees can perform the examination in English. The use of another language is submitted to COSAC's approval.

7.1 Object and contents

Examinations to verify the technical qualifications of candidates shall consist of a general, specific and practical examination for each method in which the candidate is to be certified. Oral or written translations of the written exams are forbidden. Examination for visual acuity shall also be conducted, in accordance with Table V, prior to the candidate’s first certification and periodically thereafter Additional requirements can be added during the writing of the corporate society internal application procedure, in particular to comply with the applicable laws (for example as regards ionizing radiation). Candidates to the X-ray method shall perform a previous safety radiation training. The general exam questions are made available by COFREND. The specific exam questions and the checklist used for the practical examination are made available by COSAC, in order to be revised by the employer or its clients upon request. The examination texts and samples will be available to candidate only during the examination.

7.1.1 Vision

The vision examination for trainee, Level 1, Level 2, and Level 3 personnel shall assure that the applicant’s near vision and colour perception meet the requirements of Table V. Near vision tests shall be administered annually and colour perception tests shall be administered at least every 5 years and before the candidate starts his/her trainee activity. The examination shall be administered by trained personnel appointed by the Responsible Level 3 or by qualified medical personnel. Any limitations in colour perception shall be evaluated by the Responsible Level 3 prior to certification and to the beginning of any trainee's activity. The results must be approved in writing.

Employer is responsible for the organisation of this examination for the intermediary years, between certification and recertification. The most recent certification must be included in the employer's certification file.

If the result of the vision examination performed by a certified agent changes, employer must update the certification document(s). When vision correction is necessary to pass the visual acuity exam, vision correction shall be worn during all testing/inspections.

7.1.2 General

The general examination for all levels shall be a written closed book examination, without programmable calculator, consisting in a multiple choice test questions covering the cross-section of the applicable method at the appropriate level. For Level 3, it also includes questions on other two methods.

Candidates who have a valid ISO 9712 Level 3 certification, delivered by COFREND, can use the scores they obtained in the general examination Ng 3, Ng 2.1 and Ng 2.2. Level 3 COSAC certified agents can use the scores they obtained in Nmp, Ng 3, Ng 2.1 and Ng 2.2.

7.1.3 Specific

The specific examination for all levels shall be an open book examination covering the requirements and use of the specifications, codes, equipment and operating procedures. It covers all the techniques of the appropriate method, as defined in § 3.31, and all the activity domains of § 3.5 compatible with the method.

30 multiple choice test questions, corresponding to the required qualification level and written by COSAC, will be used during the specific examination (specialised) of Level 1, 2 and 3, except for Tightness examination (LT), which includes 40 questions. 2 minutes per question will be given to the candidate. Reference material, such as specifications, tables, formulas, shall be provided if necessary.

7.1.4 Practical

The practical examination shall consist of a demonstration of proficiency in performing tasks that are typical of those to be accomplished in the performance of the candidate’s duties (activity field and qualification level). The examination covers all the techniques of the appropriate method, except when employer explicitly requires that one or more techniques shall be avoided. When a technique is not included during the first examination, it can be asked for in the future, pursuant to § 7.2.4. Checklists covering the topics detailed in the following sub-paragraphs shall be developed and completed by the examination centre examiner to assure adequate coverage and to assist in the administration and grading of the examination. The examination centre shall also determine and document how the examination results obtained by the candidate are to be documented (e.g. part maps, drawings, sketches, written descriptions, etc.).

7.1.4.1 Level 1

The candidate shall demonstrate proficiency in using a written work instruction: check that the equipment and its accessories work, assess at least one sample with each technique for which certification is sought. The candidate shall process, check, interpret and assess at least two test samples for the technique for which certification is sought. The test samples shall meet the definition of paragraph 3.6 and have different configurations. The position and the severity of the discontinuities shall not be apparent for the candidate. Candidate shall document the results. One test sample at least shall be representative of the products with which he/she mainly works. The checklist used in the approved examination centre for scoring shall evaluate competencies in: - use and setting of the equipment and its accessories, - strict compliance to the written instruction, - interpretation and evaluation of the instructions, - documentation of the results. Duration of the practical examination will be defined by COSAC.

7.1.4.2 Level 2

The candidate shall demonstrate proficiency by inspecting at least 2 test samples of differing configurations for each method, with at least one test sample for each technique for which certification is sought. When only one material configuration is to be inspected upon certification, both test samples may be of the same configuration. The test samples shall meet the definition in § 3.6 and shall be representative of the products encountered in his/her main activity. Specimens without discontinuities may be included in more than two test samples during the examination. The candidate shall document the NDT results in accordance with the applicable acceptance criteria. The checklist shall include proficiency in the use and standardization of equipment and materials, adherence to procedural details, accuracy and completeness of interpretation and evaluation of instructions, if applicable, and proficiency in writing data sheets. On the basis of a general procedure, the candidate shall write a data sheet which could be used by a Level 1 for a sample of his/her main activity.

The data sheet will define the particular prescriptions for the assessment of a sample:

- definition (selection) of the equipment - test parameters (calculations to be performed, if necessary) - standardisation (drawings, signal representation, etc.) - test performance - methodology and principle for sizing the indications - acceptance criteria (to be retranscribed on the basis of a given general procedure) - characterization of the indications (nature, type) - identification of the instructions (localisation, transfer on a plan/drawing, conformity/non conformity) - minutes of proceedings The checklist used in the approved examination centre for scoring shall evaluate competencies in: - use and setting of the equipment and its accessories, - strict compliance to the procedures, - relevance of the interpretation and assessment of the instructions, - documentation of the results, - precise and exhaustive writing of the written instructions. Duration of the practical examination will be defined by COSAC.

7.1.4.3 Level 3

The candidate shall demonstrate proficiency by preparing an NDT procedure or work instruction or data sheet (see § 7.1.4.2) appropriate to the requirements of aerospace industry. He/she shall justify his/her choices (means, methods, techniques, etc.) and define a work instruction to be used by a Level 1 individual in the appropriate method.

The checklist shall address the technical and industrial proficiency of the procedure and of the data sheet (see §7.1.4.2) prepared by the candidate.

Results of the practical examination shall be documented.

Agreed duration of the practical examination is 5 hours.

When candidate's functions include controlling products and/or sanction, and when the candidate has no valid N2 COSAC certification when he/she participates to Level 3 examination, his/her capability to execute these tasks shall be demonstrated. He/she shall then pass the practical N2 examination in the appropriate technique, pursuant to the requirements of paragraph 7.1.4.2. (except writing a work instruction). The results of the practical examination shall be documented and give the candidate the Level 3 inspection option in the appropriate technique. Validity of this option corresponds to the Level 3 certification. The Level 2 certification is not necessary for Level 3 agents who write or approve procedures or data sheets (see § 7.1.4.2), even if they are used to accept or reject the products.

7.2 Organisation of the examination

During the exam, the candidate shall possess a valid proof of his/her identity and an official notification to attend the examination. When requested by the examiner, or by a supervisor, the candidate shall show them. A work plan is communicated to the candidates in order to assure the examination occurs smoothly. All the multiple choice test questions are of true/false type. Level 1 and Level 2 examinations are performed by COSAC approved examination centres, pursuant to CER/COSAC-PR-003 procedure. Organisation and scoring of the Level 1 and Level 2 examinations are under the responsibility of a Level 3 COSAC agent who will work as examiner:

- in the method of the examination, - who has passed a Level 2 or Level 3 practical examination with the "Inspection" option, - appointed by the manager of the approved examination centre who uses his/her services.

The examining board includes the technical manager of the examination centre and the examiner(s) in charge of the candidates. Organisation and scoring of the Level 3 examination are under the responsibility of COSAC, which appoints the president of the examining board, the supervisors and the Level 3 agents or members of the COSAC committee in charge of scoring the examinations. A Level 3 agent shall not be the only examiner of the candidates for whom he/she is the hierarchical responsible or he/she has personally trained for the examination. An examination shall never be organised personally by an agent or one of his/her subordinates. During an examination, if a candidate does not comply with the rules or commits a fraudulent act, or is accomplice in such an act, he/she shall be immediately excluded and won't be able to perform the following tests. The candidate shall wait at least one year before he/she can perform again an examination. If an incident occurs, like the use of non-conform questionnaires, test instructions, material, parts or products, the examiner shall mention this incident in the minutes and notify the examining board during the final deliberation.

7.2.1 Scoring

7.2.1.1 Level 1 and 2

The "general" and "specific" written examinations are assessed by comparing the candidate's answers with the approved key to each question, respectively by COFREND and COSAC. Each correct answer corresponds to 1 point and the final score for the examination is the sum of the resulting points. For the final calculation, each examination score is expressed on the basis of 100. The scoring of the practical examination takes the following operations into consideration: 1) Knowledge of the equipment (implementation, functions, controls and setting). 2) Implementation of the test method: - Visual examination and samples preparation, - Detection of the indications and their interpretation (position, size and dimensions), - Assessment of the results according to the acceptance criteria. 3) Test minutes. 4) Reconditioning at the end of the test (demagnetization, cleaning, protection). For Level 2 candidates:

5) Metallurgic knowledge of the parts to be tested. 6) Writing a work instruction for a Level 1 agent. The detailed elements of each operation are illustrated in the following table: Table VII

7.2.1.2 Level 3

The scoring of each part of the general examination shall be established separately. A certified agent who changes industrial area receives particular scores. The "general" and "specific" written examinations are assessed by comparing the candidate's answers with the approved key to each question, respectively by COFREND and COSAC. Each correct answer corresponds to 1 point and the final score for the examination is the sum of the resulting points. The practical examination is corrected by two Level 3 examiners by comparing the candidate's script with the correction guide prepared by COSAC. Each examination has a maximum score of 100. All the examination scores shall have the same coefficient for determining the average score.

7.2.2 Criteria for passing the examination

To be eligible for certification:

- the candidate shall obtain an average score of at least 80%. - the candidate shall obtain a score of at least 70% in the general, specific and practical examination. - the Level 1 or 2 candidate shall identify all the discontinuities or conditions (ex: non conform thickness, etc.) specified by the approved centre of examination during the practical examination.

7.2.3 Re-examination

Candidates failing any general, specific or practical examination shall receive additional training before attempting re-examination of the failed exam. The additional training shall be documented and shall address those areas found deficient in the candidate's skills or knowledge. The re-examination shall not use the same written tests or test samples that were used in the failed examination. The re-examination test must contain a minimum of 25% new questions.

7.2.3.1 Partial

If a candidate obtains a score of less than 70/100 at one of the tests, he/she can attempt this test again if the re-examination takes place within 12 months after the failed examination.

A candidate who obtains no eliminatory score but fails to identify at least one of the discontinuities specified by the examination centre, and who is therefore not eligible for certification, shall attempt again the practical examination.

A candidate who fails one of the partial re-examinations can attempt it a second time. In case of failure, he/she shall apply to and attempt a new complete examination, pursuant to the procedure established for the new candidates, at least 1 month later.

7.2.3.2 Total

A candidate who doesn't comply with the criteria defined in § 7.2.2 and § 7.2.3.1 shall apply to and attempt a new complete examination, pursuant to the procedure established for the new candidates, at least 1 month later.

7.2.4 Extending a certification to a new technique

The candidate shall attempt a practical examination addressing the new technique pursuant to § 7.1.4. He/she shall comply with the acceptance criteria defined in § 7.2.2, with the scores he/she obtained in the last general and specific examinations. If he/she succeeds, the certification will be extended to this technique for the remaining validity period of the certification (cf. § 8.5).

7.3 Confirmation of the results

Level 1 and 2 examination: at the end of the examination, results are reviewed by the examining board. The examination centre confirms the qualification of the agents who comply to the conditions of § 7.2.2. The list of the qualified agents is sent to COFREND. Level 3 examination: at the end of the examination, results are reviewed by the examining board. COSAC confirms the qualification of the agents who comply the conditions of § 7.2.2. The list of the qualified agents is sent to COFREND.

7.4 Records of the results

Level 1 and 2 examination: at the end of the examination and after deliberation, results are reviewed by the examining board. The scores obtained in each test are summarized in a sheet, which is reviewed by the technical manager of the examination centre. Level 3 examination: at the end of the examination and after deliberation of the COSAC Counsel, the scores obtained for each test are summarized in a sheet, which is reviewed by the President of COSAC.

8 CERTIFICATION

If the COSAC examination doesn't cover all the employer's needs, the Responsible Level 3 shall implement a further specific and practical exam, in compliance with EN 4179.

8.1 General

Each agent qualified according to the previous paragraphs is eligible to certification. The follow up of the steps of the qualification process by the various instances is done through the information application GERICCO (GEstion par Réseau Internet de la Certification COfrend). COSAC, in quality of NANDT BOARD, defines a document called "certification document" which must be validated by the employer in order to deliver the certification. The employer is the only responsible of the certification according to EN 4179. The employer shall also deliver his/her authorisation to proceed. The employer shall define and implement a documented follow up for maintaining the competencies of the agents whose roles include testing the products and/or sanction at least every 12 months. It is agreed that the certification date is the date when the qualification is confirmed. The certification is valid for 5 years, including the Level 3 inspection option.

8.2 Administration

8.2.1 Records

A record will be created for each agent and shall include: - a copy of the appliance documents, - a record summarizing the scores obtained in each test, - the changes of employer (if applicable), The individual files are created and recorded by the COSAC secretariat when the results of the examinations of the Level 1 and Level 2 agents are sent by the approved examination centres, and by the COSAC office for Level 3 agents.

When a candidate passes the qualification examination, le COSAC secretariat sends to the employer: - the second copy of the appliance documents, - the record summarizing the scores obtained, - the certification document (certification for Level 1 and 2, degree for Level 3).

If the candidate fails in part or totally the examination, COSAC secretariat keeps the appliance documents for 12 months. These documents are updated and sent to the approved examination centre when the candidate applies to a partial or total re-examination (see § 7.2.3 - Re-examination).

8.2.2 Establishing the certification document

The certification document is established, in two copies, by the COSAC secretariat, then sent to the employer. The employer: - records one of the two completed copies in the agent's individual file, - send the second completed copy to the COSAC secretariat, who will record it in the individual file. The completed certification document shall mention:

Level 1 and 2: - the signature of the approved examination centre manager, - the COSAC stamp and the signature of its President or of an authorized member, - the validation by the employer (employer's stamp, date and signature of its representative) in order to deliver the certification and the authorisation to proceed, pursuant to EN 4179. - the termination date.

Level 3: - the COSAC stamp and the signature of its President or of an authorized member, - the validation by the employer (employer's stamp, date and signature of its representative) in order to deliver the certification and the authorisation to proceed, pursuant to EN 4179. - the termination date. A specific certification document is prepared for the Level 3 agents who obtain the "Inspection" option.

The certification document is re-established at each re-certification. The following documents are recorded in the agent's individual file by the employer and by the COSAC secretariat.

8.2.3 Changing employer

The individual file recorded by the employer shall be delivered to the agent when he/she leaves. The employer shall withdraw the certification by crossing out its stamp on the certification document. The employer shall inform COSAC of this change. If the COSAC examination doesn't cover all the employer's requirements, the Responsible Level 3 shall implement a further specific and practical exam, in compliance of EN 4179. The employer can revalidate the agent's certification by affixing his stamp, dating and signing the certification document. The validity period remains the same. The new employer shall inform COSAC of this change.

8.2.4 Storage

8.2.4.1 Unexpired documents

The employer (E) and/or COSAC (C) shall record and update a certification file for the personnel as log as the certification is in force. These files shall be available for auditing by clients or authorities. Documents which are stored in the COSAC premises and not in the employer's can be consulted in the COSAC premises by the employer upon written request. The documents stored by the employer shall be at the employee's disposal, upon request or if he/she leaves the company, whatever the reason. The files shall report, at least, for each certification in force:

COSAC documents are stored in conditions which assure privacy and protection against fire and water damages. The list of the Level 1, 2 and 3 COSAC qualified agents is available on the COFREND and COSAC websites.

8.2.4.2 Expired certification

At the end of the certification's validity period, plus one year, the file of the written and practical examination of the candidate may be destroyed by COSAC. COSAC will archieve without duration limit, the summary tables of the results of the examination sessions transmitted by the approved examination centres.

8.3 Loss of certification

NDT certifications may expire, be suspended, or be revoked.

8.3.1 Expiration

Certifications shall expire when the agent leaves the aerospace industry and goes in another industrial field, or when the certification interval (as defined in § 8.5 and § 8.6) has elapsed with no recertification issued. Certification is considered to expire at the end of the corresponding month in which COSAC validated the first qualification.

8.3.2 Suspension

Certification shall be suspended when: - annual vision examination is expired, - the agent does not work with the certified method for at least 12 consecutive months, - it is noticed that the agent's competence is defective, - the annual maintenance is expired, - the agent fails recertification examination.

The vision acuity examination and the control of competencies are considered expired at the end of the month corresponding to the date of the document.

8.3.3 Revocation

Certification shall be revoked in the following cases:

- the agent does not work with the certified method for at least 24 consecutive months, - the agent is found to be incompetence or his/her conduct is found to be unethical. - employment is terminated. When an individual is re-hired by the same employer within 24 months,

certification may be considered as suspended.

In this case the employer shall inform COSAC by sending the crossed out certification document.

8.4 Reinstatement of certifications

Certifications that have been suspended may be reinstated up to the original certification date

a) when the cause for the suspension has been corrected and verified: - by the employer (vision acuity, activity interruption, maintenance) - by the Responsible Level 3 (competency),

b) by the new aerospace industry employer (see § 8.2.3 and § 8.3.3).

Certifications that have expired or been revoked may only be reinstated by specific and practical examination equivalent to initial certification, or by the new employer, pursuant to § 8.2.3.

The employer shall establish a documented follow up of the suspensions, revocations or expirations.

8.5 Re-certification of the Level 1 and Level 2 agents

Level 1 and Level 2 agents shall be recertified at intervals of five years. Level 1 and Level 2 agents shall complete specific and practical examinations equivalent to those required for initial certification A recertification examination must cover all the techniques of the method in which the candidate is certified, except when employer explicitly requires that one or more techniques shall be avoided. It is recommended that employers submit their candidates to a training in order to update their knowledge before they attend the examination. In case of failure, the certification is immediately suspended as soon as the employer is informed.

8.6 Re-certification of Level 3 agents

Level 3 personnel shall be recertified at intervals of five years.

a) Recertification shall be accomplished: - by credit points in accordance with Annex 1, - by successful completion of specific and practical examinations equivalent to initial certification.

b) When candidate's functions include controlling products and/or sanction, and when the candidate has

no valid Level 2 COSAC certification when he/she participates to Level 3 examination, his/her capability to execute these tasks shall be demonstrated. He/she shall then pass the practical Level 2 examination in the appropriate technique(s), pursuant to the requirements of paragraph 7.1.4.2. (except writing a date sheet). The results of the practical examination shall be documented and give the candidate the Level 3 inspection option in the appropriate technique(s). Validity of this option corresponds to the Level 3 certification. The Level 2 certification is not necessary for Level 3 agents who write or approve procedures or data sheets (see § 7.1.4.2), even if they are used to accept or reject the products.

In case of failure, the certification is immediately suspended as soon as the employer is informed.

8.6.1 Administration of the re-certification

At the end of the validity period, the nondestructive testing agent attends a re-certification examination in an approved examination centre.

Appliance documents shall be prepared for the re-certification examination, according to CER/COSAC-PR-004, and sent to COSAC.

For re-certification by credit points, according to Annex 1, the candidate shall collect the certificates of his/her points and sent his/her documents, which will be reviewed by two Level 3 member of the COSAC council.

Recording of the results, establishment of the certification documents and recording of the documents are the same than for the initial certification.

9 UNEMPLOYED TESTING AGENTS

9.1 Non certified agent

It is accepted that the internal procedure for certification and the requirements connected to the employer and the Responsible Level 3 are formalized after the qualification examination. The certification document will be delivered by COSAC only when compliance of all the requirements stated in this procedure are complied with.

9.2 Ad interim workers

The company who complies with the requirements of this procedure is considered as an employer, in particular as regard the Responsible Level 3 and the internal procedure. The employer is indicated in the certification document and authorises to proceed.

9.3 Agent having a certification in force when he/she leaves his/her employer

This clause only regards agents who worked in the aerospace industry and when the duration of the interruption of their activity is not more than 12 consecutive months during the 5th year of his/her certification or re-certification. In this case, the agent is subject to the provisions of § 9.1 and/or § 9.2 of this procedure.

10 CERTIFICATION FEE(S)

Each examination is subject to a fee, received by the examination centres (Level 1 and 2) or by COSAC (Level 3). This fee covers the organisational costs of the examination (material and personnel). For each certification and re-certification examination, the employer is asked to contribute to AFENDA's management costs. The amount of these costs is annually fixed during the general meeting of AFENDA. It considers the amount of the fees paid to COFREND. The amount of this fee is annually fixed during the general meeting of COFREND.

ANNEX 1

Role of the Responsible Level 3 in the qualification process regarding the COSAC procedures - Approving employer's internal application procedure.

- Being responsible and having the authority to ensure that the employer complies with EN4179

requirements.

- Transmitting the internal application procedure to COSAC.

- Supervising trainees monitoring. Approving, when necessary, the Level 1 agent or the instructor in charge of the direct observation during the development of the necessary experience.

- Indicating, in the internal application procedure, if a Level 1 agent is authorised to perform the

interpretations and assessments to accept or reject products.

- Assuring that the training complies with the employer's needs, if an examination centre approved by COSAC is used.

- Assuring that the requirements of EN 4179 are complied with and that the training complies with the

training outlines defined by COSAC, if training is performed in a centre which is not approved by COSAC or internally. In this case, approving the centre and its trainers.

- Approving the contents of the training to the work place.

- If the training is anterior to the training outlines defined in the internal procedure, determining if it is

acceptable and defining the additional training to be attended.

- In case of non-certification within 12 months after the training program of the internal procedure, determining the reorientation training to be attended.

- For agent previously certified without COSAC, determining the conformity of their training according

to this procedure.

- Assuring that a previous radiation protection training is supplied to the concerned candidates.

- Documenting the experience gained with a previous employer and approving partially or totally its consideration.

- For personnel qualified with other programs, documenting the gained experience and defining its

equivalence.

- Appointing personnel trained for controlling the NDT agents' vision, if they are not qualified medical personnel.

- Evaluating the impact of any limitation of the perception of colours before certification.

- Determining the need for additional specific and practical examination which represents processes and products in the employer's premises.

- Verifying agent's competence before reinstate his/her suspended certification for deficient

competence.

- Determining and documenting additional training in case of failure in a general, specific or practical examination before re-examination.

ANNEX 2

Credit systems for requalification of Level 3 NDT personnel (according to § 8.6 a) A.1 PURPOSE

A.1.1 This annex specifies the requirements for re-certification without examination of the Level 3 NDT personnel according to § 8.6 a. It only applies to personnel who have a Level 3 COSAC certification in force at the time of the re-certification. A.2 REQUIREMENTS A.2.1 Employer shall send the request for re-certification to COSAC at least 4 months, and 2 months at the latest, before the certification's expiry date. A.2.2 The candidate must have been employed in a Level 3 role during at least 36 months (of which at least 12 during the last 24 months) during the last 5 years, in the method for which he/she requests the re-certification. The quantity of months is cumulative, and the months doesn't need to be consecutive for being validated. A.2.3 The Level 3 candidate shall demonstrate his/her continuous activity in the NDT method for which he/she asks a re-certification and document 8 tasks for a 5-year period. A.2.4 The candidates shall provide objective proofs that they kept updated on the current NDT technology in the method for which they ask for re-certification, by obtaining at least 24 points during the 5-year period of the certification, whatever the number of certifications (methods) obtained, by committing in one or more activities listed in the Table A.1. The confidential documents which have not been sent must be available for auditing. If the candidate changes employer during the 5-year period of validity, the points gained with the previous employer will be counted only if the current employer possesses the certifying file. A.2.5 Employer shall determine and document a credit points re-certification file of the Responsible Level 3. During the 5-year period, a certificate will only be used once for a given NDT method.

A.3 DEFINITIONS A.3.1 Committee or commissions meetings: Meetings, conferences, symposium, seminars, commercial exhibitions, poster sessions, etc. organised or sponsored by a regional, national or international organisation or NDT technical association. The above mentioned meetings, whether they are foreign or international, can only be considered if the sponsors are national or international. A.3.2 Committee projects: Specific and recognizable official activities of regional or national technical associations, committees or work groups, like inter-laboratory or individual studies, preparation of directives, annexes, specifications, technical recommendations, procedures, codes or standards, etc. Documentation can include brief or complete reports of draft projects prepared by committees or official comments written by the candidate on these documents.

Commitment model for employers of nondestructive testing agents

RIGHT TO USE THE COSAC CERTIFICATION

CODE OF ETHICS By signing this document, the employers of nondestructive testing agents candidate to the COSAC certification commit to comply with the following rules: Conduct to third parties: - make the testing operations executed by nondestructive testing agents certified according to PrEN or EN

4179, when required by the product specification or standard; - its agents won't abuse of the certification, for example for advertising purpose; - not mention its nondestructive testing agents certification, in particular when their validity has expired. Obligations to COSAC: - knowing and applying the qualification and certification rules that are in force; - use the nondestructive testing agents according to their activity area, method and competence level; - refuse any association with de fraudulent act as regards the certification rules; - inform COSAC of any conduct contrary to the code of ethics or any professional fault, or cause of

suspension of the authorisation to proceed. Obligation to the certified agents: - allow all the necessary means to perform and interpret the nondestructive tests their agents are in charge

with. - forbid any kind of pressure in order to modify the results of the test. - comply with the ethics code, if applicable to personnel in charge of the certification. - when an agent leaves, the employer shall deliver him/her the certification document after having crossed

out his/her authorisation to perform and inform COSAC of this change. The employer accepts that, in case he/she doesn't comply with this rules, COSAC reserves the right to act in

justice at its own discretion.
