    Responsible for identifying, researching, analyzing and resolving complex

    Job Requirements

    Job Description

    Move components and chargers to and from the work area as required. Use hand and power tools to assemble all parts, will be required to operatesolder gun and station.     Intake Coordinator (approx. 50%)

    As Intake Coordinator you will be the first point of communication and contact to potentialclients of our program via telephone triage, home visits and the internet. Once a client and caregiver are placed, the Intake Coordinator will completethe initial home assessment and caregiver credentialing activities.     Care Manager (approx. 50%)

    As Care Manager, you will work collaboratively as part of a case management team toassess clients, establish and implement a plan of care, conduct regularly scheduled homevisits, and oversee client care. The case management team credentials caregivers and theirhomes, and provides ongoing support and training.

    Qualifications:

    Minimum of two years experience in case management, care planning and assessment

    for elders/disabled adults in a health care, long-term care, social service or communitysetting.

    Candidate must have a bachelors degree and be licensed in social work or nursing, orhave an equivalent combination of education and experience in these or related fields.

    Skills/Knowledge Required:

    Knowledge of case management systems, community service and delivery systems foraging population required.

    Strong clinical and communication skills are essential with competent use of technologyapplications.

    Daily car travel within assigned regional geography is required.
