Director’s Message

CHÉOS has had another successful year, owing to the untiring efforts of our research scientists and members of their staff, the support of St. Paul’s Hospital and Providence Health Care Research Institute. Our Centre continues its important role, contributing to the body of knowledge on which clinicians, public health practitioners, and policy-makers draw to improve both patient care and the functioning of Canada’s health care system in general. CHÉOS’ 40 research scientists were engaged in roughly 250 projects in 2010-11, ranging from modest studies to multi-year, multi-site projects, with budgets in the millions. This report provides an overview of these. CHÉOS fosters and promotes the future generations of researchers; in this past year 27 research trainees were supported in pursuing their careers. I am proud of our collective accomplishments and am pleased to have this opportunity to report on these for 2010-11.

About CHÉOS

CHÉOS is a group of experienced researchers that conducts high impact health outcomes research. CHÉOS researchers evaluate the effectiveness of health interventions on populations to understand how to improve health and transform health systems for the future. We’re an interprofessional team based at Providence Health Care (PHC) at St. Paul’s Hospital in Vancouver. We are supported primarily through the grants held by our researchers and clients, and receive base funding from PHC. In addition to conducting our own research, we also offer our methodological expertise to other researchers. Our research support services include assistance with study design, statistics, health economics, and data management for both health outcomes research and clinical trials. Since its inception in 1997, CHÉOS has become one of Canada’s foremost health outcomes research organizations. The Centre continues to attract talented epidemiologists, statisticians, economists and analysts to work with clinical researchers from various fields of medicine.

CHÉOS investigators are known for groundbreaking research to improve care for our most vulnerable urban populations. Our research includes the study of urban populations with mental health issues, addictions, and infectious diseases. It involves the health of marginalized groups including Canada’s First Nations and ethnic minorities. Our research also includes the study of people who are critically or chronically ill and the elderly. All of these populations require complex care that challenges the fabric of our health care system. Improved care for these populations is urgently needed.

CHÉOS exists to provide and translate evidence for change. As clinicians and epidemiologists, CHÉOS researchers are embedded in the health care system to ensure our research is relevant and driven by clinical need. Furthermore, our researchers and health partners believe that research is needed to form a continuous loop of improvement where clinical practice informs research, and research informs clinical practice. Ultimately, we seek to influence change and improve health and health systems by empowering decision makers to make positive change based on evidence.

CHÉOS Research Scientists Appointed in 2010 – 2011

Garth Hunte, MD, PhD Garth Hunte is an Emergency Physician at St. Paul’s Hospital and a Fellow of the College of Family Physicians of Canada. Garth graduated in medicine from the University of Alberta, and trained in family medicine, emergency medicine, and sports medicine. He completed fellowships in sports medicine and emergency medicine, as well as a Master’s degree in Exercise Science, and a PhD at UBC’s School of Population and Public Health. His primary research interest is patient safety.

Stephen Pinney, MD Dr. Stephen Pinney completed his undergraduate training in biochemistry at Harvard

in 1987. He subsequently obtained his medical degree from McGill and then moved to Vancouver to undertake a rotating internship at St. Paul’s Hospital. He started his orthopaedic residency training at UBC. In 2010, he accepted the position of Head of the Division of Orthopaedics at Providence Health Care and Head of the UBC Division of Distal Extremity Surgery. Dr. Pinney specializes in foot and ankle surgery and operates a clinical practice out of St. Paul’s Hospital and the Vancouver Bone and Joint Clinic. Verena Strehlau, MD

Dr. Stehlau is a post-doctoral fellow in the Department of Psychiatry at UBC, and is the Field Office Coordinator for the AT HOME / Chez Soi Project. Iris Torchalla, PhD Iris Torchalla graduated as a Clinical Psychologist from the University of Tuebingen, Germany. She worked as a research assistant at the University Clinic for Psychiatry and Psychotherapy, and was a research assistant at the British Columbia Centre of Excellence for Women’s Health, holding two CIHR-funded postdoctoral research fellowships. She now works with CHÉOS, focusing on developing and improving

interventions for individuals with concurrent disorders.

Research Conducted at CHÉOS This Year CHÉOS’ 40 scientists had over 250 research projects underway in 2010-11 ranging from modest studies to multi-year, multi-site projects. Conferences and Educational Sessions

CHÉOS sponsored and participated in several national and international conferences in 2010 – 2011. Some examples of these include the following:

Health of the Homeless Summit 2010 – June 21-22, 2010. Researchers at CHÉOS are involved in a number of research initiatives involving homeless populations and the pervasive health disparities they experience. The underlying objectives of these projects are to bring to attention to the significant health risks associated with homelessness, and the need for integrated solutions in the sustainable and effective treatment of their health care needs. Novel and unique initiatives were presented, as well as discussions surrounding treatment approaches and other critical considerations.

A Mental Health Strategy for Canada and British Columbia – December 13, 2010. Mental health and addiction are responsible for about 25% of the burden of disease. This has tremendous consequences for individuals, families and society. The Mental Health Commission of Canada, under the leadership of Michael Kirby, initiated a dialogue on the core elements of a national strategy to address this pressing health care issue. The provincial government in BC also announced the development of a 10-year plan for mental health in 2008. Both are critical for the future of health care, and acknowledge the underfunding of mental health in Canada, and the necessity of a paradigm shift in system development internationally.

Lost in Translation – February 15-17, 2011. The driving force behind this conference was to create an opportunity to overcome scientific and clinical isolation, to stimulate curiosity, and most importantly, to arrive at some clinically relevant answers. The common goal is the implementation of better health care, the development of more effective interventions and a better understanding arising from new and improved hypotheses.

Resources and Services The Centre for Health Evaluation and Outcomes Sciences (CHÉOS) helps clinicians and researchers to publish evidenced-based academic papers on health outcomes. CHÉOS offers the following services:

The Cedar Project

“If you believe that the children are our future,

then the future is now.” Chief Wayne Christian, Splats’in-Secwepemc First Nation

The Cedar Project is a community-based research initiative that addresses hepatitis C and HIV vulnerabilities among young Aboriginal people who use illicit drugs in three cities in British Columbia. On a global scale, the historical trauma associated with the process of colonizing Indigenous people is commonly considered to be the explanation for the profound health disparities they experience all over the world. To date, there is little data that accurately describes the extent to which survivors of residential schools and their children are affected by HIV/AIDS in Canada. The Cedar Project seeks to understand the relationship between historical trauma including having a parent or grandparent who attended residential school or being in the child welfare system and vulnerability to HIV and hepatitis C infection among Aboriginal young people who use drugs. The study is led by CHÉOS scientists Drs. Patricia Spittal and Martin Schechter, and is conducted in full partnership with Aboriginal leadership and AIDS service organizations. The Cedar Project Partnership includes: Chief Wayne Christian and Splats’in Secwepemc First Nation, Elders Violet Bizoki, Earl Henderson, Positive Living North, Red Road HIV/AIDS Network, Katharina Patterson, Vancouver Native Health, Prince George Native Friendship Centre, Carrier Sekani Family Services, Healing Our Spirit, Central Interior Native Health, Adams Lake Indian Band and Neskonlith Indian Band. To our knowledge, the Cedar Project is the first Indigenous 'at risk' young people's cohort in North America. Since the Cedar Project began in 2003, a wealth of data has been accumulated, allowing Cedar investigators to expand the breadth of their research. They have found increases in the rate of both HCV and HIV infection among study participants, as well as an increase in transition to injection drug use. While the majority of infections are related to injection drug use, factors that explain elevated risk and transmission of HIV among Aboriginal young people who use illicit drugs are not well understood. Researchers now want to identify how certain members of the cohort avoid being infected, and develop ways of making healthcare more accessible for those who are HIV positive. The NAOMI and SALOME Studies The North American Opiate Medication Initiative (NAOMI) was a randomized trial aimed at testing whether medically prescribed diacetylmorphine, the active ingredient in heroin, was more effective than methadone therapy for individuals with chronic opioid dependence who were not benefiting from other conventional treatments. The results have been published and showed that patients treated with injectable diacetylmorphine were more likely to stay in treatment and more likely to reduce their use of illegal drugs and other illegal activities than patients treated with oral methadone. In the NAOMI study, we also provided injectable hydromorphone (Dilaudid®), a licensed medication, to participants. An unexpected finding was that injection patients could not accurately discriminate whether they were receiving diacetylmorphine or hydromorphone. We also observed similar results and benefits with both these drugs although the small number of participants receiving hydromorphone did not permit us to draw any definite and scientifically valid conclusions as to the efficacy of hydromorphone as a treatment option. Should hydromorphone be proven to be as good as diacetylmorphine, the benefits of this form of injectable treatment may be more feasible and achievable without the emotional and regulatory barriers often presented by heroin maintenance. Therefore, the NAOMI investigators designed a study to test this hypothesis, as a next step of the NAOMI study. (Continued on next page.)

The NAOMI study has been concluded, and now the Study to Assess Longer-term Opioid Medication Effectiveness (SALOME) is a clinical trial that will test whether hydromorphone (Dilaudid®), a licensed medication, is as good as diacetylmorphine, the active ingredient of heroin, at benefiting people suffering from chronic opioid addiction who are not benefiting sufficiently from other treatments. This study will also test if those effectively treated with these two injectable medications can be successfully switched and retained to the oral formulations of the medications. SALOME is a two-stage Phase III, single site (Vancouver), randomized, double blind controlled trial involving a total of 322 individuals with chronic opioid-dependency who currently are not sufficiently benefiting from conventional therapies, such as methadone treatment. Enrolment is anticipated to begin in the fall of 2011. SALOME addresses critical social and ethical concerns dealing with addiction. Opioid-dependent people are in need of treatment options to avoid marginalization from the health care system. This study aims to answer questions that could lead to improvements in the health of persons with chronic addictions and identify new ways of reintegrating this population into society. The SALOME study is led by CHÉOS scientists Drs. Eugenia Oviedo-Joekes and Michael Krausz. Collaboration for Outcome Research and Evaluation

The University of British Columbia’s Collaboration for Outcomes Research and Evaluation (CORE) group addresses the need for clinical and economic research and education in pharmaceutical outcomes assessment. The CORE team is a multidisciplinary group of researchers with expertise in clinical pharmacy, pharmacoepidemiology, health economics, health services research, program evaluation and health promotion research. The CORE group is headed by CHÉOS research scientists, Drs. Carlo Marra and Larry Lynd. Dr. Marra is an Associate Professor in the Faculty of Pharmaceutical Sciences at UBC, and a Tier II Canada Research Chair in Pharmaceutical Outcomes. He holds Scholar Awards from the Canadian Arthritis Network and the Michael Smith Foundation for Health Research as well. His main research interests have been in health economics, quality of life research, and pharmacoepidemiology. (Continued on next page.)

Dr. Lynd is an Associate Professor in the Faculty of Pharmaceutical Sciences at UBC and has an adjunct appointment to the School of Population & Public Health, Faculty of Medicine. He holds a Scholar Award from the Michael Smith Foundation for Health Research and is a CIHR New Investigator. He also sits on the Board of Directors of the Canadian Association of Population Therapeutics, and serves on the National Drug Scheduling Advisory Committee and the Special Advisory Committee to the Respiratory and Allergy Therapies division of Health Canada. Addiction & Concurrent Disorders Group The Addiction and Concurrent Disorders Group at CHÉOS conducts research that looks at correlations between social determinants of health and childhood trauma, and thus, mental illness and addiction. The catalytic relationship between early life trauma, substance use disorders and other mental illnesses is explored through a number of studies underway in the Addiction and Concurrent Disorders Group, including the At Home/Chez Soi study, the BC Homelessness Survey and the SALOME study, among others. CHÉOS researchers have long held an interest in studies aimed at alleviating the health problems of people with drug addictions. Dr. Michael Krausz, the newly-appointed UBC/Providence Health Care BC Leadership Chair in Addiction Research, and his colleagues at CHÉOS, approach the mental health and drug problem in Vancouver through a multi-dimensional interdisciplinary model that is rooted in principles of harm reduction and treatment, addressing both the source of the problem and its consequences. Proponents of the harm reduction approach believe in reducing the immediate harmful consequences of drug use through pragmatic, low threshold programs. Dr. Krausz and his colleagues conduct their work in the belief that interventions should be practical, involving evidence-based treatments that focus on reducing harms, maximizing intervention options, and maintaining human rights.

Pharmacoeconomics program Led by Dr. Aslam Anis, the Pharmacoeconomics Program is a multi-disciplinary team consisting of physicians, economists, bio-statisticians, pharmacists and epidemiologists. The Pharmacoeconomics Program undertakes many different projects in a number of areas of study, including pharmaceutical pricing and regulation, health technology assessment, economic burden of illness and health services research. The work of the Pharmacoeconomics Program encompasses many different health conditions including HIV/AIDS, arthritis, asthma and obesity.
