To order breast pumps or accessories, please complete the bottom portion of this form by typing answers and submit to Lisa Schlientz, by email, at Lisa.Schlientz@health.mo.gov by COB on Friday, January 15, 2021. You will receive a confirmation email stating that your order has been received. The state agency is offering two different multi-user pumps for this order, based on feedback from local agencies. Both the Hygeia Enjoye and Hygeia Endeare are multi-user, closed system pumps with a three year warranty. The Endeare is very similar to the Medela Lactina. Please see differences in pumps here, and choose the pump that will be more useful for your participants.

The state agency will place orders after closing the order opportunity. Due to the effect of Covid on supply chains and shipping, it is likely that delivery may be delayed. The state agency will send out a notice about shipments as the information becomes available.

All products will be shipped directly from the supplier. The following procedure must be followed in order to verify receipt of shipment for payment and to receive the required WIC tags:

1. Immediately upon receipt of shipment, please locate the packing slip and verify your agency received the products and quantities stated. If all items have not been received, check to see if items are noted as backordered.

2. If items are missing, but not noted as backordered, wait to send the packing slip until all items have been received.

3. If all items are delivered, or correctly noted as backordered, sign the packing slip to verify the order is correct.

4. Fax the signed packing slip to the WIC state agency at 573-526-1470. WIC tags will be mailed upon receipt of your packing slip.

5. If items were backordered, when they are received follow steps 3 and 4.

6. If your agency did not receive what was stated on the packing slip, please contact the WIC state agency for further instructions.
