    6 MistakesCataract Patients Make That Cause

    Unnecessary Complications in Treatment and How

    to Avoid them

    Mistake 1: Choosing the Wrong Doctor

    Many patients do not ask if the Doctor has enough relevant experience. Many times,

    this leads to an inexperienced Doctor performing the surgeries. These Doctors learn

    from the mistakes they make on your precious eyes.

    What you should do:

    Ask how many Cataract Surgeries your Doctor has performed. Ask for details of past

    patients and feel free to call up a few past patients to get their opinion. Ask about the

    current kind of surgery that the Doctor is performing and how many eyes he hasoperated upon using this current procedure. (This report provides you with an overview

    Mistake 2: Not using the eye drops prescribed by the Doctor as per

    his recommendations.

    When patients do not follow the doctors instructions in the use of the prescribed eye

    drops, they may experience pain and swelling inside the eye. Worse, they may even get

    an infection and unnecessarily complicate their treatment.

    What you should do

    Make sure you follow the Doctors prescription totally. Use the exact dosage for the

    exact period of time recommended by your Doctor. If the Doctor has prescribed a

    painkiller, take it as per his instructions. Chances are it also functions to bring down

    swelling and promote healing along with reducing pain. Not taking it may slow down the

    healing process.

    Mistake 3: Performing tasks contrary to Doctors Instructions

    Patients who insist on lifting heavy objects, bending down from the waist and performing

    tasks that the Doctor has instructed them to avoid, are putting themselves at great risk.These tasks increase the pressure in the eye and can cause complications in the

    operated eye.

    What you should do

    Avoid certain tasks for a few weeks after your cataract surgery. Your Doctor will give

    you clear instructions about what these tasks are. By following your Doctors

    instructions, you are helping your body to heal better and faster, without trouble.

    Mistake 4: Not discussing your current medication in detail with your

    Doctor.

    There are certain medicines (like Tamsulosin) that can cause serious problems (such as

    Intraoperative floppy iris syndrome (IFIS) ) in the eye after a cataract surgery even

    when the medicine has been discontinued for year before the surgery in some

    cases.

    What you should do:

    Give a full, detailed exhaustive list of all the medicines you are taking currently and have

    taken in the recent past to your doctor. S/He can then advise you on managing the

    medication to avoid complications during the surgery.

    Mistake 5: Not going for proper Follow-up check-ups

    Patients often skip follow-up visits. In this case, some problems that may be prevented if

    detected earlier, are allowed to manifest till they are major complications.

    What you should do

    Visit the Doctor when the visit is scheduled. It is up to you to keep track of your date of

    operation and the number of weeks going by. Make sure you meet your doctor as per

    his/her recommendations. Follow-up check-ups are required at different stages of the

    healing process so that the Doctor can ensure that you are healing properly and there

    What is cataract?

    A cataract is an eye disease in which the normally clear lens of the eye becomes cloudy or opaque,

    causing a decrease in vision. Cataracts are usually a very gradual process of normal aging but can

    occasionally develop rapidly. They commonly affect both eyes, but it is possible for a cataract in one

    eye to advance more rapidly. Cataracts are very common, affecting roughly 60% of people over the

    age of 60.

    Why do Cataracts Occur?Precisely why cataracts occur is unknown.

    But as we age, some of the protein mayclump together and start to cloud a smallarea of the lens. This is a cataract, and overtime, it may grow larger and cloud more ofthe lens, making it harder to see.

    Many studies suggest that exposuretoultraviolet light is associated with cataractdevelopment. people with diabetes, users ofsteroids, diuretics, hormone replacementtherapy (HRT) and major tranquilizers areat risk for developing a cataract.

    What is the treatment for cataracts?Many cataracts are not bothersome, In that situation, no surgical treatment is

    necessary. However, the only true treatment for cataract is surgical removal of the

    cloudy lens.

    Surgery is suggested if the patient loses the ability to perform necessary activities of

    everyday life, such as driving, reading, or looking at computer or video screens, even

    with glasses, and there is the expectation that vision will improve as a result of the

    surgery.

    Depending on a patient's specific visual needs, surgery is sometimes done on cataracts

    that are not very dense or surgery can wait until the cataract and the vision gets more

    cloudy. Since the lens of the eye is necessary to accurately focus light onto the retinal

    surface and removal of the cataract involves removal of the lens, modern cataract

    surgery combines removal of the lens with placement of a new artificial lens into theeye. Cataract removal is very common in India.

    What is the progression of cataracts?The rate of progression of cataracts is usually predictable and surgery is successful in

    restoring vision in a large majority of cases. If other diseases are present within the eye,

    the degree of visual improvement will be limited by the other disease process. Your

    EYE Doctor can usually determine this in advance.

    When should you have Cataract Surgery?

    Most cataracts associated with aging develop slowly, and many patients may not noticevisual loss until it is fairly advanced.

    Despite being told that you have cataracts, it is not imperative to have surgery to

    remove them until they begin to affect your vision.A Cataract needs to be removed only

    when vision loss interferes with your everyday activities, such as driving, reading,

    participating in sports, or watching TV.

    Thus, the decision and timing to proceed with cataract surgery is individualized for each

    patient. You and your Doctor can make this decision together. Your doctor will be able

    to tell you how much of your vision loss is due to cataracts and the type of visualrecovery that may be expected if surgery is chosen.

    If the eye has other diseases that have caused visual loss such asglaucoma,macular

    degeneration,diabetic retinopathy,or optic nerve damage from glaucoma, cataract

    surgery may not improve the vision significantly.

    People who need cataract surgery may also have other eye conditions, such as age-

    related macular degeneration or glaucoma. If you have other eye conditions in addition

    to a cataract, please talk with your eye doctor to learn about the risks, benefits,

    alternatives, and expected results of cataract surgery in your particular case.

    If both eyes have cataracts and surgery is agreed upon, the surgery on the second eye

    is generally planned at least a week after the first eye. There is usually no harm in

    waiting a much longer period of time between the two eye operations.

    What are the different Types of Surgery?There are five types of cataract surgery to remove your cataract and replace it with an

    intraocular lens (IOL) implant to help improve your vision. They are

    Phacoemulsification, Femtosecond Laser-Assisted Cataract Surgery,

    Extracapsular Cataract Surgery, Intracapsular cataract extraction (ICCE),Cryoextraction(A form of ICCE). Your eye doctor can explain the differences and help

    determine which method better for you. The most common procedure is

    Phacoemulsification.

    What Does Cataract Surgery Involve?Cataract surgery involves removing the cloudy lens in your eye and replacing it with an

    What to expect in Cataract Surgery?What Tests Are Generally Performed Before Cataract Surgery?

    A week or two before cataract surgery, your doctor will do some tests on your eyes.

    What to Expect Just Before Cataract Surgery

    It is important to remember to follow all of your preoperative instructions.

    As cataract surgery is done in a Hospital / Clinic, arrangements should be made

    with family or friends to transport you home after the surgery is complete.

    You will have a medical check-up by your regular doctor to make sure it is safe to

    have cataract surgery. This check up will usually include blood tests and an EKG.

    You will be asked not to eat or drink anything 12 hours before your cataract

    surgery.You may also be asked to apply eye drops on the day before surgery.

    What Happens During Cataract Surgery?

    To prepare your eye for cataract surgery, drops will be put into your eye to dilate the

    pupil and the area around your eye will be washed and cleansed.

    Cataract surgery usually lasts less than one hour ( many times only 15 minutes) and is

    almost painless. Many patients choose to stay awake during surgery. Others may need

    to be put to sleep for a short period of time.

    If you are awake, you will have an anesthetic to numb the nerves in and around your

    eye.

    After cataract surgery, a patch may be placed over your eye.After leaving the operating

    room, you will be brought to a recovery room where your doctor will prescribe several

    eye drops that you will need to take for a few weeks postoperatively.

    You will rest for a while. Your medical team will monitor your eye for any problems, such

    as bleeding.

    Most patients who have cataract surgery can go home within an hour of the procedure.

    You should always arrange for someone to drive you home.

    While you may notice some discomfort, most patients do not experience significant pain

    following surgery; if you do you experience decreasing vision or significant pain, you

    should contact your ophthalmologist immediately.

    What Happens After Cataract Surgery?

    You may experience little Itching and mild discomfort are normal after cataract surgery.

    Some fluid discharge is also common. Your eye may be sensitive to light and touch.

    After one or two days, moderate discomfort should disappear. If you have any

    discomfort that continues, contact your eye doctor to get this treated.

    For a few days after surgery, your doctor may ask you to

    use eye drops to help healing and decrease the risk of

    infection. Ask your doctor about how to use your eye drops,

    how often to use them, and what effects they can have.

    You will need to wear an eye shield or eyeglasses to help

    protect your eye. Avoid rubbing or pressing on your eye.

    When you are home, try not to bend from the waist to pick up objects on the floor. Do

    not lift any heavy objects. You can walk, climb stairs, and do light household chores.

    In most cases, healing will be complete within eight weeks. Your doctor will schedule

    exams to check on how you are progressing.

    Following surgery, you will need to return for visits within the first few days and again

    within the first few weeks after surgery. During this time period, you will be using several

    eyedrops which help protect against infection and inflammation.

    Within several days, most people notice that their vision is improving and that they are

    able to return to work. During the several office visits that follow, your doctor will monitor

    for complications.

    Once vision has stabilized, your doctor will fit you with glasses if needed. The type of

    intraocular lens you have implanted will determine to some extent the type of glasses

    required for optimal vision.

    What Problems Can Develop After CataractSurgery?Problems after cataract surgery are very rare, problems can include infection, bleeding,

    inflammation (pain, redness, swelling), loss of vision, double vision, and high or low eye

    pressure. If any of this occurs, please meet your Doctor immediately since these

    problems can usually be treated successfully.

    Problems generally occur due to patients non-compliance with Doctorspost-operativecare instructions and when patients do not give

    complete, detailed, exhaustive lists of current medication they are

    taking. For example, a common drug used for patients with Prostate

    problems can cause a complication called Floppy Iris Syndrome after

    cataract Surgery.

    When you discuss your current medication properly with your Doctors, they can advise

    you on managing medication to minimize the risk of complications.

    When Will Your Vision Return To Normal?You can return quickly to many everyday activities, but your visionmay be blurry. The healing eye needs time to adjust so that it can

    focus properly with your other eye, especially if your other eye has

    a cataract. Be sure to ask your eye doctor when it is safe for you

    to resume driving.

    If you received anIOL(lens implant), you may notice that colors

    have become brighter than before. This is because IOL is clear,

    unlike your natural lens which may have had a yellowish or brownish tint. Typically, you

    will become accustomed to the improved vision within a few months after receiving anIOL. Also, when your eye heals, you may need new glasses or contact lenses

    depending on the type of lens you had implanted and what vision correction was

    achieved.
