The Ulnar Nerve
Case 1A 23-year-old woman developed weakness in her right hand and numbness in her pinky and ring finger after sleeping on her elbows during a long plane trip.
• The ulnar nerve originates from the medial cord of the brachial plexus (C8 and T1 nerve roots). The

• Grossly, the myelinated tissue appears white and is called the white matter. The cell bodies of neurons are darker and are called the gray matter. In the brain, the gray matter is located on the periphery, while the white matter is located on the inside. In the spinal cord, the gray matter forms an “H” shape inside the spinal cord, while the white matter is on the outside. The appearance of the white and gray matter on MRI depends on whether the image is T1-weighted or T2-weighted. On a T1-weighted pulse sequence, the gray matter looks gray and the white matter looks white. On a T2-weighted pulse sequence, this pattern is reversed.

B: Axial T2WI of the braindemonstrates normal gray
and white matter.
C: Axial T2WI of the spinal corddeonstrates normal gray and

The Myelin Sheath• The myelin sheath is a collection of lipids and
proteins that surrounds each axon. Within the CNS, myelin is produced by oligodendrocytes, while in the PNS it is made by Schwann cells. A single oligodendrocyte can myelinate up to 50 axons, while a Schwann cell myelinates a single axon. The function of myelin is to speed the electrical signal on axons. In myelinated axons, the electrical signal can jump from one node of Ranvier to another, referred to as saltatory conduction. In axons that have lost their myelin, there is a continuous and slower propagation of the action potential along the axon.

Multiple Sclerosis and Related Disorders Clinical Guide to Diagnosis, Medical Management, and Rehabilitation
Alexander Rae-Grant, MD, FRCP(C), Staff Neurologist and Director of Multiple Sclerosis Education; Robert J. Fox, MS, FAAN, MD, Medical Director; Francois Bethoux, MD, Director of Rehabilitation Services; all of Mellen Center for Multiple Sclerosis Treatment and Research, Cleveland Clinic Foundation, Cleveland, OHMultiple Sclerosis and Related Disorders provides evidence-based data and experience-based guidance for delivering quality long-term care to MS patients. Information on disease history, pathophysiology, and biology is included to provide clinicians with a framework for understanding current
diagnosis, monitoring, and treatment strategies for these disorders. In addition to thoroughly reviewing the newest disease-modifying treatments, the authors have devoted significant focus to the symptoms that frequently manifest and their treatment options. Symptoms and functional limitations are the “face of the disease” for patients, and present their own set of management challenges for practitioners.
The book proceeds through diagnosis (initial symptoms, criteria and classification, imaging, lab tests, and differential diagnosis), approved treatments for the various MS types including emerging therapies where appropriate, symptom management, rehabilitative issues, general health and wellness, and an overview of MS clinical trials. Special populations, societal and family issues, and related disorders that are often mistaken for MS are also covered. Dedicated chapters on neuromyelitis optica and acute disseminated encephalomyelitis incorporate newer diagnostic criteria. Because comorbidities often make the management of MS-related disability more complex, the book addresses these comorbidities as part of a comprehensive management plan.
To enhance the clinical utility, critical-to-know information and management pearls are boxed for quick reference and most chapters include lists of “Key Points” for clinicians, and for patients and families. Illustrations, tables, graphs, assessment scales, and up-to-date MRI imaging inform the text throughout. The treatment chapters include specific recommendations where available and highlight areas of controversy. Illustrative cases go beyond the literature to amplify clinical recommendations and provide real-world guidance for practitioners.
Multiple Sclerosis and Related Disorders Features: q Comprehensive clinical reference for all members of the MS care teamq Focus on practical approaches to diagnosis, treatment, counseling, and rehabilitative strategiesq Reviews the latest disease modifying therapiesq Extensive chapters on symptom management and women’s issues in MS q Edited and written primarily by expert clinicians at Cleveland Clinic/Mellen Centerq Evidence- and experience-based guidance q Dedicated chapters on neuromyelitis optica and acute disseminated encephalomyelitis

Inherited Metabolic EpilepsiesPhillip L. Pearl, MD, Division Chief, Child Neurology, Children’s National Medical Center; and Professor of Neurol-ogy, Pediatrics, and Music, The George Washington University School of Medicine and Columbian College of Arts and Sciences, Washington, DC
The explosion of information in neurogenetics and metabolism mandates increasing awareness of appropriate diagnostic and therapeutic strategies in the setting of certain epilepsies, especially those of very early onset. There are over 200 inherited disorders that are associated with seizures and prompt identification and intervention is crucial for a positive outcome. This text brings together leading authorities presenting state-of-the-art clinical reviews covering the science, recognition, and treatment of the inherited metabolic epilepsies and related disorders.
Inherited Metabolic Epilepsies opens with a section on general principles for diagnosis and targeted intervention including screening protocols, laboratory testing, neuroimaging, seizure patterns and EEG findings, new technologies, and the ketogenic diet in metabolic epilepsies. The next two sections are devoted to the cohort of specific small molecule disorders (aminoacidopathies, organic acidopathies, mitochondrial disorders, urea cycle disorders, neurotransmitter disorders, and glucose-related disorders) and large molecule disorders (lysomal storage disorders, peroxisomal diseases, glycosylation defects, and leukodystrophies) that are treatable yet can be so vexing to clinicians and investigators. The book concludes with a clinical algorithm designed to be a resource for the physician in search of direction when considering an inherited metabolic disorder as the explanation for a patient with epilepsy.

Amyotrophic Lateral SclerosisA Patient Care Guide for Clinicians
Richard S. Bedlack, MD, Associate Professor of Neurology, Duke University Medical School and Director, Duke MDA/ALS Clinic, Durham, NC; Hiroshi Mitsumoto, MD, Wesley J. Howe Professor of Neurology, Columbia University Medical Center and Director, Eleanor and Lou Gehrig MDA/ALS Research Center and Neuromuscular Division, Neurological Institute of New York, New York, NY
Amyotrophic Lateral Sclerosis: A Patient Care Guide for Clinicians is a practical reference for clinicians caring for ALS patients that brings together the collective wisdom of those at the forefront of patient-oriented research and practice. The book compiles recent findings of both evidence-based and experience-based research to provide clinicians with tools that improve quality and length of life for people with ALS.
To present a truly multidisciplinary approach to ALS, this book mirrors the organization of a large clinic with separate departments working collaboratively. It begins with a review of current under-standings of ALS including diagnostic criteria, genetic and sporadic subtypes, epidemiology, co-morbidities, and prognosis. From there the book is divided into chapters that include neurological as-sessment, nursing care and coordination, speech and swallowing interventions, nutrition and nutrition therapy, physical therapy, occupational therapy, respiratory therapy, assistive technology, social work practice related to ALS, and web-based resources. Each chapter is led by experts from that discipline who review evidence- and experience-based care options. In addition, the entire North American ALS Research Group (ALSRG) has had a chance to weigh in as well, making this a unique and well-rounded resource. The book addresses everything from breaking the news of an ALS diagnosis to end-of-life care and bereavement. By putting experts in conversation with each other, both within and across individual disciplines, Amyotrophic Lateral Sclerosis: A Patient Care Guide for Clinicians provides comprehensive, real-world care information that can’t be found anywhere else.

State-of-the-Art Clinical Guidance for Management of Children With Neurocritical Illness
Pediatric Neurocritical CareEDITORS: Nicholas S. Abend, MD, Assistant Professor of Neurology and Pediatrics; Mark A. Helfaer, MD, Professor of Anesthesia and Critical Care Medicine, both of University of Pennsylvania School of Medicine, Children’s Hospital of Philadelphia, Philadelphia, PA
“This book is a must-read for all neurologists in training and for practicing physicians in need of direction for the future of neurocritical care in pediatrics. It is well written, easy to understand, and comprehensive in scope.” —Doody’s Reviews
Caring for children with neurocritical illness often requires multi-specialty collaboration and the understanding of an ever-expanding body of data. This book gives clinicians the up-to-date, concise, and clinically relevant guidance they need to provide optimal care to children with these acute neurologic disorders. The chapters in Pediatric Neurocritical Care are authored by experts in each individual topic, and co-edited by a neurologist and intensivist to ensure that all topics have been fully addressed from both perspectives.
The book covers the complete continuum of care from assessment, monitoring, and condition specific management through rehabilitation. Early chapters present differential diagnosis and management approaches to common overarching problems such as coma, headache, and elevated intracranial pressure, followed by chapters focusing on the evaluation and management of specific conditions including traumatic brain injury, stroke, seizures, central nervous system infections, and demyelinating disorders. The final chapters address important associated psychological, social, and ethical issues.
To reflect the full spectrum of specialties involved in the burgeoning field of pediatric neurocritical care, the book brings together an international group of experts from multiple disciplines including critical care medicine, anesthesiology, neurology, neurosurgery, rehabilitation medicine, psychology, and pediatric subspecialties.

Textbook of Peripheral Neuropathy is a practical but authoritative reference for clinicians in any medical specialty who are evaluat-
ing and treating patients with signs and symptoms of a peripheral neuropathy. Reviewing the full spectrum of clinically significant neuropathies, the book contains chapters on common and rare forms including mononeuropathy in the upper and lower extremities, mononeuritis multiplex, diffuse and sym-metric polyneuropathies, brachial and lumbrosacral plexopathies, and spinal root disorders-disorders that can mimic diffuse and/or focal neuropathies, complicating diagnosis and evaluation. Coverage encompasses both inherited and acquired diseases, including neuropathies arising from physical injury, diabetes, alcoholism, toxins, autoimmune respons-es, nutritional deficiencies, vascular and metabolic disorders, medication-induced neuropathies, and
idiopathic conditions. The textbook provides an evidence-based approach to testing, differential diagnosis, and treatment, and should serve as a trusted resource for healthcare professionals confronting the many manifestations of peripheral neuropathy in clinical practice. The chapters are written by internationally renowned expert contributors with deep clinical experience and contain numerous tables, figures, and algo-rithms providing clear diagnostic and management guidelines. Alternative Therapies For EpilepsyOrrin Devinsky, MD Profes-sor of Neurology, Neurosur-gery and Psychiatry, New York University School of
Medicine; Steven C. Schachter, MD, Professor of Neurology, Harvard Medical School; Steven V. Pacia, MD, Associate Professor of Neurology, New York University School of Medicine
“So many common, important and practi-cal issues are addressed in this book that it is must have on any health professional or individual with epilepsy’s library. In summary, this particular text is an excellent guide for anyone who is truly interested in getting questions answered that are often not covered in medical visits. I recommend this book to anyone interested in learning more about CAM for epilepsy.”—Joseph I. Sirven, MD, Epilepsy.com
Written by three distinguished epileptologists with a long-standing interest in alternative therapies, this book provides an evidence-based consider-ation of the use of complementary and alternative (CAM) therapies in epilepsy care. Organized by problem, the authors review alternative approaches to epilepsy-related conditions to help physicians, healthcare professionals, CAM providers, and patients understand the options and safely integrate treatments that work into their regimens.
The book begins with an overview of the therapies themselves, including herbal remedies, nutrition, alternative pharmacological therapies, physical treatments, and neurobehavioral approaches, and also discusses medication-related considerations and caveats. The next group of chapters covers CAM and preventive approaches to mitigating the effects of epilepsy and epilepsy therapies, such as drug toxicity and side effects of anti-epileptic drugs (AEDs), seizures, enhancing cognitive function, issues for women (pregnancy, breast feeding, menopause), and managing anxiety and depres-sion. The final part of the book focuses on quality of life and lifestyle modifications to reduce seizure risk, including techniques for stress reduction, sleep disturbances and aids to normalize, alcohol and recreational drugs, and environmental factors.June 2012 • 232 pp. • Softcover • ISBN 9781936287321 • E-book ISBN 9781617050664 • $49.00/$41.65

This full-color text and atlas is organized simply and logically with a core focus on major disease categories. The first of 14 chapters discusses the goals of the neuropathologic exam with representative pictures of the brainand spinal cord, and tables of histologic methods. The second chapter presents characteristic histologic reactionsof neural tissue to diseases and pathologic conditions endemic to the nervous system.
Additional chapters address major disease categories:
• Pathologic and clinical characteristics• Gross, histologic, and clinical features of individual diseases• Pathogenic mechanisms• Case studies• End-of-chapter review questions with answers
This complete and current review of the field is an invaluable reference for neurologists, pathologists, and residents.
About the AuthorCatherine Haberland, MD, is a Clinical Professor of Neurology, Pathology, and Psychiatry at the ChicagoMedical School at Rosalind Franklin University, and Attending Neurologist and Neuropathologist at theVeterans Administration Medical Center, North Chicago, Illinois.

From reviews of the first edition…“Its wider appeal for the novice or junior practicing electrophysiologist isits descriptions and diagrams detailing electrode positioning and examples oftypical, expected waveforms. [Keep] this book at the bedside, especially forsome of those more obscure cutaneous sensory nerves.”
—Journal of Neurology, Neurosurgery, and Psychiatry
This second edition continues to offer practitioners access to a huge databaseof normal values, developed using the most modern scientific protocols, forvirtually any patient population. Normal ranges take into account age, sex, height, and body mass to provide a complete set of reliable values for everydayclinical practice.
Benefits of this outstanding reference:• Covers the most up-to-date techniques for conducting nerve sensory tests• Codifies the acceptable differences in latency, amplitude, and nerve conduction
velocity between nerves of the same or opposite limbs, allowing for a moreprecise diagnosis
• Provides written descriptions and schematic depictions of electrode placement• Offers a concise list of references and suggested readings for each study• Includes advantages and pitfalls of alternative techniques
This superb reference can be used to quickly review a given study or to lookup reference values that have not been committed to memory. It providestrainees and clinicians with a reliable set of normal values that they can usewith confidence.
About the Authors: Ralph M. Buschbacher, M.D. and Nathan D. Prahlow, M.D.are both Professors in the Department of Medicine and Rehabilitation atIndiana University School of Medicine.

"Surprisingly, there has been no major monograph devoted to Parkinson's disease alone.Factor and Weiner fill this gap…. There are detailed chapters on individual symptoms, clinicalpharmacology, and other diseases that include prominent parkinsonian features…. Soundadvice is provided on quality of life, caregiving, and financial considerations.”—New England Journal of Medicine
“Drs. Factor and Weiner succeed in their goal of providing a ‘comprehensive…review on themultiple aspects of the disease and putting them into perspective’…. An informative read, with a very good take on the current management of Parkinson’s disease… it supplants theolder movement disorder texts as the one [to] reach for first when reviewing a Parkinson’s disease topic.”—Archives of Neurology
Parkinson’s disease is the second most common neurodegenerative disease. This completelyrevised and updated second edition of Parkinson’s Disease: Diagnosis and ClinicalManagement covers all important advances in our understanding of the etiology, pathogenesis, investigation, and management of Parkinson’s disease. It includes 23 new chapters, with updated information on genetics, pathology, biomarkers, pathogenesis, impulse control disorders in Parkinson’s disease, and key outcome measures.
Together, the chapters in this new edition form a comprehensive review of the many issues facing clinical neurologists, physiatrists, and PD specialists today. Lucid and easily readablefrom beginning to end, each chapter also stands on its own as a scholarly review of the individual subject. This encyclopedic contribution to the field is vital for all those who treat andcare for patients as well as for libraries and other professional collections.
