Case Management Fundamentals

WHAT IS CASE MANAGEMENT? Case Management is a youth-centered goal-oriented process for assessing the strengths and needs of an individual for participant services; developing a plan to:•Attain their goals •Assist him/her to obtain the determined services•Facilitating their use of services in obtaining their goals (education, goal-focused employment, i.e., long-term employment, etc)

KEY COMPONENTS –Thought out written process –Staff who “get it”–Organized case files –Services to offer –Strategy to discuss youth goals, progress, challenges and next steps
